Tetraplegia is a profound impairment of mobility manifesting as a paralysis of all 4 extremities owing to cervical spinal cord injury. The purpose of this article is to provide an update and analyze current management, treatment options, and outcomes of surgical reconstruction of arm and hand function. Surgical restoration of elbow and wrist extension or handgrip has tremendous potential to improve autonomy, mobility, and critical abilities, for example, eating, personal care, and self-catheterization and productive work in at least 70% of tetraplegic patients. Tendon and nerve transfers, teno

BACKGROUND: The balance of the finger is maintained through a complex interaction of intrinsic and extrinsic mechanisms that coordinate moti … oft-tissue relationships to re-establish physiological force distribution. Surgical options include tendon transfer, intrinsic release, pulley reconstruction, centralization procedures and nerve reconstruction, dependent on underlying pathology.
CONCLUSION: Finger function depends on finely tuned balance of intrinsics and extrinsics. Understanding the dynamic an …

PURPOSE: Open reduction and internal fixation of distal radius fractures often necessitates release of the brachioradialis from the radial s … ause of the much larger contributions of the biceps and brachialis. Use of the brachioradialis as a tendon transfer donor should not be limited by concerns of elbow flexion loss, and the tendon could be considered as an autograft donor.

BACKGROUND: Foot drop from peroneal nerve palsy causes steppage gait and functional disability. Tendon transfer is reliable but sacrifices native biomechanics. Distal nerve transfer (DNT) reinnervates dorsiflexors, potentially restoring more physiologic function. This study aimed to systematically review and meta-analyze the clinical outcomes of DNT for foot drop and to identify key prognostic and technical factors that inform contemporary reconstructive decision-making.
METHODS: Following PRISMA, we searched MEDLINE, EMBASE, and Cochrane (through September 2025) for clinical studies of DNT t…

Introduction: Peroneal disorders are a common cause of ankle pain and lateral instability and have been described in as much as 77% of patients with lateral ankle instability. Clicking, swelling, pain, and tenderness in the peroneal tendons track are frequent symptoms, but they can be confused with other causes of lateral ankle pain. The management of peroneal disorders can be conservative or surgical. When the conservative treatment fails, surgery is indicated, and open or tendoscopic synovectomy, tubularization, tenodesis or tendon transfers can be performed. The authors present a surgical t…

Spontaneous attritional extensor tendon ruptures of the index finger due to carpal bone lesions are uncommon. Here, we report the case of a … osteophyte. He underwent surgery 24 days after the incident. First, the fragment was excised; then tendon transfer was performed. EIP and EDC2 tendons were bundled using a side-to-side suture and connected to the middle extensor digitorum communis tendon using interlacing sutures. Histopathological findings of the resected bone were compatible with osteoarthriti …

This assessment helps to identify muscles that could be used during nerve or tendon transfer for reconstruction of extensive upper limb paralysis.

OBJECTIVE: To study the utility and functional benefits of an implanted functional electrical stimulation (FES) system for hand grasp and re … se. In conjunction with implantation of the FES hand system, surgical reconstruction in the form of tendon transfers, tendon lengthenings and releases, and joint arthrodeses was performed to augment stimulated hand function. Rehabilitation of the tendon transfers and training in the use of the FES hand system were provided.

The author gives a literature review of the management of spasticity carried out. Physical agents used such as ice, heat and vibration are d … ectomies. Orthopaedic procedures involve muscle and tendon lengthening, release of contractures and tendon transfers. Surgery is discussed in terms of function. In considering the options, available care must be taken to achieve and maintain optimal functional level.

The radial forearm flap has proven to be versatile for free vascularized composite tissue transfers as well as for ipsilateral upper extremity reconstructions that require no microsurgical expertise. The most common objection to this otherwise advantageous donor area has been the subsequent nonaesthetic donor-site deformity. In addition, skin grafts frequently fail over the flexor carpi radialis tendon leading to chronic skin breakdown or at best tendon adhesions. Both these concerns may be ameliorated by means of tissue expansion of the remaining dorsal forearm skin to then allow removal of t

From 1984 to 1993, 36 vascularized toe joints were transferred in 33 patients. The present study group excludes 3 toe joint transfers to elbow and temporomandibular joints and 4 toe joint to hand transfers lost to follow-up. The final study group includes 29 vascularized toe joint transfers in 27 patients, 21 males and 6 females. All were performed for posttraumatic reconstruction, except one transfer for congenital deformity. Follow-up averaged 32.4 months. Mean range of motion was 34 degrees in toe metatarsophalangeal joint to hand metacarpophalangeal joint transfers, 32 degrees in toe proxi

INTRODUCTION AND IMPORTANCE: Flexor pollicis longus (FPL) tendon injury is a debilitating condition that rarely occurs after volar plate fix … ndition and documentation indicating a rupture of his FPL tendon in his right wrist, he underwent a tendon transfer.
CLINICAL DISCUSSION: While multiple factors may contribute to rupture, plate positioning seems to be the most critical. However, no clear cause-effect relationship has been established. The risk of rupture after volar plate fixation may never be e …

PURPOSE: Reverse shoulder arthroplasty (RSA) can restore active elevation in rotator-cuff-deficient shoulders. However, RSA cannot restore active external rotation. The combination of latissimus dorsi transfer with RSA has been reported to restore both active elevation and external rotation. We hypothesised that in the combined procedure, harvesting the latissimus dorsi with a small piece of bone, leads to good tendon integrity, low rupture rates and good clinical outcome.
METHODS: Between 2004 and 2010, 13 patients (13 shoulders) were treated with RSA in combination with latissimus dorsi tra

In all cases, the nail entry site was directly related to the location of EPL. In many of the cases the EPL dysfunction occurred early on but it's recognition was often delayed.

We report the effect of introducing a dedicated Ponseti service on the five-year treatment outcomes of children with idiopathic clubfoot. Between 2002 and 2004, 100 feet (66 children; 50 boys and 16 girls) were treated in a general paediatric orthopaedic clinic. Of these, 96 feet (96%) responded to initial casting, 85 requiring a tenotomy of the tendo-Achillis. Recurrent deformity occurred in 38 feet and was successfully treated in 22 by repeat casting and/or tenotomy and/or transfer of the tendon of tibialis anterior, The remaining 16 required an extensive surgical release. Between 2005 and 2
