Automated dysphagia characterization in head and neck cancer patients using videofluoroscopic swallowing studies.

BACKGROUND: Dysphagia is one of the most common toxicities following head and neck cancer (HNC) radiotherapy (RT). Videofluoroscopic Swallowing Studies (VFSS) are the gold standard for diagnosing and assessing dysphagia, but current evaluation methods are manual, subjective, and time-consuming. This study introduces a novel framework for the automated analysis of VFSS to characterize dysphagia in HNC patients. METHOD: The proposed methodology integrates three key steps: (i) a deep learning-based labeling framework, trained iteratively to identify ten regions of interest; (ii) extraction of 23

Can the Repetitive Saliva Swallow Test Predict Airway Invasion in All Cause Dysphagia?

OBJECTIVES: The repetitive saliva swallow test (RSST) is a simple, noninvasive screening tool for oropharyngeal dysphagia, validated to dete … racteristic curve analysis, RSST had an area under the curve of 0.64, with an optimal cutoff of ≤ 6 swallows, yielding 89% sensitivity and 34% specificity. CONCLUSION: The RSST demonstrates statistical compatibility for airway invasion screening in an all-cause dysphagia population. However, its high cutoff values hinder its clinical utility due to modest …

Effect of deterioration of swallowing functions on the frailty status in older adults: a longitudinal cohort study.

BACKGROUND: Deterioration of swallowing function (DSF) is common among aging adults and can lead to negative health outcomes such as increased risk of malnutrition. However, the longitudinal relationship between the DSF and frailty status remains unclear. This study aimed to investigate whether declining swallowing function is linked to frailty progression in independent older adults. METHODS: This study included 795 community-dwelling Japanese individuals aged ≥ 65 years who participated in two surveys: one at baseline and one after two years. Swallowing function was evaluated using the repe

Patient Perceptions on a Wearable Sensor Technology for Swallowing: A Qualitative Study with Patients with Parkinson's Disease.

Wearable sensors show promise for monitoring breathing and swallowing events in patients with dysphagia. However, little is known about factors that facilitate their use for swallowing rehabilitation, particularly in individuals with Parkinson's disease (PD). Examining these factors in patients with PD (PwPD) is critical given the prevalence of motor disorders in this population. The aims of this qualitative study were to explore PwPD's: 1) impressions of a novel wearable sensor and customized iOS application developed for monitoring swallowing and breathing; 2) recommendations and desired fun

Association between the Hospital Anxiety and Depression Scale and Swallowing Function in Dysphagic Patients in Japan.

OBJECTIVE: Dysphagia affects 2.3 %-16 % of the general population and increases with age. It can lead to malnutrition, weight loss, aspiration pneumonia, and emotional symptoms such as anxiety and depression. Mental health disorders impact appetite and muscle mass, further worsening dysphagia. Additionally, cultural and economic factors influence anxiety and depression, which can either result from or contribute to dysphagia. Studies on the relationship between anxiety, depression, and swallowing function using FEES are limited in Asian populations. The Hospital Anxiety and Depression Scale (H

Speech-swallow dissociation in velopharyngeal closure for differentiating amyotrophic lateral sclerosis and myasthenia gravis.

OBJECTIVE: Speech-swallow dissociation (SSD) in velopharyngeal closure on laryngoscopy is a sign of pseudobulbar palsy. We hypothesized that this finding could differentiate amyotrophic lateral sclerosis (ALS) from myasthenia gravis (MG). This study aimed to identify laryngoscopic findings useful in differentiating these two diseases. METHODS: ALS and MG patients with bulbar symptoms who underwent fiberoptic laryngoscopy in our hospital were retrospectively examined. The following laryngoscopic items were evaluated: velopharyngeal incompetence (VPI) in phonation and swallowing, pharyngeal con

Swallowing dysfunctions in patients with disorders of consciousness: Evidence from neuroimaging data, assessment, and management.

Following severe brain injuries, a subset of patients may remain in an altered state of consciousness; most of these patients require artificial feeding. Currently, a functional oral phase and the presence of exclusive oral feeding may constitute signs of consciousness. Additionally, the presence of pharyngo-laryngeal secretions, saliva aspiration, cough reflex and tracheostomy are related to the level of consciousness. However, the link between swallowing and consciousness is yet to be fully understood. The primary aim of this review is to establish a comprehensive overview of the relationshi

Shared Neuroanatomical Substrates for Co-Occurring Swallowing and Communication Impairments after Acute Stroke.

INTRODUCTION: Post-stroke dysphagia and communication impairments occur in two-thirds of acute stroke survivors. Identifying the shared neuroanatomical substrate for related impairments could facilitate the development of cross-system therapies. Our purpose was to elucidate discrete brain regions predictive of the combined presence of dysphagia alongside dysarthria and/or aphasia post-stroke. METHODS: We included 40 right hemisphere stroke (RHS) and 67 left hemisphere stroke (LHS) patients from an acute ischemic stroke cohort with lesions demarcated on diffusion-weighted imaging. We undertook

Swallowing-related muscle inflammation and fibrosis induced by a single dose of radiation exposure in mice.

BACKGROUND: Although radiotherapy is commonly used to treat head and neck cancer, it may lead to radiation-associated dysphagia (RAD). There are various causes of RAD, however, the mechanism has not yet been fully identified. Currently, the only effective treatment for RAD is rehabilitation. Additionally, there are few available animal models of RAD, necessitating the development of new models to establish and evaluate RAD treatments. We hypothesize that radiation-induced neck muscle fibrosis could be one of the causes of RAD due to impairment of laryngeal elevation. Therefore, in this study,
