Unusual skin mass (primary cutaneous mucinous carcinoma).

Primary mucinous carcinoma of the skin is a rare malignant tumour of sweat gland origin. Diagnostic concerns include its deceptively benign appearance in some cases and the difficulty in differentiating it from secondary mucinous carcinoma of skin metastasising from a primary source elsewhere. A case of a 75-year-old man is reported who presented with a slowly growing painless mass near the lateral canthus of the right eye for about 2 years. Clinically, a diagnosis of basal cell carcinoma was made whereas histopathology revealed mucinous carcinoma of the skin. The primary source of the tumour

Gorlin Syndrome Presentation and the Importance of Differential Diagnosis of Skin Cancer: A Case Report.

In a busy community practice, clinical skin findings can often be misinterpreted. Skin cancers can sometimes mimic rashes like psoriasis, eczema or prurigo nodularis in both appearance and symptoms. Gorlin syndrome is one such genetic syndrome, characterized by the eruption of multiple and early onset basal cell carcinomas (BCCs), which can be mistaken for a rash. We describe a 68-year-old female who presented to the dermatology office with a previous history of over 30 BCCs that had been previously biopsied and/or surgically removed. However, the patient had been lost to follow up for several

A Case of Infundibulocystic Basal Cell Carcinoma Clinically Mimicking a Melanocytic Nevus Associated with Epidermal Cysts.

A 52-year-old Japanese woman presented with a 1.5-cm black, glossy, flat, pediculated lump that clinically mimicked a melanocytic nevus on the left temporal side of her head. The subcutaneous tumor beneath the nodule was elastic and hard. A histological examination showed that the tumor was well circumscribed with an exo- and endophytic growth 2.4 × 1.9 cm in size. The lesion contained several keratinous cysts and was composed of funicular fascicles containing squamoid cells. Excessive mucinous material deposition was observed around the tumor periphery and a palisading arrangement of nuclei i

Muckle-Wells syndrome in the setting of basal cell nevus syndrome.

Muckle-Wells syndrome (MWS) is a rare disorder inherited in an autosomal-dominant fashion that belongs to a group of hereditary periodic fever syndromes. It specifically belongs to the cryopyrin-associated periodic syndromes (CAPSs) in which there is a mutation in the NLRP3 (NLR family pyrin domain containing 3) gene that leads to overproduction of IL-1β, the source of the multisystem inflammatory symptoms. Muckle-Wells syndrome is characterized by a recurrent urticarial eruption that is associated with episodic fever, myalgia, arthralgia, malaise, progressive sensorineural hearing loss, and a

Use of anteromedial thigh perforator flap and immunological implications of Gorlin-Goltz syndrome: a case study.

Gorlin-Goltz syndrome is mainly characterised by the development of numerous multicentric and relapsing cutaneous basal cell carcinomas (BCCs). A major problem for patients with Gorlin-Goltz syndrome is the large amount of BCCs that can invade the deep underlying structures, especially the face. Here, we describe the case of a 23-year-old male affected by Gorlin-Goltz syndrome. He had recurrent BCCs on a hairless scalp and dorsum since he was 17 years old and underwent four surgical procedures to excise BCCs, including a reconstruction with anteromedial thigh perforator flap. For each of the s

Syndrome in question: Gorlin-Goltz syndrome.

The Nevoid Basal Cell Carcinoma Syndrome (NBCCS) is an uncommon disorder caused by a mutation in Patched, tumor suppressor gene. It is mainly characterized by numerous early onset basal cell carcinomas, odontogenic cysts of jaw and skeletal abnormalities. Due to the wide clinical spectrum, treatment and management of its modalities are not standardized and should be individualized and monitored by a multidisciplinary team. We report a typical case in a 30-year-old man with multiple basal cell carcinomas, keratotic pits of palmar creases and bifid ribs, with a history of several corrective surg

Metastatic basal cell carcinoma: a frequent disease with rare clinical evolution.

Basal cell carcinoma (BCC) is the most common malignancy worldwide. BCC has a benign evaluation, despite the high rate of local recurrence. Conversely, metastatic BCC (mBCC) is extremely rare. In this article, we present a case of an 86-year-old woman with a history of forearm mBCC, excised in 2009. In 2011, she experienced a left axilla enlargement and the evaluation revealed metastasis of mBCC. The patient began chemotherapy but had regional progression. After she had undergone radiotherapy, initiation of a clinical phase II trial was proposed, which she refused. The patient died in March 20

Reflectance Confocal Microscopy Features of Focal Dermal Mucinosis Differ from Those Described for Basal Cell Carcinoma: Report of Two Cases.

The purpose of this study was to describe the reflectance confocal microscopy (RCM) features of focal dermal mucinosis (FDM). The entity clinically and dermatoscopically mimics other diagnostic entities, most notably nonpigmented basal cell carcinoma. We describe two cases that highlight the dermatoscopic, RCM and histopathological attributes of FDM. RCM features such as dermal foci of dense collagen bundles oriented in the same direction, foci of haphazardly oriented thin collagen fibers separated by dark structureless areas and the absence of dark silhouettes and tumor islands are clues for

Optical coherence tomography and its role for delineating the thickness of keratinocyte dysplasia and neoplasia.

Optical coherence tomography (OCT) can produce cross-sectional and en face, non-invasive, real-time images of skin. OCT produces high-resolution images at a micrometre resolution and has a maximum 2-mm penetration depth, which places OCT in the imaging gap between ultrasound and confocal microscopy. Much OCT research has been performed on keratinocyte dysplasia and neoplasia, primarily including basal cell carcinoma (BCC) and actinic keratosis. In regards to BCC and actinic keratosis, architectural disarray of the epidermis is an overall characteristic finding in OCT images. OCT can reliably d

Nevoid basal cell carcinoma syndrome with a unilateral giant ovarian fibroma in a Japanese 6-year-old girl.

Nevoid basal cell carcinoma syndrome (NBCCS) is characterized by basal cell carcinoma, skeletal abnormalities, benign tumors including ovarian fibroma, and various other phenotypic expressions. Ovarian fibromas in NBCCS before puberty are very rare. We report a 6-year-old prepubescent girl with NBCCS showing skeletal abnormalities, medulloblastoma, and ovarian fibromas. The patient was referred to our hospital owing to abdominal distension. On admission, a huge elastic hard tumor was palpable and computed tomography showed a huge tumor of the left ovary. We performed a left salpingo-oophorecto

The use of a custom earmold to prevent recurrent external auditory canal stenosis.

The treatment of external auditory canal (EAC) stenoses often requires the prolonged use of a stent or splint. Traditional stents often occlude the EAC, resulting in a temporary conductive hearing loss. This case report describes a custom canal earmold with a large bore used as a stent in a patient with EAC stenosis. The customized earmold stent successfully prevented restenosis, while the large bore provided ventilation and improved hearing subjectively during the stenting phase.

Multiple basal cell carcinomas in xeroderma pigmentosum treated with imiquimod 5% cream.

We report successful treatment of multiple basal cell carcinomas with imiquimod 5% cream in a 16-year-old boy with xeroderma pigmentosum and review the possibility of prophylactic role of imiquimod in the disease. Imiquimod cream was applied uniformly over all the basal cell carcinoma lesions and background pigmented skin, once at bedtime on every alternate day for 12 weeks. Besides the basal cell carcinomas, the background hyperpigmentation and keratotic papules also cleared, and the skin texture improved. The lesions did not recur at the treated sites during the follow up of 1 year.

Bazex-Dupré-Christol syndrome: an ectodermal dysplasia with skin appendage neoplasms.

Bazex-Dupré-Christol syndrome is a rare X-linked genodermatosis characterized by early-onset nonmelanoma skin cancers, atrophoderma follicularis, hypotrichosis, hypohidrosis, and multiple milia. Its molecular basis remains unknown and nosologic classification is debated. We report a 5-year-old child presenting sparse hair, reduced sweating, ice-pick skin depressions of the dorsum of hands, facial and limb milia, perianal skin hyperpigmentation, and hyperpigmented papules of the axillae and neck. His mother showed similar features but lacked hair involvement. Histologic examination of a skin pa

Ex vivo dermoscopy of melanocytic tumors: time for dermatopathologists to learn dermoscopy.

BACKGROUND: We have identified cases of skin cancer with discordances between clinical, dermoscopic, and histopathologic findings that were likely due to sampling errors in the pathology laboratory. This has prompted us to explore the use of ex vivo dermoscopy as an ancillary method of gross pathology, which may serve to guide tissue sectioning. Noncontact polarized dermoscopy was applied to pigmented lesions before excision and at least 6 hours after specimen fixation in formalin. OBSERVATIONS: The orientation of the lesion, overall dermoscopic pattern, and dermoscopic pigmented structures (

Gorlin's syndrome presenting with myolipoma of tongue base.

OBJECTIVE: We report the first case of tongue base myolipoma associated with Gorlin's syndrome. METHOD: Case report and review of world literature. RESULTS: A 39-year-old man with known Gorlin's syndrome presented with progressive dysphagia. Subsequent magnetic resonance imaging scan and biopsy confirmed the rare diagnosis of myolipoma arising from the tongue base. In view of the benign nature of this mass, it was debulked rather than completely excised in order to preserve swallowing function. CONCLUSION: To our knowledge, this is the first report of the coexistence of two rare conditions
