Transoral laser surgery for supraglottic cancer.

The goal of treatment for supraglottic cancer is to achieve cure and to preserve laryngeal function. Organ preservation strategies include b … or to those of the conventional open approach, in terms of the time required to restore swallowing, tracheotomy rate, incidence of pharyngocutaneous fistulae, and shorter hospital stay. The management of the neck remains of paramount importance, as survival of patients with supraglottic cancer depends more on cervical metastasis than on the primary tumor. Mos …

Awake fibrecapnic intubation: a novel technique for intubation in head and neck cancer patients with a difficult airway.

Awake fibreoptic intubation is the gold standard for difficult airway management but failures are reported in the literature in up to 13% of cases. In case of failure, a tracheotomy is often indicated. We describe a novel technique for intubation in head and neck cancer patients with a difficult airway that we call awake fibrecapnic intubation. The aim of this study was to investigate the feasibility of this technique. We studied prospectively 15 consecutive intubations in head and neck cancer patients before diagnostic or therapeutic surgical procedures. After topical anaesthesia, a fibrescop…

Management of congenital laryngeal malformations.

Congenital malformations of the larynx are relatively rare but may be life-threatening. The most common causes include laryngomalacia, vocal cord paralysis, and subglottic stenosis. The last 20 years has seen major advances in the field of surgical correction of such anomalies also serving to reduce the number of tracheotomies in children and the inherent dangers they pose. Success rates for the most popular surgical procedures have been favorable. These include supraglottoplasty for cases of severe laryngomalacia, in which relief of respiratory symptoms has been shown to occur in excess of 80

Microbial flora of the subglottis in intubated pediatric patients.

Infection of the intubated subglottis is felt to be one of the many factors involved in the pathogenesis of acquired cicatricial subglottic stenosis. The precise role of infection is unclear and the microbial flora has not been established. An analysis of subglottic culture material, from 22 intubated pediatric patients undergoing tracheotomy, has been performed to establish the nature of the subglottic microbial flora. Fifty-nine isolates were found, including 19 types of organisms. The number of isolates per patient ranged from one to eight, with an average of 2.7 isolates per patient. The m…

[Surgical treatment of myasthenia by thymectomy. A report on 248 cases (author's transl)].

Good results were obtained in 80 p. cent of 248 patients seen after more than 15 months following the operation. Results are grouped as a function of the different characteristic features of the patients and three groups can be defined: failures, improvement, remissions. These results contradict those who assert that thymectomy is of no value for severe cases. Cervicotomy is performed for non-tumoral forms or for small tumors; sternotomy is reserved for large median thymomas; anterolateral thoracotomy for laterally located thymomas and tracheotomized patients. Postoperative tracheotomy is very…

Topical chemotherapy of recurrent respiratory papillomatosis. A preliminary report.

Topical chemotherapy with the antimetabolite 5-fluorouracil (5-FU) has been used as an adjunct to surgical laser excision of squamous papillomas arising in the larynx and in the tracheobronchial tree. The drug has been administered intensively to eight patients with tracheotomies and disease in both anatomic regions. These patients received repeated instillations of 5-FU at concentrations from 0.5 to 5% into the larynx and tracheobronchial tree while in the hospital and at home. More than 4,500 doses of the drug have been administered during our phase 1 study. There has been no significant tox

A meta-analysis of prehospital airway control techniques part II: alternative airway devices and cricothyrotomy success rates.

BACKGROUND: Airway management is a key component of prehospital care for seriously ill and injured patients. Oral endotracheal intubation (OETI) is the definitive airway of choice in most emergency medical services (EMS) systems. However, OETI may not be an approved skill for some clinicians or may prove problematic in certain patients because of anatomic abnormalities, trauma, or inadequate relaxation. In these situations alternative airways are frequently employed. However, the reported success rates for these devices vary widely, and established benchmarks are lacking. OBJECTIVE: We sought

Arytenoidectomy in children.

Vocal cord paralysis is the second most common cause of stridor in early infancy, and as many as 52% of patients will not recover spontaneously. Bilateral vocal cord paralysis often requires a tracheotomy for airway distress. If resolution of the bilateral vocal cord paralysis does not allow for decannulation, arytenoidectomy is an option. A retrospective review of 30 children with bilateral vocal cord paralysis who underwent an arytenoidectomy was undertaken. An external arytenoidectomy via laryngofissure was performed in 19 patients, a laser arytenoidectomy in 12 patients, and a Woodman proc…

Skin grafting of the larynx.

Skin grafting as a method of restoring the lumen in cases of laryngeal stenosis has been promoted and practiced in the past. Lately, little has been heard of the method. I have personally used skin grafting in almost every case of repair of acute injury or chronic stenosis of the larynx in the last several years. The rationale for grafting is (1) to cover any areas of epithelial loss or dehiscence during the healing period, (2) to reduce the chance of prolonged granulation and subsequent contracture, and (3) to serve as a logical material to use as the interface between a stent and the lumen o

Foreign bodies in the tracheobronchial tree.

Between 1972 and 1984, we treated 66 patients with foreign bodies (FBs) in the tracheobronhial tree. Twenty-six patients (39.4 percent) were between one and two years old; 52 (78.8 percent) were under age ten. Forty-four FBs (67 percent) were fruit and vegetable seeds and nuts. Soft organic material (meat, cucumber peels) was found in four patients, chicken bones in six, pins and needles in six, other nonorganic materials (toys, stone, broken thermometer) in six. In 55 patients, the FB was removed at bronchoscopy; three patients coughed up the FB shortly after bronchoscopy. Eight patients were
