[A rheumatologist's point of view on lumbago].

The rheumatologist has to be sure the low back pain of more than three months duration has a mechanical origin, i.e. that it is not due to a tumoral, infectious, metabolic or traumatic vertebral disease. He has to recognise a low back pain from visceral origin and, at the opposite, a functional disorder. The mechanical low back pain may arise from the intervertebral disk, the articular process, a spondylolisthesis, etc... The management includes various physical and medicamentous means; the surgical procedures must be exceptional and have to be done only in highly selected patients. There is a

An intradural extramedullary tuberculoma of the spinal cord in a non-HIV-infected patient: case report and review of the literature.

Spinal tuberculomas are extrapulmonary manifestations of tuberculosis involving the central nervous system. They are characterized as extradural, intradural extramedullary, or intradural, according to their location. Intradural extramedullary tuberculomas are extremely rare. There have been only 24 case reports found in English language literature. Our case is the only documented intradural extramedullary tuberculoma in a non-HIV-infected patient in North America. A literature review using a Medline search from 1966 to the present is performed to characterize the clinical spectrum of the three

Congenital kyphosis.

Congenital kyphosis is an uncommon, but potentially devastating anomaly of the spine. Without treatment, the tendency is for severe, and sometimes catastrophic deformity. It is the most common etiology for spinal cord compression due to spine deformity, excluding tuberculosis. Few patients have nonprogressive benign lesions. Late treatment is difficult and dangerous. Braces and other forms of non-operative treatment do not work. The best treatment is early posterior fusion, before the curve reaches 50 degrees and before age 5 years. For curves over 60 degrees and over age 5, both posterior and

Single-stage posterior debridement, decompression and transpedicular screw fixation for the treatment of thoracolumbar junction (T12-L1) tuberculosis with associated neurological deficit: a multicentre retrospective study.

BACKGROUND: A multicentre retrospective study was conducted to evaluate the safety and efficacy of single-stage posterior debridement, decompression and transpedicular screw fixation for the treatment of thoracolumbar junction (T12-L1) tuberculosis in patients with associated neurological deficit. METHODS: Thoracolumbar junction (T12-L1) tuberculosis patients (n = 69) with neurological deficit who underwent single-stage posterior debridement, decompression and transpedicular screw fixation from January 2005 to January 2015 were included in the study. Antituberculosis therapy was performed bot

Tuberculous Longitudinally Extensive Transverse Myelitis: A Case Report and Literature Review.

Longitudinally extensive transverse myelitis (LETM) is a rare but severe manifestation of central nervous system (CNS) tuberculosis (TB). Because of its nonspecific clinical presentation and broad differential diagnosis, early recognition remains challenging. We report the case of a 27-year-old man with concurrent pulmonary and meningeal TB who developed rapidly progressive quadriparesis. Magnetic resonance imaging demonstrated longitudinally extensive T2-hyperintense lesions extending from C2 to T12, accompanied by diffuse meningeal enhancement, consistent with TB LETM. The patient was treate

Destructive spondyloarthropathy mimicking spondylitis in long-term hemodialysis patients.

A 63-year-old man with end-stage renal disease (ESRD) who had been undergoing hemodialysis for 18 years suffered persistent neck pain, progressive quadriparesis, and a deteriorating ataxic gait during the 6 months before admission. A sudden onset of aggravating quadriparesis and an inability to ambulate occurred during his trip to Sydney, Australia, 1 week before this admission. Vertebral tuberculosis osteomyelitis of the C5/6 segment was considered and treated in a hospital there. Findings from cervical magnetic resonance imaging (MRI; low signal intensity on both T1- and T2-weighted images)

Surgical Management of Post-tuberculous Spinal Deformity in Adults: A Case Series Report from a Nigerian Tertiary Orthopaedic Centre.

Post-tuberculous spinal deformity remains an important cause of chronic morbidity following healed spinal tuberculosis, particularly in low- and middle-income countries. Residual kyphosis may result in persistent pain, sagittal imbalance, cosmetic deformity and late neurological deterioration. Surgical correction is technically demanding because of rigid deformity and compromised bone quality, yet it may offer meaningful functional recovery when appropriately indicated. We report a case series of three adult patients with post-tuberculous spinal deformity managed surgically at a Nigerian terti

Post-tubercular Lumbosacral Kyphotic Deformity in a 11-Year-Old Child: A Case Report.

CASE: An 11-year-old girl with intact neurology presented with a lumbosacral kyphotic deformity due to healed tuberculosis. Radiological imaging showed sagittal balanced spine with compensatory thoracic lordosis and cervical kyphosis. She underwent L4 and L5 posterior vertebral column resection (PVCR) with posterior instrumentation from L2 to pelvis. The patient demonstrated immediate correction of compensatory curves postoperatively. At 3-year follow-up, she returned to her activities of daily living with Oswestry Disability Index and Scoliosis Research Society scores of 12 and 4.8% respectiv

Spondylodiscitis: is really all well defined?

The term spondylodiscitis describes the infection of both the intervertebral disc space and the adjacent vertebrae. Pyogenic Vertebral Osteomyelitis (PVO) is more common in older patients (mean age 59-69 years) with a male preponderance (52-69%). Recent studies reported an alarming increase of incidence over the last 20 years, due to the increase of diagnostic sensibility, the increase of the average lifetime and to the consequent association of chronic disabling pathologies, of immunosuppression, of surgical or invasive procedure. Improvements in radiological diagnosis, surgical techniques, a

[Posteriorinstrumental spinal fixation in tuberculous spondylitis and osteomyelitis of the vertebral bodies].

The short- and long-term results of surgical treatment were studied in 74 patients with tuberculous spondylitis and in 20 patients with chronic osteomyelitis of the vertebral column. All the patients underwent radical reconstructive operations on the anterior vertebral column if they had an active inflammatory process. The operations were supplemented by an interstitial posterior instrumental fixation with the Hurrington manipulation reduction frames or the plates, developed at the Central Institute of Traumatology and Orthopedics, in 11 cases and with the Cotrel-Dubosse tools in 28 cases. The

Lewis Albert Sayre.

Lewis Albert Sayre (1820-1900) was an American pioneer in the field of orthopedic surgery, and medicine in general. In addition to much work on spinal and other deformities, Sayre was a founder of what later became the New York University Medical School, where he headed the first department of orthopedics in the United States. Sayre also was a charter member of the American Medical Association and served as its vice president and later as president. He was instrumental in developing the Journal of the American Medical Association, and his extensive work as Resident Physician of New York City l
