Psychiatric comorbidity in medication overuse headache patients with pre-existing headache type of episodic tension-type headache.

BACKGROUND: Medication overuse headache (MOH) mostly evolves from migraine and episodic tension-type headache (ETTH). Chronic tension-type h … n scores were significantly higher in Group E than Group C. The mostly diagnosed type was obsessive-compulsive personality disorder in all the three groups, and was statistically significant in Group M than Group C. CONCLUSION: Psychiatric comorbidity in MOH patients with pre-existing ETTH is common as in those with pre-existing migraine headache and MOH with regard to developi …

Symptom profiles in pediatric obsessive-compulsive disorder (OCD): the effects of comorbid grooming conditions.

This study sought to examine possible differences in phenomenological features and/or symptom severity of children diagnosed with obsessive-compulsive disorder (OCD) and a comorbid grooming condition (i.e., skin picking and trichotillomania). A total of 202 children receiving a primary diagnosis of OCD were classified into two distinct groups: (1) OCD alone (n=154) and (2) OCD plus a comorbid grooming condition (OCD+grooming; n=48). Analyses revealed that those children presenting with a comorbid grooming condition demonstrated different symptom profiles than those with OCD alone. In addition,

What is reflex sympathetic dystrophy?

In the literature there is no unanimity with respect to the diagnosis of reflex sympathetic dystrophy (RSD). Frequently, the diagnosis is established on mere clinical grounds. In our opinion, however, bone scintigraphy is of major importance for the diagnosis. Using this examination, true RSD can be clearly differentiated from other conditions which are incorrectly diagnosed and treated as RSD. If the bone scan is not suggestive of RSD, the clinical picture, radiological examination and vascular scan may lead to the correct diagnosis. This may be a pseudodystrophy, in which a hypovascularizati

Effectiveness of pharmacotherapy for pathological gambling: a chart review.

Although pathological gambling is a relatively common disorder, there exists only limited information regarding the effectiveness of pharmacotherapy for this illness. This study examines which medications may be effective, dose and duration of medication trials needed to achieve response, and possible predictors of response. Using a chart review, 50 adult outpatients with DSM-IV pathological gambling treated in clinical practice were assessed regarding response to a variety of medications, including augmentation strategies, and response to concomitant psychotherapy. All subjects received pharm

Reexamination of the relationship between birth order and obsessive-compulsive disorder.

In order to examine the hypothesis that being a firstborn or only child is specifically associated with obsessive-compulsive disorder, the birth order positions and sibship sizes of 62 patients with obsessive-compulsive disorder were compared with those of 60 agoraphobic and 92 depressed patients. No significant group differences were found for men, women, or both sexes combined. Results conflict with earlier findings which supported the hypothesized relationship between birth order status and development of obsessive-compulsive patterns in men. In addition to possible differences in methodolo

Depression: Predisposing factors.

Factors predisposing to a depressive state had been investigated in a prospective study on a stratified population, accounted for in the preceding article (Nyström & Lindegård (1975a) ). Of the 114 persons seeking psychiatric care during the period of observation, 37 had a depression as the main diagnosis. As compared to their 370 matched controls, these patients showed a significant overrepresentation of psychasthenic traits (subvalidity according to Sjöbring), a tendency to ruminate, shyness in company and lack of endurance, certain asthenic subclinical symptoms such as habitual fatigue and

Suicidal behavior in ADHD: the role of comorbidity, psychosocial adversity, personality and genetic factors.

Adult ADHD is associated with increased risk for suicide attempts, as indicated by investigations of population- and community-based cohorts. However, there is little data regarding suicide attempts in a clinical setting. To address this, we used a comprehensively phenotyped clinical adult ADHD (aADHD) cohort to assess to which extent comorbidity, psychosocial adversity, personality, and ADHD symptoms contribute to suicidal behavior in ADHD. Furthermore, we investigated a triallelic variation in the serotonin transporter-linked polymorphic region (5-HTTLPR), which has previously been associate

Aberrant functional connectivity of neural circuits associated with thought-action fusion in patients with obsessive-compulsive disorder.

BACKGROUND: Cognitive theories of obsessive-compulsive disorder (OCD) stress the importance of dysfunctional beliefs in the development and maintenance of the disorder. However, a neurobiological understanding of these cognitive models, including thought-action fusion (TAF), is surprisingly lacking. Thus, this functional magnetic resonance imaging study aimed to investigate whether altered functional connectivity (FC) is associated with the TAF paradigm in OCD patients. METHODS: Forty-one OCD patients and 47 healthy controls (HCs) participated in a functional magnetic resonance imaging study

Validity and clinical interpretability of Overall Anxiety Severity And Impairment Scale (OASIS).

BACKGROUND: The Overall Anxiety Severity and Impairment Scale is a brief generic measure for anxiety that encompasses frequency and intensity as well as behavioral and functional aspects of anxiety. This study was conducted to elucidate aspects of reliability, validity, and interpretability, such as equivalence of factor loadings across non-clinical and clinical populations, convergence and discriminance of related variables, and performance of detecting diagnostic and medical status of anxiety disorders. METHODS: Non-clinical and clinical Japanese populations were taken from a panelist pool

Hoarding, hermitage, and the law: why we love the collyer brothers.

Interest in hoarding behavior has intensified, as it works its way through DSM-V deliberations and treatment models. Meanwhile, both documentarians and fiction writers have embraced accounts of individuals with disposophobia and romanticized versions of the Collyer brothers, the Hermits of Harlem. In this article, I examine the range of media and professional attention given to hoarders and their problems and then focus on a potential role for forensic mental health professionals. The psycholegal problems of hoarders include health and zoning code violations that evolve into criminal charges,

Sexual behavior that is "out of control": a theoretical conceptual approach.

At this stage of our knowledge, it seems reasonable to assume that out-of-control sexual behavior results from a variety of etiological mechanisms associated with different behavioral patterns that share Goodman's two key features of addictive behavior: (1) a recurrent failure to control the sexual behavior; and (2) continuation of the behavior despite harmful consequences. Any overriding definition relevant to clinical management seems premature until we better understand the various patterns and their likely determinants. The concepts of "compulsivity" and "addiction" may have explanatory va

Specificity of emotional maintenance processes among contamination fears and blood-injection-injury fears.

Research evidence consistently demonstrates a relation between disgust and anxiety-related pathology. Despite ample evidence implicating a functional role of disgust in anxiety, limited research has focused on the process by which disgust influences anxiety. Recent evidence indicates that fear of responding with disgust predicts contamination fears, thus elucidating a process by which disgust contributes to contamination fears. In the current study, we tested whether fear of responding with disgust is specific to contamination fears or generalizes to blood-injection-injury (BII) fears. Undergr

Are trauma and dissociation related to treatment resistance in patients with obsessive-compulsive disorder?

OBJECTIVE: Previous research has indicated a relation between obsessive-compulsive disorder (OCD), childhood traumatic experiences and higher levels of dissociation that appears to relate to negative treatment outcome for OCD. The aim of the present study is to investigate whether childhood trauma and dissociation are related to severity of OCD in adulthood. We also intend to examine the association between treatment resistance, dissociation, and each form of trauma. METHODS: Participants included 120 individuals diagnosed with OCD; 58 (48.3 %) of them met the criteria for treatment-resistant

An effectiveness study of group therapy for anxiety disorders.

This work examines the transportability of group cognitive-behavioral therapy (GCBT) for a wide range of anxiety disorders to a college counseling center. Participants were 31 clients with a primary diagnosis of generalized anxiety disorder, panic disorder with agoraphobia, social phobia, specific phobia, or obsessive-compulsive disorder who identified dysfunction in one or more of eight daily function areas. Clients were not excluded on the basis of medication use, severity or frequency of panic attacks, age, or comorbidity. Of the 31 participants, there were 17 treatment completers, defined

Neurostimulation to Improve Cognitive Flexibility.

Cognitive flexibility, the capacity to adapt behaviors in response to changing environments, is impaired across mental illnesses, including depression, anxiety, addiction, and obsessive-compulsive disorder. Cortico-striatal-cortical circuits are integral to cognition and goal-directed behavior and disruptions in these circuits are linked to cognitive inflexibility in mental illnesses. We review evidence that neurostimulation of these circuits can improve cognitive flexibility and ameliorate symptoms, and that this may be a mechanism of action of current clinical therapies. Further, we discuss

COVID-19 and OCD: Potential impact of exposure and response prevention therapy.

This brief clinical review critically assesses the use of exposure and response prevention therapy (ERP) for patients with obsessive-compulsive disorder (OCD) in light of the COVID-19 pandemic. We discuss the ethical and practical considerations that clinicians employed in past infectious disease outbreaks, as well as general safety measures routinely practiced in the conduct of exposure therapy. During this time, concerns regarding the feasibility of ERP have emerged, especially with strict guidelines on social distancing and on following other preventative behaviors. While ERP may have to be
