Ability to learn inhaler technique in relation to cognitive scores and tests of praxis in old age.

Clinical observations have shown that some older patients are unable to learn to use a metered dose inhaler (MDI) despite having a normal abbreviated mental test (AMT) score, possibly because of dyspraxia or unrecognised cognitive impairment. Thirty inhaler-naive inpatients (age 76-94) with an AMT score of 8-10 (normal) were studied. Standard MDI training was given and the level of competence reached was scored (inhalation score). A separate observer performed the minimental test (MMT), Barthel index, geriatric depression score (GDS), ideational dyspraxia test (IDT), and ideomotor dyspraxia te

Anterior cruciate ligament injury. The competitive athlete.

The competitive elite athlete who sustains an anterior cruciate ligament (ACL) rupture has few options for treatment. If they wish to continue to compete at the preinjury level, then the only viable option is to undergo an ACL reconstruction. Otherwise, the athlete is at a substantial risk with a ACL deficient knee of sustaining subsequent degenerative changes in the knee at a young age. The athlete's presentation following injury includes reporting of a contact or noncontact acceleration or deceleration twisting injury to the knee, a 'pop' or the sensation of the joint 'coming apart', haemart

Psychotherapy and mental health teams.

Mental health teams in different configurations and settings are under increasing pressure to offer formal psychotherapies as well as psychologically informed management to large numbers of 'difficult' patients with severe and complex presentations. This pressure has arisen variously from consumers, governmental agencies and commissioning bodies. Although these teams are an important resource, they receive limited training, supervision or support in models of psychotherapy, especially those incorporating a relational dimension and offering a coherent 'common language'. This commonly results in

Ethical analysis of living organ donation.

In 2003, the first 3-way living kidney donor-swap was performed at Johns Hopkins Hospital in Baltimore, Md. Three new donor protocols includingpaired donation now allow unrelated individuals to serve as donors. Some ethicists have suggested that emotionally unrelated individuals not be permitted to donate because they will not experience the same satisfaction that a family member who is a donor experiences. Others who frame living donation as an autonomous choice do not see emotionally unrelated or even nondirected donation as ethically problematic. This article uses an ethical framework of pr

Emergency management of agitation in schizophrenia.

Schizophrenia is a common psychiatric condition, affecting approximately 1% of the population. Acute emergent presentations often include hallucinations, delusions, thought, and speech disorders. Agitation is common among emergency patients with schizophrenia. Decisional capacity should be assessed in all patients. Reversible causes of agitation should be ruled out, including infection, metabolic disorders, endocrine disorders, trauma, pain, noncompliance, toxicological disorders, and structural brain abnormalities. Agitation may be managed acutely using a combination of pharmacological agents

Ethical challenges in geriatrics and long-term care.

BACKGROUND: The Canadian population, like that of other western countries, is aging. OBJECTIVE: To review ethical issues related to clinical practice, research, and resource allocation in the geriatric and long-term care population. METHODS: Through the use of case histories and relevant literature review, we explore issues in geriatrics and long-term care. CONCLUSIONS: It is incumbent on health-care providers involved in geriatrics and long-term care to be sensitive to the ethical issues that surround decision-making in the later years of life. It is through this process that we can be hum

Tobacco and the institutionalized mentally retarded: usage choices and ethical considerations.

Patterns of tobacco use were observed among 749 people diagnosed with mental retardation residing in a state-operated facility. Specifically, individual preference for tobacco products and frequency of use were documented. Subjects were observed using several types of tobacco products: cigarettes, cigars, chewing tobacco, snuff, and cigarette butts. Approximately 7% (n = 52) used at least one form of tobacco; ten individuals used more than one tobacco product. Interestingly, 20.5% of the individuals diagnosed with mild or moderate mental retardation (n = 122) consumed tobacco products, a patte

Functional connectivity signatures of trauma-specific reflective functioning: The interplay between childhood trauma and maternal competence.

BACKGROUND: Childhood maltreatment yields enduring neurobiological consequences that impact later parenting, with trauma-specific reflective functioning (RF-T) serving as a critical resilience mechanism. OBJECTIVE: This study investigated the psychological and electrophysiological interplay between childhood trauma, RF-T, and parenting sense of competence (PSOC). METHODS: Sixty-six mothers with a history of childhood maltreatment underwent EEG recording while viewing Indonesian children's facial expressions (happy, fearful, and angry). RF-T was assessed using the Failure to Mentalize Trauma

Shared Experience Versus Professional Expertise: Preferences for Features of Informal and Formal Mental Health Support Among College Students.

Informal caregiving serves vital functions in the USA, filling important gaps in formal care systems. However, informal caregiving for psychological disorders has received relatively limited research attention. In this study, we evaluated perspectives towards informal and formal caregiving in a sample of 554 college students attending a large public university in the Deep South and contrasted the relative importance of care elements in the context of informal and formal care. Results from evaluation of preferred elements of care between informal care and formal care were consistent with person

Physical, mental and behavioral health indicators in relation to academic performance in European boys and girls: the I.Family study.

BACKGROUND: Academic performance in children is associated with a range of health-related factors, including physical fitness, mental well-being, sleep, and behavioral patterns. While previous studies have examined these factors individually, fewer have assessed their independent associations with academic achievement while accounting for other relevant health indicators. This study uses data from the I.Family study to explore how physical, mental, sleep-related, and behavioral health indicators relate to academic achievement among European adolescents, considering each factor's contribution w

Digitalization in Dentistry: Dentists' Perceptions of Digital Stressors and Resources and Their Association with Digital Stress in Germany-A Qualitative Study.

Background: The digital transformation in dentistry is increasingly reshaping treatment procedures, offering new opportunities and advancements. While digitalization promises enhanced efficiency and quality of care through the standardization, acceleration, and simplification of workflows, it also introduces challenges related to mental health. Studies investigating digitization-associated stressors and resources, as well as health- and work-related outcomes, in the dental sector are still rare. In the context of ongoing digitalization, further studies are needed to examine the need for and th

Patient perspectives on the Tailored intervention for Anxiety and Depression Management in COPD (TANDEM): a qualitative evaluation.

BACKGROUND: Chronic obstructive pulmonary disease (COPD) is commonly associated with anxiety/depression which can affect self-management and quality of life. The TANDEM trial evaluated a cognitive behavioural approach intervention targeting COPD-related symptoms of anxiety and/or depression, comprising up to eight one-to-one sessions delivered by respiratory healthcare professionals prior to pulmonary rehabilitation (PR). The intervention showed no improvement in anxiety/depression or uptake/completion of PR. We present patient perspectives of the intervention to help understand these results.

Gender, racial-ethnic, and socioeconomic disparities in the development of social-emotional competence among elementary school students.

Social-emotional competence (SEC) has been demonstrated to be a crucial factor for student mental health and is malleable through the high-quality implementation of effective school-based social and emotional learning (SEL) programs. SEL is now widely practiced in the United States as a Tier 1 strategy for the entire student body, yet it remains unclear whether disparities exist in the development of SEC across socio-culturally classified subgroups of students. Also, despite the field's widespread concern about teacher bias in assessing SEC within diverse student bodies, little evidence is ava

Post-incident responses (debriefing) after restrictive interventions in mental health inpatient settings: A realist synthesis.

BACKGROUND: Restrictive interventions (restraint, seclusion and rapid tranquilisation) are used widely in adult mental health inpatient settings despite national and international priorities to reduce them. Post-incident responses (sometimes referred to as debriefing), the immediate support and subsequent reflection and learning that should follow restrictive interventions, are recommended by the National Institute for Health and Care Excellence and embedded in restraint reduction frameworks, but their required components and mechanisms of effect remain poorly specified. AIM: To explain how p
