An irreducible variant of femoral neck fracture: a minimally traumatic reduction technique.

We present 25 cases of irreducible variant femoral neck fractures that require surgical management after routine manipulative manoeuvre attempts have failed. In our study, an irreducible variant of femoral neck fractures is defined as a reduction that cannot be achieved after multiple attempts at closed reduction. This was evident radiographically, as seen in displaced–impacted femoral neck fractures when the proximal femur compacts and rotates along with the distal part, and anatomical reduction cannot be achieved with manipulative manoeuvres. Another rare situation also included is when the

Arthroscopically assisted fixation of Hawkins type II talar neck fractures: a case series.

AIMS: Arthroscopically controlled fracture reduction in combination with percutaneous screw fixation may be an alternative approach to open surgery to treat talar neck fractures. The purpose of this study was thus to present preliminary results on arthroscopically reduced talar neck fractures. PATIENTS AND METHODS: A total of seven consecutive patients (four women and three men, mean age 39 years (19 to 61)) underwent attempted surgical treatment of a closed Hawkins type II talar neck fracture using arthroscopically assisted reduction and percutaneous screw fixation. Functional and radiologic

Acute total hip arthroplasty for anterior hip fracture-dislocation: a case report.

Hip fracture-dislocations accompanied by femoral head or acetabular fractures remain challenging, and the optimal treatment strategy is still controversial. Anterior hip fracture-dislocations are particularly rare, and reports describing acute total hip arthroplasty (THA) for this injury pattern are limited. We report the case of a 74-year-old active woman who sustained an anterior hip fracture-dislocation with femoral head and acetabular anterior wall fractures after a low-energy fall. The injury was classified as Pipkin type IV and Brumback type 4B. Following closed reduction, acute THA was

Reevaluation of the Scaphoid Fracture: What Is the Current Best Evidence?

Scaphoid fractures are common and notorious for their troublesome healing. The aim of this review is to reevaluate the current best evidence for the diagnosis, classification, and treatment of scaphoid fractures and nonunions. MRI and CT are used to establish a "definitive diagnosis" with comparable diagnostic accuracy although neither is 100% specific. Current classifications cannot reliably predict union or outcomes; hence, a descriptive analysis of fracture location, type, and extent of displacement remains most useful. Treatment of a nondisplaced scaphoid waist fracture remains an individu

Minimally invasive metatarsal fracture repair with locking plates in a guanaco (Lama guanicoe).

An open grade I transverse diaphyseal fracture of the metatarsal bone in a 5-yr-old female guanaco (Lama guanicoe) was successfully stabilized with a locking plate system, which is a minimally invasive approach. A conical coupling screw-plate locking system with 3.0-mm-thick plates and 3.5-mm locking head screws was applied, after closed and indirect reduction of the bone segments, with an orthogonal configuration of the plates. The implants were protected postoperatively with a splinted bandage. The fracture healed without complications, the limb function was successfully recovered, and no ma

Giant life-threatening external carotid artery pseudoaneurysm caused by a mandibular condylar fracture.

OBJECTIVE: Vascular lesions are rare complications associated to mandibular condylar fractures. This paper aims to describe a case of a giant pseudoaneurysm involving the external carotid artery (ECA) caused by a condylar fracture. CASE REPORT: A 33-year-old man was the victim of traffic accident and presented with a panfacial fracture, including a bilateral condylar fracture. The condylar fracture was treated by closed reduction, and 4 weeks after treatment, the patient developed facial edema, which suggested postoperative infection. An attempt at draining it resulted in intensive bleeding.

Long term results of open reduction management of condylar fracture: a 20 years follow-up. Case report.

Management of condylar fracture may be surgical (open reduction) or non surgical (closed treatment). The age of the patient is a key factor in the choice of treatment. Condylar fracture requires early treatment to avoid ankylosis, to reduce facial deformity, aesthetic problems and functional disturbances. The protocol described in this case report showed excellent results and achieved all the treatment objectives. Early mobilization is the key in treating condylar fractures and to allow functional remodelling. It is important that every specialist should be able to suspect condylar fracture wh

Antegrade nailing for fractures of the distal femur.

Thirty-five fractures of the distal femur were treated by antegrade intramedullary nailing. All of the fractures were treated by a closed technique. In the cases in which there was extension of the fracture to the knee joint, the fracture initially was stabilized with percutaneous lag screws. Four patients died before fracture healing was complete and 11 were lost to followup. There were 14 extraarticular (Type A) and six intraarticular (Type C1 and Type C2) fractures. All of these fractures healed in a mean time of 17.27 weeks. Functional outcome using the Hospital for Special Surgery knee sc

[Birth trauma as the cause of fracture of the distal epiphysis of the humerus. A case report].

The authors report on a case of fracture of the distal humeral epiphysis sustained at birth. This is extremely rare. The main problem in treatment of this kind of fracture is correct and timely diagnosis. In almost all reported cases the fracture has been confused with elbow dislocation. The reason for this mistake is difficult roentgenological orientation owing to invisible ossification nuclei. Ultrasono und examination is recommended for diagnosis. The treatment of the injury is conservative. Closed reduction and immobilization for 3 weeks in an above-the-elbow plaster cast is advised. The e

Percutaneous Kirschner-wire pinning for severely displaced distal radial fractures in children. A report of 157 cases.

Distal radial fractures are common in children. Recent outcome studies have cast doubt on the success of treatment by closed reduction and application of plaster. The most important risk factor for poor outcome is translation of the fracture. If a distal radial fracture is displaced by more than half the diameter of the bone at the fracture site it should be classified as high risk. We performed percutaneous Kirschner-wire pinning on 157 such high-risk distal radial fractures in children under 16 years of age. The predicted early and late failure rate was reduced from 60% to 14% and only 1.5%

Extensor tendon involvement in Smith's and Galeazzi's fractures.

Extensor tendon involvement in three Smith's and two Galeazzi's fractures are reported. In two cases of previous Smith's fracture, the extensor indicis proprius tendon had been entrapped and ruptured in one case, and in the other the extensor digitorum communis tendon of the index was pierced by the fragment. Both showed a severe tenodesis effect on the index finger and thumb. In a new case of Smith's fracture, long extensor tendons of the fingers and thumb had been trapped beneath a dorsally displaced third fragment making closed reduction impossible. In the two cases of Galeazzi's fracture;

Epiphyseal injuries of the foot and ankle.

A thorough knowledge of functional growth plate anatomy and physiology is essential to proper management of epiphyseal foot and ankle injuries. The ability to classify foot and ankle fractures according to the Salter-Harris anatomic and radiographic classification provides useful prognostic information that may affect treatment. The Dias-Tachdjian mechanistic classification system for pediatric ankle fractures provides useful information about the extent of osseous and soft tissue injury and the best method of closed reduction and correlates well with the Lauge-Hansen system, which is widely u

Data on inter-locking nail in humeral shaft fractures among Indian patients.

Humeral shaft fractures are commonly seen orthopaedics injuries. Open reduction with internal fixation (ORIF) with plating is a gold standard procedure despite various issues such as infection, radial nerve palsy and no-nunion. Close reduction with interlocking nails (ILN) is not a very popular procedure. Therefore, it is of interest to collect data on the significance of interlocking nail in different pattern of humerus shaft fracture. 30 patients with closed humeral shaft fracture participated in this study. The fractures were classified according to their descriptive location as proximal, m

Modified tension band wiring fixation for avulsion fractures of the calcaneus in osteoporotic bone: a review of three patients.

Calcaneal avulsion fractures are not uncommon, and they are probably more likely in patients with osteoporosis. Closed manipulation for this type of fracture often fails to achieve acceptable reduction, and open reduction and internal fixation are usually required. However, open reduction and internal fixation with either a lag screw or Steinmann pins do not provide satisfactory fixation in patients with diabetes and elderly patients because of the presence of porotic bone. Levi described a tension band fixation system used to treat a calcaneal avulsion fracture using a simple technique perfor

Minimally invasive plate osteosythesis of fractures of the radius and ulna in a primate.

A 25-year-old female mandrill (Mandrillus sphinx - a primate and part of the Old World monkey group) was presented with a mildly comminuted, diaphyseal, radial fracture associated with a transverse ulnar fracture. Minimally invasive plate osteosynthesis techniques were used to achieve fixation of both the radial and the ulnar fractures. First, closed fracture reduction was achieved with a distraction frame consisting of a motorized circular external skeletal fixator. Next, dual percutaneous radio-ulnar plating was performed using a 2.7 limited-contact dynamic compression plate on the cranial a…

Robot-assisted fracture reduction and treatment outcomes in type III and IV fragility fractures of the pelvis: A retrospective cohort study.

INTRODUCTION: This study compared robot-assisted fracture reduction (RAFR) with manual closed reduction followed by navigation-assisted fixation in surgically treated patients with fragility fractures of the pelvis (FFP) type III or IV. Outcomes after operative versus initial non-operative management were also examined. PATIENTS AND METHODS: This retrospective cohort study included 115 patients aged ≥ 60 years treated between March 2021 and June 2025: 39 underwent RAFR, 34 manual closed reduction with navigation-assisted fixation, and 42 initial non-operative treatment. The primary outcome wa

Femoral neck fractures: current management.

Femoral neck fractures are a commonly encountered injury in orthopaedic practice and result in significant morbidity and mortality. It is essential that surgeons are able to recognize specific fracture patterns and patient characteristics that indicate the use of particular implants and methods to effectively manage these injuries. Use of the Garden and Pauwels classification systems has remained the practical mainstay of femoral neck fracture characterization that help dictate appropriate treatment. Operative options include in situ fixation, closed or open reduction and internal fixation, he

Paediatric fracture dislocation of the forearm: an unusual presentation.

Traumatic injuries of the elbow in children pose a diagnostic challenge. They demand precise knowledge of radiographic anatomy, growth characteristics of healthy and injured bones as well as specific fracture dynamics in this age group. Dislocation of the elbow is rare in children, accounting for 3-6% of all childhood elbow injuries. Fractures at the distal humerus and proximal forearm have been extensively discussed in the literature. We describe a unique case of a 4-year-old child presenting with a traumatic radial head fracture and ulnar dislocation not previously reported. Diagnosis and re

Magnitude, Direction, Temporal Patterns, and Frequency of Loss of Distal Radius Fracture Reduction in Women 50 Years and Older.

PURPOSE: To determine the magnitude, direction, temporal patterns, and frequency of reduction loss following nonsurgical, closed treatment of distal radius fractures in women 50 years and older and correlate these observations with bone mineral density and age. METHODS: We reviewed registry data on 1,148 patients 50 years and older with distal radius fractures managed by closed reduction and cast immobilization. Radial inclination (RI), ulnar variance (UV), and radial tilt (RT) were measured immediately and at 1, 2, 3, 6, 9, and 12 weeks after reduction. Magnitude, direction, frequency, and p
