PURPOSE: The purpose of this study was to create symptom indexes, that is scores derived from questionnaires to accurately and efficiently measure symptoms of interstitial cystitis/bladder pain syndrome and chronic prostatitis/chronic pelvic pain syndrome, collectively referred to as urological chronic pelvic pain syndromes. We created these indexes empirically by investigating the structure of symptoms using exploratory factor analysis.

Gout is an increasingly common metabolic disorder worldwide. Classical presentation is with acute attacks of arthritis affecting the first metatarsophalangeal joint. With disease progression, tophi may also appear. We present an unusual case of nasal gout in a 55-year-old man who was referred to the Ear, Nose and Throat department with irregularity over the nasal bones and episodic pain. We discuss the work up, diagnosis and management of this case and review the limited literature on this topic.

A 20-year-old male collegiate soccer goalkeeper presented to an athletic trainer during the season complaining of right (dominant kicking leg) groin pain. The athletic trainer identified a mass and hematoma and suspected myositis ossificans. The patient was referred to the team physician, who ordered radiographs and magnetic resonance imaging to confirm the diagnosis and rule out other injuries. Dysmenorrhea affects quality of life, but is often inadequately treated in teenagers. The mainstay of treatment is NSAID, which must be started pre-emptively and in adequate dose. If NSAID provides insufficient pain relief, or when contraception is required, combined oral contraceptives can be prescribed concomitantly. Hormonal IUD can also be used in teenagers. If dysmenorrhea persists despite appropriate treatment, the patient must be referred to a gynecologist. Endometriosis, the most common cause of severe dysmenorrhea, can manifest already during adolescence.

A 68-year-old woman with a history of multifocal uterine leiomyomas presented with left groin pain and was referred for cross-sectional imaging to assess for the presence of an inguinal hernia. In this patient, MRI demonstrated a round ligament leiomyoma encased in the proximal left inguinal canal. Leiomyomas are the most common benign gynecologic tumors, however round ligament leiomyomas are very rare. The purpose of this case report is to highlight a rare manifestation of a common entity.

We report a classical presentation of trichobezoar in an 8 year old girl who was referred for consultation querying a probable abdominal neoplasm in the left upper quadrant. A large mobile mass was palpable in the left upper quadrant extending toward the umbilicus. Of note, she also had symptoms of recurrent abdominal pain, nausea, vomiting and halitosis. Although a rare occurrence, trichobezoar must be considered as a differential diagnosis in a patient who had a history of pica, presenting with an abdominal mass.

Hip dysplasia is a common congenital disorder, and identified as a source of pain, functional impairment and increased risk for development of osteoarthritis. Early diagnosis and reference is essential in symptomatic hip dysplasia, since patients may be candidates for joint preserving surgery. Assessment concentrates on history, physical examination and plain radiography. If radiography confirms the diagnosis (CE-angle < 25 degrees), the patient should be referred for expert evaluation. The assessment of hip dysplasia should be well-known to GPs, orthopaedics and radiologists.

A 7-year-old boy was referred to us with a 3-month history of swelling and pain on his left ankle. On laboratory evaluation, he only had an elevated erythrocyte sedimentation rate. X-ray showed an extensive lytic lesion of the talus bone. Chest roentgenogram was normal. The Mantuex skin test was negative. Histological examination of the biopsy tissue showed granuloma and caseating necrosis of tuberculosis. Talectomy with 6 months of antituberculous chemotheraphy resulted in a good clinical outcome.

Familial Mediterranean fever (FMF) is a genetic disorder characterized by acute episodes of fever with some combination of severe abdominal pain, pleurisy, arthritis, and skin rash. The case of a patient with recurrent urticaria referred for study of drug allergy is presented. After allergy had been ruled out, the urticaria was attributed to previously undiagnosed symptoms of an underlying systemic disease: FME. Urticaria is the least frequent cutaneous manifestation of this disease, and genetic analysis was required to confirm the diagnosis.

Otalgia is a common symptom in the pediatric age group. The most common cause is acute otitis media, followed by otitis externa. In the majority of cases, the diagnosis can be established by a thorough history and a careful otologic examination. When the otologic findings are normal and the etiology is obscure, a thorough work-up to determine the source of referred pain is essential. Laboratory investigations are usually not necessary. Treatment should always be directed at the underlying cause.

OBJECTIVE: Central sensitization (CS) with low peripheral pain thresholds (PPTs) is a common finding among patients with chronic pain after whiplash (CPWI). While it has been proposed that myofascial myofascial trigger points (MTrPs) may act as modulators of central sensitization, previously reported findings are conflicting and inconclusive. The present study was designed to investigate immediate responsiveness of CS to alterations in nociceptive input.
