Physical examination of the carpal bones by orthopaedic and accident and emergency surgeons.

Our aim was to test knowledge of carpal bone surface anatomy among orthopaedic and accident and emergency surgeons. A survey of 58 surgeons was conducted. Each was asked to palpate seven points on five commonly injured carpal bones. Eight surgeons declined to participate. Five of the remaining 50 correctly palpated all surface markings. Six failed to accurately palpate even a single point. The scaphoid waist and trapezial ridge were accurately palpated by 47 surgeons. The median score for all participants was 3 correctly identified points. The majority of orthopaedic and accident and emergency

The arterial anatomy of the human carpus. Part II: The intraosseous vascularity.

The intraosseous vascular anatomy of the carpal bones of 25 fresh cadaver limbs was studied by injection and Spalteholz clearing techniques. The carpal bones were classified into three general groups based on the size and location of nutrient vessels, the presence or absence of intraosseous anastomoses, and the dependence of large areas of bone on a single intraosseous vessel. Group I included the scaphoid, capitate, and 20% of the lunates in this series. Each had large areas of bone dependent on a single intraosseous vessel and was considered at greater risk to develop avascular necrosis foll

The distribution of bone islands and juxta-articular bone cysts in the growing hand.

Bone islands and juxta-articular bone cysts are relatively common incidental findings when X-rays are taken for other purposes. We have identified that the incidence of bone islands in the hands of asymptomatic children between the ages of 5 and 13 years is 3.8% and the incidence of cysts in the same population is 2.8%. Bone islands were most common in the scaphoid, whilst juxta-articular bone cysts were most common in the capitate. The age at which they appear has not been reported previously. This study first identifies their presence in the hands of children of age 10 years and 2 months (bo

Mid-term Outcome of the All-Dorsal Augmented Scapholunate Ligament Reconstruction for Chronic Scapholunate Instability.

BACKGROUND: Chronic scapholunate instability can be treated with the all-dorsal scapholunate augmented reconstruction This study examined the mid-term outcome of a multicentre series of this technique. METHODS: Patients with chronic symptomatic scapholunate instability that underwent surgical treatment were included. Patients with wrist arthritis were excluded. Assessment consisted of clinical (range of motion, grip strength, pain), functional (Disabilities of the Arm, Shoulder and Hand [DASH] score; Patient-Related Wrist Evaluation [PRWE] score and patient satisfaction), and radiographic mea

[The scaphocapitate fracture syndrome (Fenton)].

The scaphocapitate fracture syndrome as a greater arc injury is a special form of the perilunate fracture dislocation. It is a combination of fractures of the scaphoid and capitate bones. This severe injury of the carpus is rare and therefore difficult to diagnose. We report the case of a 22-year-old handball player, who fell on the outstretched hand with the wrist in extension. Open reduction was performed via a dorsal approach and both fractures were treated operatively with Herbert screws. After three months the patient was able to play handball again with a good functional result.

[Cinematography, a new diagnostic procedure in evaluation of the injured painful wrist joint].

By the X-ray Cineradiografie we are able to examine and to judge the dynamic of the wrist bones by 50 pictures/sec. in comparison to one another and also depending on their ligaments. We did an investigation of 170 patients with painful wrist. With the method we were able to make up a clear diagnosis and to propose the therapy. I.e.: If consecutive shortening of the radius after distal radius fracture resulting ingruency of the wrist joint is relevant, or a scaphoid pseudarthrosis is fixed elastically, or a scaphoic dissociation is effective. The variations were shown in comparison to normal c

[Clinical features and X-ray manifestations of mucopolysaccharidosis-II-B].

Mucopolysaccharidosis-II (MPS-II) and MPS-I are both characterized by gargoylist dwarfism. MPS-II is of X-linked recessive heredity. Five cases of MPS-II-B in our series were followed up for ten years. Corneal opacity and dysmnesia were not found in all of them. Although MPS-II possessed many radiologic similarities to MPS-I, the former presented certain characteristic features such as butterfish-like vertebra, and small triangular malformation of carpal bones and tarsal scaphoid etc.

[Carpal instabilities].

The ligaments of the wrist and the bony configuration--beside muscles and tendons--are responsible for stability in the wrist joint, which is impaired in certain pathological conditions: unstable fractures of the scaphoid, scapholunate dissociations, perilunate fracture-dislocations, malunited Colles' fractures, Kienböcks disease, rheumatoid arthritis and posttraumatic laxity of ligaments. The differentiation between dorsal and palmar instability is made by radiological examination and measuring of the scapholunate, capitolunate and radiolunate angles. The understanding of normal anatomy and (

Advances in wrist arthroscopy.

Since its introduction more than three decades ago, wrist arthroscopy has continually evolved. The procedure has a wide list of indications, including diagnostic and management applications. The scope of practice for the wrist arthroscopic surgeon includes management of triangulofibrocartilage complex pathology, evaluation and management of carpal instability, assistance in fracture reduction of the radius and scaphoid, treatment of trapeziometacarpal synovitis and arthritis, distal ulnar and carpal bone excisions, and salvage procedures. In addition, innovations such as new portals and smalle

Arthroscopically assisted percutaneous fixation for trans-scaphoid perilunate fracture dislocation.

Trans-scaphoid perilunate fracture dislocation is a complex carpal dislocation causing marked disruption of the carpal structures. Open treatment has been accepted as standard for this injury. We have used arthroscopically assisted percutaneous screw fixation and bone grafting to treat this injury in four patients. The functional outcome was good. All patients achieved solid union without nonunion or malunion. The complication and morbidity was relatively low; all patients had proper alignment and there was no evidence of instability or avascular necrosis or midcarpal arthritis.

Pregnancy-associated osteoporosis: an underestimated and underdiagnosed severe disease. A review of two cases in short- and long-term follow-up.

Pregnancy-associated osteoporosis is an uncommon condition characterized by the occurrence of painful fractures during late pregnancy or lactation. To date the pathophysiology of this entity of bone disorder is still uncertain, and its therapeutical management is poorly defined. We report two clinical cases: a 10-years follow-up with pain medication and intermittent antiresorptive therapy courses, subsequent traumatic vertebral fracture and actually fracture of scaphoid after inadequate trauma. Beside this long-term course a young female patient with pregnancy-associated osteoporosis and painf

CMC Mimickers: Differential Diagnosis and Work-Up for Radial-Sided Wrist Pain.

In patients with radial-sided wrist pain, a myriad of possible etiologies exists and as such, a detailed history, examination, and, where indicated, imaging is obtained to try and aid with its diagnosis. The purpose of this article is to provide an overview of radial-sided wrist pain, diagnostic modalities and discuss current treatment options. More detailed information is out-of-scope for this article and if needed, we would guide the reader to seek out other selected texts, as indicated.

A comparison of hand function in Colles and scaphoid casts using a modified Jebsen Hand Function Test.

This study investigates the impact of Colles versus scaphoid casts on hand function in modern day activities using the Jebsen Hand Function Test with additional modified subtests including texting on a mobile phone, using a computer mouse and typing on a computer keyboard. Twenty healthy volunteers were recruited for the study. Study participants performed worse in a scaphoid cast compared to a Colles cast; most evident in tasks involving fine motor movements such as writing, picking up small objects and stacking checkers (mean time difference 2.3 seconds, 1.5 seconds and 1.2 seconds, respecti

[Non-traumatic osteoarthritis of the wrist: chondrocalcinosis].

Post-traumatic arthritis of the wrist is a common disorder, mostly after scapho lunate or scaphoid injury. Some patients in our experience and in literature have no known trauma, are bilateral and have a mean age much higher than usual post-traumatic cases. Radiological (during an often-extensive medical history) and biological studies of these patients led us to think there is a form of chondrocalcinosis of the wrist, with a four stages evolution, similar to SLAC and SNAC wrist but with often no scapho lunate gap, vertical embedding of the scaphoid in the radius and chalky incrustation of the

Rotary subluxation of the scaphoid. Revisited.

The SL ligament complex and volar radiocarpal ligaments provide scaphoid stability both at the SL joint and, more importantly, at its distal radius articulation. With disruption or attenuation of these ligaments, rotational displacement of the scaphoid occurs about its longitudinal axis. This results in distal scaphoid flexion and proximal pole subluxation dorsally, toward the distal radius rim. Rotary subluxation of the scaphoid accurately describes this condition, and represents a spectrum of radioscaphoid instability based upon the severity and chronicity of ligamentous disruption. Diagnosi

Anterior dislocation of the lunate due to a high-energy trauma.

Semilunate and perilunate dislocation is an injury that mostly occurs when the subject is exposed to a high-energy trauma. Considered severe, it compromises the stability and function of the wrist. The case presented is that of a 50-year-old male patient who after having a motorcycle accident arrived at the ER with neurovascular affectation, swelling, deformity and a considerable decrease of the wrist joint movement. He was also referring severe pain. A Henry (Volar) intervention was done with the help of external fixation of the lunate, followed by K-wire fixation, once a prior closed reducti

Coronal fracture of the scaphoid--a case report and literature review.

Coronal fracture of the scaphoid is a rare configuration of scaphoid fracture. Only three cases have been found to be reported in English literature. In our case, a 25-year-old man sustained right wrist injury after a fall with an outstretched hand, was suspected of suffering from a scaphoid fracture as shown on the pre-operative plain radiograph. Coronal fracture of the patient's scaphoid was found intra-operatively and was fixed with two headless compression screws in dorsal-to-volar direction. Post-operative outcome was clinically good as patient was symptom-free, and a computed tomography
