Type 1 IFN-induced protein MxA and plasmacytoid dendritic cells in lesions of morphea.

Morphea is an autoimmune sclerotic skin disease of unknown pathogenesis. As type 1 interferons (IFN) have been implicated in the pathogenesis of systemic sclerosis, we proposed that type 1 IFN promote localized inflammation and fibrosis in morphea. To investigate the expression of the type 1 IFN-inducible protein myxovirus A (MxA) and the presence of plasmacytoid dendritic cells (pDC) in lesions of morphea, lesional skin of 10 patients with morphea was examined by immunohistochemistry for the presence of the type 1 IFN-inducible protein, myxovirus A (MxA), and the pDC markers, CD123 and BDCA-2

Scleroderma dermal fibroblasts overexpress vascular endothelial growth factor due to autocrine transforming growth factor β signaling.

OBJECTIVES: Overexpression of vascular endothelial growth factor (VEGF) in scleroderma (SSc) skin may play a role in the pathogenesis of the disease. Our study was undertaken to evaluate whether dermal fibroblasts function as one of the sources of the increased VEGF in SSc, and to clarify its mechanism. METHODS: Protein and mRNA levels of VEGF were analyzed using immunoblotting, enzyme-linked immunosorbent assay, and real-time PCR. The DNA-binding ability of Smad3 was evaluated by DNA affinity precipitation. RESULTS: VEGF mRNA expression in vivo was increased in SSc skin compared to skin wit

Synchronous angiosarcoma, melanoma and morphea of the breast skin 14 years after radiotherapy for mammary carcinoma.

With the improvement in survival after breast cancer there has been increasing interest in the long-term effects of radiotherapy, including the development of tumours. Compared with the general population, breast cancer survivors have a 10-50% higher risk of developing a second cancer. Radiotherapy may play a role in the onset of such lesions. We describe here the case of a 68-year-old woman who developed synchronous cutaneous angiosarcoma, melanoma and morphea of the breast skin and the local area, 14 years after radiotherapy for breast carcinoma. Given the risk of post-radiation secondary pr

'It hurts when I walk:' venous stasis disease--differential diagnosis and treatment.

Venous stasis disease represents 70% to 90% of lower-extremity ulcers treated. Venous hypertension brought about by inadequate venous return and defective valvular systems is the main culprit. Positive outcomes in patient care can be achieved by an ongoing partnership between the health care provider and the patient to control the disease and its effects. Appropriate assessment and management require a long-term commitment by a multidisciplinary team to encourage and enhance patient compliance and to prevent recurrence. In the next issue: arterial disease.

Soft tissue calcifications in the lower extremities of severely diabetic patients simulating venous stasis or collagen vascular disease.

The purpose of this work is to report soft tissue calcifications in severely diabetic patients that simulate venous stasis or scleroderma, without other stigmata of these diseases. Findings from lower extremity radiographs were reviewed on two patients with severe diabetes mellitus and abnormal soft tissue calcifications. Findings were correlated with clinical history, physical exam findings, and laboratory values. Both patients in this study demonstrate radiographic findings of lower extremity soft tissue calcifications similar to those seen in venous stasis or scleroderma. Clinical history,

Large, nonhealing scalp ulcer associated with scarring alopecia and sclerodermatous change in a patient with porphyria cutanea tarda.

Porphyria cutanea tarda (PCT) is a disorder caused by deficient activity of a liver enzyme that leads to the accumulation of photoactive metabolites in the skin. The initial clinical presentation commonly includes mechanical fragility of the skin and blisters in sun-exposed areas. Sclerodermatous skin changes and scarring alopecia are described in up to 20% of patients. Although these unique skin changes have been reported in the literature, information regarding nonhealing ulcers of extraordinary size is lacking. We report a unique cutaneous manifestation of PCT that is not well documented: u

[Clinical picture of Lyme boreliosis].

Prevalence of Lyme borreliosis in Europe has been well esablished during the last decade. The highest morbidity in Poland, exceeding 100 cases (100.000) year, was demonstrated in north-estern part of the country. Additional small endemic areas were also described in south-western and central part of the country. Clinical picture of the disease do not differ significantly from observed in other parts of Europe. Lyme borreliosis is characterized with a wide variety of manifestations recognized as typical: erythema migrans, borrelial lymphocytoma, acrodermatitis chronica atrophicans, arthritis, f

Composition of the inflammatory infiltrate in pediatric penile lichen sclerosus et atrophicus (balanitis xerotica obliterans): a prospective, comparative immunophenotyping study.

Dermatopathological evaluation of pediatric preputial inflammatory disease rarely allows for specific diagnosis other than pediatric penile lichen sclerosus et atrophicus (balanitis xerotica obliterans, LSA/BXO). A prospective immunopathological study was performed on 20 consecutive, unselected, clinically and histopathologically confirmed LSA/BXO cases to determine the relative presence of T and B lymphocytes. There were seven cases with early stages of disease, eight with florid disease, and five with later stages of disease. Two ritual circumcision specimens and 12 specimens with non-LSA/BX

[Vascular, pulmonary and neoplastic pathology in a construction worker. Problems of etiological diagnosis].

Although etiological diagnosis represent a major issue in occupational health, it is frequently difficult to achieve because of the concurrent effect of a multitude of both work-related and non-work-related pathogenic factors. A construction worker, exposed for over 20 years to silica and silicate dust, asbestos dust and vibrations transmitted through the hand-arm axis, complained of dyspnoea a cough and peripheral paraesthesia. Two years later he developed scleroderma-like skin lesions in the appendages, and a small-cell bronchogenic carcinoma was subsequently revealed by radiological and bro

[Serodiagnosis in dermatological diseases in Borrelia burgdorferi infections].

185 patients with dermatological symptoms of Borrelia burgdorferi infection (erythema migrans, lymphadenosis cutis benigna, acrodermatitis chronica atrophicans) and morphea were examined for Borrelia burgdorferi antibodies; in addition, sera from 173 patients were tested for exclusion of Borrelia infection. Commercially available immunofluorescence tests and enzyme immunoassays supplied by four different companies were evaluated. To investigate the specificity of these assays, sera of 34 patients with syphilis in different stages and sera from 98 control persons were examined. None of the assa

Blood flow through the brachial artery at different temperatures in patients with Raynaud's phenomenon.

Blood flow through the brachial and radial artery at different temperatures and during reactive hyperaemia was measured in 23 patients with Raynaud's phenomenon (9 patients with Raynaud's disease, 7 - Raynaud's syndrome with thoracic outlet syndrome (TOS), and 8 - Raynaud's syndrome and scleroderma), and 11 control subjects. Blood flow was determined by an ALVAR pulse-wave Doppler system enabling measurement of vessel diameter and blood flow rate. The measurements suggest that the blood flow through brachial artery reflects the changes of blood flow through the hand immersed in water of differ

[Arndt-Gottron scleromyxedema and associated phenomena].

Scleromyxedema is an uncommon fibromucinous connective tissue disease characterized by accumulation of mucinous material in the dermis. A monoclonal paraprotein is regularly identified. A review of the literature (57 cases) shows the exceptional association of scleromyxedema with multiple myeloma (8.7%) and macroglobulinemia Waldenström (3.5%). A man with scleromyxedema, IgG lambda paraproteinemia, and sclerodactylia--as a special sign of scleromyxedema--is reported. Melphalan is the drug of choice in serious cases, but not effective in sclerodactylia.

Vascularized omentum for facial contour restoration.

Our experience with the omental flap for facial soft-tissue augmentation in 7 patients is presented, along with our method of fixation of the graft to prevent gravitational descent. The history of the omental free flap, as well as the previously applied techniques for facial soft-tissue augmentation, is reviewed. It is our conclusion that the omental free flap is a safe, reliable technique. It can be readily molded to exactly conform to the patient's deformity and produces good results both in repose and during facial expression. We believe that gravitational descent of the graft can be preven

Clinical and Dermoscopic Features of Pigmentary Changes in Scleroderma and Its Association with Systemic Involvement - A Descriptive Study from a Tertiary Care Centre in South India.

BACKGROUND: Systemic sclerosis (SSc) is a multisystem disease with the highest case-specific mortality among other autoimmune diseases. Dermatological manifestations include skin thickening, Raynaud's phenomenon, digital ulcers, pitted scarring and pigmentary abnormalities. Vascular abnormalities in systemic sclerosis are responsible for various systemic complications including pulmonary hypertension and scleroderma renal crisis. The underlying vascular pathology responsible for organ involvement might also result in skin manifestations including Raynaud's phenomenon and digital ulcers. Howeve
