Predictors of postoperative complications in extensor tendon repairs.

OBJECTIVES: This study looked at what factors lead to complications after extensor tendon repair, with a focus on how the location of the injury affects patient outcomes. PATIENTS AND METHODS: This retrospective cohort study looked at 206 patients with extensor tendon injuries. All patients were treated at one center using the same surgical and rehabilitation protocols. We collected data on patient demographics, details of the injuries, and any complications after surgery. Logistic regression was employed to identify predictors of complications. RESULTS: The complication rate was 22.3%, with

The role of tendon and ligament vasculature in ageing and injury.

Ageing is a fundamental biological process marked by declining physiological function and the accumulation of stress-induced damage. It contributes to chronic disease and drives widespread alterations across organ systems, including the vasculature. Ageing of the macro- and microvasculature impairs nutrient delivery, promotes tissue degeneration, and reduces repair capacity in many tissues, including those of the musculoskeletal system. Tendons and ligaments become increasingly injury-prone with age, yet the relationship between vascular ageing and tendon and ligament in health and disease rem…

vNOTES retroperitoneal uterosacropexy using the semitendinosus tendon.

OBJECTIVE: The objective of this report is to demonstrate the feasibility of combining the harvested semitendinosus tendon technique (HoTT) technique, originally described laparoscopically, with a retroperitoneal vaginal natural orifice transluminal endoscopic surgery (vNOTES) approach for central compartment prolapse repair. METHODS: A 55-year-old woman with stage III uterine prolapse underwent native tissue sacropexy using an autologous semitendinosus tendon via the retroperitoneal vNOTES approach. Via a pararectal entry, the longitudinal ligament was identified, and the uterus was fixed to

Quantitative Sodium MRI of the Human Patellar Tendon.

Sodium MRI can be used in musculoskeletal imaging to assess tissue composition because sodium concentration is related to glycosaminoglycan (GAG) content. In tendinopathy, the GAG content is elevated, resulting in a high signal in sodium MRI. Because biochemical changes typically precede structural alterations, sodium MRI may be a useful indicator for detecting early tendon matrix changes associated with tendinopathy. Quantitative sodium concentration determination has only been applied to Achilles tendons but not yet to smaller tendons, such as the patellar tendon. Due to its smaller diameter…

Gout and Patellar Tendon Tears: An Uncommon Intersection.

Patellar tendon (PT) tears are a rare cause of anterior knee pain and are usually classified as either partial or complete, with various potential etiologies. We present a unique case of a PT tear in a young male patient, precipitated by a history of gout. This case highlights the uncommon intersection of PT tears and gouty arthritis involving the entheses. The patient was a 36-year-old man of Indian origin who presented with a two-week history of atraumatic right knee swelling, with no identifiable precipitating factors aside from a known history of gout flares. Clinical examination revealed

Early Tendon Transfer in Diabetic Hand Infection.

Hand infections in diabetic patients are a serious concern, as they can lead to significant functional impairment and, if not promptly treated, may result in extensive tissue destruction or even sepsis. Unlike diabetic foot infections, hand infections are less understood and often present unique challenges in management. Diabetic patients are at an increased risk due to factors such as neuropathy, impaired immune function, and poor wound healing. These infections, if neglected, can result in permanent disability. This report presents a rare case of a diabetic hand infection that was complicate

Patellar Tendon Repair With Bioinductive Implant Augmentation.

Patellar tendon ruptures are relatively uncommon injuries that often occur in the setting of chronic patellar tendinopathy, particularly in those participating in high-level pivoting sports and jumping athletics secondary to overuse. Tear patterns vary, with the most common being avulsions from the inferior pole of the patella. For complete tears resulting in extensor mechanism compromise, surgical treatment typically entails acute repair. In this technique, we describe the use of a bioinductive bovine collagen implant to augment a patellar tendon repair in the setting of chronic patellar tend

Surgical Repair of Proximal Hamstring Tendon Avulsion.

Injuries to the proximal hamstring muscle complex are common in athletes and range from strains to tendinous and bony avulsions. The lesion mechanism typically involves an eccentric contraction of the hamstring muscles during abrupt hip hyperflexion with the knee in extension. Low-speed injuries occur in high kicks and splits, whereas tendon avulsions are common in high-speed activities, such as running and ballet. Clinically, patients present with pain, subcutaneous hematoma, and, sometimes, a palpable defect. Additional signs include limited knee extension and involvement of the sciatic nerv

Paratenon effusion of the Achilles tendon: a rare finding.

We present a case of Achilles tendon swelling and pain related to paratenon effusion. Further ultrasound assessment showed no evidence of ac … loping rheumatoid arthritis. Further studies have shown that the paratenon surrounding the extensor tendons consists of a lining layer formed by fibroblasts. However, more research is needed to characterize these fibroblasts and compare them to those found in the synovium.

Oral fluoroquinolones and the risk of Achilles tendon rupture.

• Using nationwide Danish health register data, associations between fluoroquinolone use and elevated hazard rates of Achilles tendon rupture were found. • Associations were most pronounced with increasing dosage and within 30 days following fluoroquinolone use. • Despite recent findings calling the previously suggested associations between fluoroquinolones and aortic and valvular diseases into question, advised caution in relation to tendinopathies still seems relevant.

Tendon transfers for the management of irreparable subscapularis tears.

Rotator cuff pathology is the main cause of shoulder pain and dysfunction in older adults. When a rotator cuff tear involves the subscapularis tendon, the symptoms are usually more severe and the prognosis after surgery must be guarded. Isolated subscapularis tears represent 18% of all rotator cuff tears and arthroscopic repair is a good alternative primary treatment. However, when the tendon is deemed irreparable, tendon transfers are the only option for younger or high-functioning patients. The aim of this review is to describe the indications, biomechanical principles, and outcomes which ha

Current treatment concepts for Achilles tendon rupture.

Achilles tendon rupture is a common and primary cause of lower limb tendon injury suffered during sports-related activities. The causes of Achilles tendon rupture include the calf muscle and tendon overuse, poor tendon quality, and various medical conditions. Historically, acute Achilles tendon rupture was treated conservatively. However, historical techniques are now associated with an increased risk of rerupture. To address this problem, open repair has been proposed. Open repair is associated with a reduced risk of rerupture; however, it is also closely associated with wound complications,

Anatomic Popliteus Tendon and Tibiofibular Joint Reconstructions.

BACKGROUND: The popliteus tendon is a primary stabilizer of external knee rotation and has been described as the "fifth ligament of the knee." Injuries involving the posterolateral corner of the knee commonly involve the popliteus tendon; isolated injury to the popliteus tendon is relatively rare and usually involves a rotatory injury mechanism and symptoms of instability and pain. In this patient, there was a concomitant injury to the proximal tibiofibular joint. INDICATIONS: Popliteus tendon reconstruction is indicated for patients with abnormal external rotation on examination and symptoms
