Autologous serum eyedrops for dry eyes and epithelial defects: clinical and in vitro toxicity studies.

BACKGROUND/AIMS: Autologous serum drops have been reported to be beneficial in keratoconjunctivitis sicca (KCS) and persistent epithelial defects (PED). A clinical pilot study was carried out to examine these potential uses and in vitro toxicity testing on corneal epithelial cell cultures was performed to compare the effect of serum drops with unpreserved hypromellose (hydroxypropylmethylcellulose 0.3%). METHODS: Patients with KCS and PED, unresponsive to conventional treatment were recruited. Patients were examined before treatment, at 1 and 2 weeks after initiation, and then 2 weekly until

Traumatic globe rupture after deep anterior lamellar keratoplasty-A novel management technique.

A traumatic wound dehiscence can occur many years after surgery in 2-6% of all keratoplasties. Intraocular tissue prolapse can lead to severe visual loss. Deep anterior lamellar keratoplasty (DALK) leads to higher wound stability because of the intact Descemet membrane. We report a case of a novel management of wound dehiscence following DALK. A 59-year-old patient underwent uncomplicated DALK for advanced keratoconus. Eighteen months later he experienced a traumatic wound dehiscence (globe rupture) after a fall. Visual acuity decreased to light perception on the affected eye, the iris and pro

Renaissance of an Emerging Pathogen-Pythium insidiosum Keratitis: An Indian Perspective.

Pythium insidiosum keratitis is an aggressive, vision-threatening corneal infection increasingly recognized across India, particularly among agricultural workers during monsoon season. Frequently misclassified as fungal keratitis, it responds poorly to conventional antifungals because the oomycete has a cellulose- and β-glucan-rich wall and lacks ergosterol, leading to therapeutic delay and rapid stromal melt. Indian cohorts have refined a practical diagnostic signature such as tentacular or reticular peripheral infiltrates with guttering, early endothelial plaque and hypopyon, and minimal sat

A Review of Monkeypox Ocular Manifestations and Complications: Insights for the 2022 Outbreak.

Monkeypox (MPVX) infection has been associated with multiorgan presentations. Thus, monkeypox infection's early and late complications are of particular concern, prompting health systems to decipher threatening sequels and their possible countermeasures. The current article will review the clinical signs and symptoms of the present and former outbreaks, differential diagnoses, workup and treatment of the ocular manifestations of MPXV infection in detail. One of the uncommon yet considerable MPXV complications is ocular involvement. These injuries are classified as (1) more frequent and benign

Substance P promotes susceptibility to Pseudomonas aeruginosa keratitis in resistant mice: anti-inflammatory mediators downregulated.

PURPOSE: Studies have shown that blocking substance P (SP) binding to neurokinin 1 receptor with spantide I prevents Pseudomonas aeruginosa-induced corneal perforation in susceptible C57BL/6 mice. This study tested the effect of SP injection on the resistance response (cornea heals) of BALB/c mice. METHODS: The day before infection, mice were injected intraperitoneally with SP or PBS. Disease was graded by clinical score, slit lamp, plate count, real-time RT-PCR, and ELISA assays, and polymorphonuclear neutrophils (PMNs) were quantitated using a myeloperoxidase assay. In additional experiment…

Ocular injuries and the most common acute conditions in ophthalmology.

Ocular traumas include a wide range of injuries from trivial conditions to extensive perforating injuries that can lead to visual function impairment of various ranges and even to the loss of the eye itself. In terms of first aid, not only an ophthalmologist but also a physician of any specialty can effectively intervene and minimize the consequences of these conditions. Open globe injuries represent prognostically the most severe conditions which have to be treated by an experienced ophthalmologist. Closed injuries are most commonly manifested as corneal erosions, foreign bodies on the surfac
