Effect of the calcium antagonist diltiazem on atrioventricular conduction in chronic atrial fibrillation.

The effect of oral diltiazem treatment on the mean ventricular rate was studied in 10 patients with stable atrial fibrillation (AF). The profile of mean ventricular rate was analyzed by means of 24-hour electrocardiographic recordings. Both single-dose (120 mg) and maintenance therapy (80 mg 3 times daily) reduced the mean ventricular rate significantly. After the single dose, the effect set in after 120 +/- 40 minutes (mean +/- standard deviation) and persisted for 347 +/- 84 minutes. Histograms of RR intervals were plotted and their changes after diltiazem therapy were also analyzed. The sho

Right atrial-ventricular dissociation and entrainment while pacing from high right atrium and coronary sinus during circus movement tachycardias.

Thirteen patients with circus movement tachycardias (CMT) were studied. Twelve had left-sided, and one, right-sided, accessory pathways. Entrainment was possible during overdrive high right atrial stimulation in 13/13 patients, and during coronary sinus pacing in 10/12 patients. The minimal pacing rates required for this to occur were 10 to 31 beats/min faster than those of the tachycardias. Short episodes of right atrial-ventricular dissociation occurred while pacing from the high right atrium (6/13 patients), but not from the coronary sinus (0/13 patients). It is possible to explain this phe

The syndromes of familial atrioventricular block with sinus bradycardia: prognostic indices, electrophysiologic and histopathologic correlates.

A family with adult onset heart block (AOHB) and sinus bradycardia (SB) was investigated 5 yr after previous study. A 44-yr-old female previously 'normal' presented ECG evidence of 1st-degree and 2nd-degree atrioventricular block (AVB) with normal intraventricular conduction and episodic 3rd-degree AVB. His bundle electrocardiography (HBE) demonstrated A--H prolongation (245msec) with normal H--V time during sinus rhythm and 3rd-degree AV nodal block with a His bundle escape rhythm. Sinus node function served in first-order relatives. HBE and sinus node function were normal in a 38-yr-old sist

Meta-analysis of cardiovascular outcome trials assessing the impact of glucagon-like peptide-1 receptor agonists on major cardiac arrhythmias.

OBJECTIVE: Glucagon-like peptide-1 receptor agonists (GLP-1RAs), a group of novel antidiabetic agents, demonstrated beneficial cardiovascular effects in recent large, placebo-controlled randomised clinical trials (RCTs); their clear antiarrhythmic benefit has not been yet underlined. The purpose of the present meta-analysis is to clarify the impact of GLP-1RAs on different types of cardiac arrhythmias. METHODS: We searched PubMed from its inception up to 8 October 2020 for all available cardiovascular and renal outcome, placebo-controlled RCTs utilising GLP-1RAs versus placebo. The present me

Genomic and physiological analyses of the zebrafish atrioventricular canal reveal molecular building blocks of the secondary pacemaker region.

The atrioventricular canal (AVC) is the site where key structures responsible for functional division between heart regions are established, most importantly, the atrioventricular (AV) conduction system and cardiac valves. To elucidate the mechanism underlying AVC development and function, we utilized transgenic zebrafish line sqet31Et expressing EGFP in the AVC to isolate this cell population and profile its transcriptome at 48 and 72 hpf. The zebrafish AVC transcriptome exhibits hallmarks of mammalian AV node, including the expression of genes implicated in its development and those encoding

A new and simple technique for vagal ganglia ablation in a patient with functional atrioventricular block: Electroanatomical approach.

Increased parasympathetic tone may cause symptomatic functional atrioventricular block (AVB) and necessitate pacemaker implantation. In these patients, where there is no structural damage to the conduction system, removal of the vagal activity using radiofrequency ablation seems to be a theoretically rational approach. Several methods have been used to determine suitable areas for vagal ganglia ablation. The aim of this report was to describe a new method to detect parasympathetic innervation sites without the need to use additional equipment or extend procedure time. A 51-year-old man was ref

Cardiac sarcoidosis presenting with syncope and rapidly progressive atrioventricular block: a case report.

BACKGROUND: Cardiac sarcoidosis (CS) accounts for a substantial morbidity and mortality. Early recognition of CS is important to prevent such detrimental consequences. A definite diagnosis of cardiac sarcoidosis remains challenging. Even after the diagnosis of CS is established, the appropriate dose and duration of corticosteroids in the treatment of CS have not been well-defined. CASE SUMMARY: In this report, we discuss a case of a 50-year-old man who presented with recurrent syncope. Electrocardiogram revealed sinus rhythm with left bundle branch block. Telemetry captured high-grade atriove

Ion channel-kinase TRPM7 is required for maintaining cardiac automaticity.

Sick sinus syndrome and atrioventricular block are common clinical problems, often necessitating permanent pacemaker placement, yet the pathophysiology of these conditions remains poorly understood. Here we show that Transient Receptor Potential Melastatin 7 (TRPM7), a divalent-permeant channel-kinase of unknown function, is highly expressed in embryonic myocardium and sinoatrial node (SAN) and is required for cardiac automaticity in these specialized tissues. TRPM7 disruption in vitro, in cultured embryonic cardiomyocytes, significantly reduces spontaneous Ca(2+) transient firing rates and is

Meta-analysis of ablation of atrial flutter and supraventricular tachycardia.

The purpose of this study was to perform a systematic review and meta-analysis to evaluate the safety and efficacy of radiofrequency ablation (RFA) of typical atrial flutter (AFL) and atrioventricular node-dependent supraventricular tachycardia (SVT) in adult patients. Medline and EMBASE were searched (1990 to 2007) for all study design trials of RFA. Data relating to single- and multiple-procedure success, arrhythmia recurrence, repeat ablation, adverse events, and death were extracted. For RFA in AFL, 18 primary studies with 22 treatment arms and 1,323 patients were identified. Single-proced…

Cardiac rhythm after Mustard repair and after arterial switch operation for complete transposition.

We compared the prevalence of arrhythmias among the first consecutive 45 patients with complete transposition (concordant atrioventricular and discordant ventriculo-arterial connexions) after arterial switch operation and the last 47 patients after Mustard repair in infancy. Both groups had 24-hour Holter electrocardiographic studies at similar periods of follow up (24 +/- 14 and 25 +/- 18 months). A second group of patients undergoing the Mustard procedure had been repaired at an older age before 1981. They were studied to determine the frequency of disturbances of rhythm during later postope

Effects of amiodarone on supraventricular tachycardia involving bypass tracts.

This study evaluates whether the electrophysiologic effects of i.v. amiodarone in patients with reentrant supraventricular tachycardia (SVT) can predict the efficacy of long-term oral therapy with this drug. The effects of oral and i.v. amiodarone were studied in 27 patients with SVT. In 14 the SVT circuit involved a concealed atrioventricular bypass for retrograde conduction (Group I), and in 13 a concealed atrio-His bypass (Group II). Intravenous amiodarone induced significant prolongation of the AH interval, the refractory periods of the atrium, atrioventricular node, His-Purkinje system an…

Antidromic Nodoventricular Reciprocating Tachycardia With Left-Ventricular Insertion: Insights From High-Density Mapping.

We describe the case of a 38-year-old male with a long history of paroxysmal supraventricular tachycardia (PSVT) who was found to have typical slow-fast atrioventricular nodal reentrant tachycardia (AVNRT) and nodoventricular reentrant tachycardia (NVRT). During an electrophysiology study, a wide complex tachycardia with left bundle branch block (LBBB) morphology with V2 transition was induced, exhibiting ventriculo-atrial (VA) dissociation and characteristics suggesting an AV node-ventricle reentrant circuit. Careful mapping and pacing maneuvers supported the diagnosis of a nodoventricular re
