Evaluation of assistive devices after a course in joint protection.

This study assessed a joint protection education program and investigated the costs and effects of assistive devices. Fifty-three women with seropositive rheumatoid arthritis, whose ages ranged from 29 to 65 years, attended a standardized joint protection course (13 hours). A self-report questionnaire was constructed listing the most common devices, and the subjects were asked to check which devices they had received, which ones they used and did not use, and why. Pain during activities of daily living and when using assistive devices was measured using the visual analogue scale. Results show

Ultrasound and the Pavlik harness in CDH.

Sixty-nine hips in 62 patients were treated by the Pavlik harness for congenital dislocation. Ultrasonography showed three degrees of residual head displacement when the harness was first applied. In type A, the femoral head showed contact with the inner posterior wall of the acetabulum. In type B, it contacted the posterior margin of the socket, with its centre at or anterior to the acetabular rim. In type C, the femoral head was displaced outside the socket, with its centre posterior to the acetabular rim. All 51 hips with type A displacement remained reduced. Of nine hips of type B, five we

[Rehabilitation of paraplegic patients].

Comprehensive management and care of spinal-cord-lesioned patients start at the site of injury or at the moment of the onset of paralysis, depend with its consequences on the treatment in the acute phase, pass to the reactivation and rehabilitation phase and continue throughout all life of such patients. Considering that even a complete transverse lesion is only an instable, temporarily balanced situation, every small complication can destroy this balance and may lead from total independence to total dependence. The Vancouver microstomia orthosis.

Eighteen microstomia orthoses are compared with respect to their characteristics, use, and limitations. A new prosthesis, the Vancouver microstomia orthosis, was designed to incorporate the advantages of many of the orthoses and eliminate some drawbacks. Ten adults with microstomia secondary to second- or third-degree circumoral flame burns were fitted with the prosthesis and observed over a 12-month period. In 9 weeks or less, microstomia was corrected with an average gain of 7 mm in the horizontal and 13 mm in the vertical active range of motion. The measures before and after this treatment

Limb salvage in infected lower extremity gangrene.

Four diabetic patients with gangrene of the forefoot and infection ascending above the ankle were treated with open amputation of the foot combined with open fasciotomy and debridement of the involved proximal muscle compartments. All four patients healed their wounds and returned to their premorbid community ambulation status. The management and indications in these unusual patients are discussed.

Current trends in the treatment of congenital dislocation of the hip.

The best results in CDH are after early diagnosis and treatment. More carefully taught examination techniques of the newborn are important. With the addition of ultrasound examination of the hip, the number of children with CDH diagnosed in the newborn period should increase. The Pavlik harness has had increased use in the child up to 6 months of age. It allows a simple reduction technique for approximately 90% of these children. If skillfully used, the risk of avascular necrosis can approach zero. The early gentle reduction will improve the results of treatment in avoiding avascular necrosis.

Transfer of Soleus Muscular Branch of Tibial Nerve to Deep Fibular Nerve to Repair Foot Drop After Common Peroneal Nerve Injury: A Retrospective Study.

OBJECTIVE: Common peroneal nerve (CPN) injury that leads to foot drop is difficult to manage and treat. We present a new strategy for management of foot drop after CPN injury. The soleus muscular branch of the tibial nerve is directly transferred to the deep fibular nerve, providing partial restoration of motor function. METHODS: We retrospectively reviewed eight patients treated for CPN injury between 2017 and 2019. The soleus muscular branch of the tibial nerve was transferred to the deep fibular nerve to repair foot drop. Electrophysiology was conducted, and motor function was assessed. Mo

Patient Response to an Integrated Orthotic and Rehabilitation Initiative for Traumatic Injuries: The PRIORITI-MTF Study.

Although limb salvage is now possible for many high-energy open fractures and crush injuries to the distal tibia, ankle, hindfoot, and midfoot, orthotic options are limited. The Intrepid Dynamic Exoskeletal Orthosis (IDEO) is a custom, energy-storing carbon fiber orthosis developed for trauma patients undergoing limb salvage. The IDEO differs from other orthoses in that it allows patients with ankle weakness to have more normal ankle biomechanics and increased ankle power. This article describes the design of a study to evaluate the effectiveness of the IDEO when delivered together with a high

Pathological Fracture of Calcaneum: A Case Report.

INTRODUCTION: Only a few reports have described calcaneus fractures after total knee arthroplasty (TKA). Therefore, in this report, we describe a case of calcaneal avulsion fracture that occurred 5 weeks after a TKA in a relatively young male patient with syphilis. CASE REPORT: A 63-year-old man with syphilis had Charcot joint of the right knee. The patient developed severe varus deformity and contracture and experienced severe pain in the knee. TKA was performed to alleviate the pain and improve the patient's gait. The patient noticed slight heel pain 4 weeks after the TKA, and a calcaneus a

Endoscopy-assisted early correction of single-suture metopic craniosynostosis: a 19-year experience.

In BriefThe long-term results of treating infants with metopic craniosynostosis by using endoscopic, minimally invasive techniques are reported. The impetus arose from the lack of consistent and favorable outcomes associated with calvarial vault remodeling techniques and from the very traumatic and invasive nature of these procedures. The results presented show excellent and consistent long-term outcomes that are superior to traditional methods and are associated with minimal trauma, blood loss, and anesthetic exposure, and with short surgical times.

Has the Threshold for Epiphysiodesis Versus Lengthening Changed in the Era of Magnetically Controlled Nails?

INTRODUCTION: Limb length discrepancy (LLD) is common in both the pediatric and adult population. Length inequalities can be due to a multitude of etiologies including congenital, developmental, and acquired causes. There has been little consensus on the morbidity of LLD and, as a result, the threshold necessary for treatment of LLD to prevent morbidity. Advances in magnetically controlled lengthening devices achieve greater accuracy and patient satisfaction and create an opportunity to lower the threshold for limb lengthening. DISCUSSION: Asymptomatic LLD is relatively common in both pediatr

Retention of the Structure and Function of Heparan Sulfate Biomaterials After Gamma Irradiation.

Heparan sulfate (HS) is a highly heterogeneous polysaccharide implicated in many important biological processes. Our previous work has demonstrated that a particular affinity-selected HS (referred to henceforth as "HS3") is capable of enhancing the osteogenic effects of bone morphogenetic protein 2 (BMP2). Here, we gamma-irradiated HS with 26 kGy of ionizing radiation to determine how this affected the structure, composition, and function. Initial structural studies were performed on a commercial preparation of HS as a proof-of-concept. Gamma irradiation of this HS preparation did not signific

Assessment of the Effect of Improved Medial Linkage Reciprocating Orthosis Compared to Isocentric Orthosis on the Motor Function in Spinal Cord Injury Patients.

BACKGROUND: Spinal cord injuries (SCI) result in profound mobility impairments, particularly the inability to walk. Traditional orthotic devices, while aiding mobility, often lead to high energy expenditure and discomfort. This study aimed to develop a novel orthosis that enhances comfort, improves movement parameters, and reduces energy consumption by incorporating medial linkage joints and reciprocal mechanisms into the existing Medial Linkage Reciprocating Gait Orthosis (MLRGO). The effectiveness of this new device was evaluated against both the Isocentric Reciprocating Gait Orthosis (IRGO)…

Decoding hand and wrist movement intention from chronic stroke survivors with hemiparesis using a user-friendly, wearable EMG-based neural interface.

OBJECTIVE: Seventy-five percent of stroke survivors, caregivers, and health care professionals (HCP) believe current therapy practices are insufficient, specifically calling out the upper extremity as an area where innovation is needed to develop highly usable prosthetics/orthotics for the stroke population. A promising method for controlling upper extremity technologies is to infer movement intention non-invasively from surface electromyography (EMG). However, existing technologies are often limited to research settings and struggle to meet user needs. APPROACH: To address these limitations,
