[The combination of eye muscle surgery with other interventions on or in the eye].

Strabismus and motility disorders of the eye can be linked in many different ways to intraocular diseases and disorders of the lid and orbit. Different functional aspects have to be considered before performing combined eye muscle and intraocular or adnexal surgery. This allows to avoid unnecessary operations, to choose the right dosage of the operation, to limit the risk of diplopia, and finally to achieve a satisfying result for the patient.

Progressive ataxia and palatal tremor (PAPT): clinical and MRI assessment with review of palatal tremors.

Palatal tremor has been subdivided into essential (EPT) and symptomatic palatal tremor (SPT). A subgroup of the SPT form has a syndrome of progressive ataxia and palatal tremor (PAPT). Published details of cases of PAPT are sparse and the disorder appears heterogeneous. We present clinical and MRI features of six patients with sporadic PAPT who attended The University Health Network between 1991 and 2002. Eye movements were recorded using a magnetic search coil technique. We review previously reported cases of PAPT from the English language literature and relate this disorder to EPT and SPT. P

Positive polymerase chain reaction and histology with borderline serology in Parinaud's oculoglandular syndrome.

PURPOSE: To report a case of Parinaud's oculoglandular syndrome (POS) in which, despite a borderline serology, polymerase chain reaction (PCR) testing for a conjunctival biopsy was positive for Bartonella henselae, a source of cat-scratch disease. A Steiner silver stain demonstrated the organism. METHODS: Case Report. RESULTS: A 65-year-old man was seen for a foreign body in his left eye (OS) associated with chemosis and a preauricular node. CONCLUSION: B. henselae is a known cause of POS. This gram-negative pleomorphic rod has been more frequently discovered in connection with this syndrom

[Ocular hydrodynamics in patients with occluding and stenosing disease of main cerebral vessels after reconstructive surgery of internal carotid and vertebral arteries].

Ocular hydrodynamics was investigated using the tonography method in 58 patients, to whom reconstructive operations on the affected vessels were performed. After reconstructive operation on vertebral arteries ocular hydrodynamics didn't change significantly, on a carotis interna (ACI)--the positive effect was revealed on the side of the operation performed, and if the cerebrum arterial circle function was preserved--on the contralateral side as well. If extra- and intracranial parts of ACI were affected after conduction of reconstructive operation and restoration of the artery's passability oc

Bifid epiglottis associated with Joubert's syndrome.

This report presents the case of a 2-year-old boy who had a bifid epiglottis associated with Joubert's syndrome. He had episodic tachypnea and apnea, inspiratory stridor, aspiration, and growth and mental retardation. Direct laryngoscopy demonstrated agenesis of the right half of the epiglottis and hypertrophied mucosa over the arytenoid cartilage. After the hypertrophied mucosa was partially vaporized with a CO2 laser, the inspiratory stridor soon improved and the aspiration was alleviated. Brain magnetic resonance imaging showed cerebellar vermian agenesis and enlargement of the fourth ventr

[Periodic decerebrate spasm with ocular dipping, Cheyne-Stokes respiration and hypersympathetic activity].

Decerebrate spasm is a generalized muscular spasm produced by some stimuli on decerebrate posture. Such spasm are called "tonic fit" or "decerebrate extensor spasm". We reported a 50-year-old man with periodic decerebrate spasm after cerebral hemorrhage. On admission, the patient was comatose. The pupils were round but anisocoric and did not react to light. Corneal reflexes were absent. The face, arms, and legs did not move voluntarily. Two weeks after admission, he was found in decerebrate rigidity. Periodic decerebrate spasms were also observed and were accompanied by ocular dipping. Cheyne-

Monocular elevation paresis caused by an oculomotor fascicular impairment.

A 54-year-old man developed an acquired monocular elevation paresis. Forced lid closure or oculocephalic reflexes failed to elevate the affected eye in contrast to the contralateral eye, which turned fully upward using either maneuver. A mass lesion compressing the lateral aspect of the right oculomotor nerve at the exit of the brainstem was found. The monocular elevation paresis in this patient reflects damage of the oculomotor nerve fascicles that supply the inferior oblique and superior rectus muscles. This case supports the accepted topographical fascicular arrangement of the oculomotor ne

Visual-evoked cortical potentials in dissociated vertical deviation.

It is debatable whether chiasmatic misrouting of temporal optic-nerve fibers (similar to that found in ocular albinism) is also characteristic of dissociated vertical deviation. Pattern appearance, pattern reversal, and diffuse-flash, monocular full-field visual-evoked cortical potentials were recorded from albino and normal human subjects and subjects with dissociated vertical deviation. Pattern appearance was the most reliable stimulus for evaluating lateralization (albino-type misrouting) in adult albino patients, and diffuse-flash stimulation was almost as reliable in children. Pattern rev

Midfacial fractures and the eye: the development of a system for detecting patients at risk of eye injury--a prospective evaluation.

Midfacial trauma is often complicated by ocular disorder. A scoring system has been devised to help the maxillofacial surgeon identify patients who warrant referral to an ophthalmologist. A prospective pilot study was carried out on 100 patients with midfacial fractures to evaluate the effectiveness of this system in clinical practice. The sensitivity value was 94.4% and the specificity value was 89%. Only 1 patient, who clinically warranted referral to an ophthalmologist, was missed by the system whilst 9 others were incorrectly classified as warranting referral. The results of this evaluatio

Intracranial germinoma: report of seven cases.

Seven children with histologically verified intracranial germinomas, treated between 1983-1987, are reviewed. The four boys were 12, 14, 15, and 16 years old; two girls were 9, and one girl was 10 years old. Four tumors were suprasellar, and three were in the pineal region. Parinaud's sign was the first symptom in the pineal tumors and diabetes insipidus in the suprasellar ones. The latter four patients also had pituitary deficiencies. The tumors were totally removed in three patients, partially in three, and one patient had biopsy only. Two patients had disseminated tumor. All patients receiv

Strabismus surgery outcomes for internuclear ophthalmoplegia with exotropia.

Exotropia (XT) in internuclear ophthalmoplegia (INO) is a difficult problem to treat. The purpose of this study is to describe surgical approaches in treating INO and glean insight into various pre-operative factors that may affect outcomes for XT in INO. We conducted a retrospective review from 1 January 1995 - 5 January 2021 and identified seven patients with INO who underwent strabismus surgery for XT. Patient age, sex, etiology of INO, pre-operative alignment and sensorimotor exam, presence of diplopia, surgery performed, subsequent surgeries, use of adjustable sutures, post-operative alig

Eye-tracking-aided characterization of saccades and antisaccades in SYNE1 ataxia patients: a pilot study.

BACKGROUND: SYNE1 ataxia is an autosomal recessive hereditary condition, the main characteristic features of which are gait and limb ataxia and cerebellar dysarthria. Reports have revealed that the clinical phenotype of SYNE1 ataxia is more complex than the first published cases with pure cerebellar signs indicated. The aim of this study was to characterize eye movement alterations in the first diagnosed Hungarian SYNE1 ataxia patients. RESULTS: Saccades and antisaccades were examined with an eye tracker device in 3 SYNE1 (one patient has two frameshift mutations [c.8515_8516insA, p.Met2839As
