Predicting wound healing outcomes: a comparative accuracy analysis of AI-driven indices and percent area reduction.

OBJECTIVE: Wounds represent a major global health and economic burden, with chronic wounds affecting millions annually and costing medical care providers over $126 billion in the USA alone. Current assessment tools, such as percent area reduction (PAR), are widely used but limited by subjectivity and suboptimal predictive accuracy, particularly for complex wound types. This study aimed to evaluate the performance of an artificial intelligence (AI)-powered Healing Index (HI) in predicting delayed healing compared with PAR, leveraging the growing integration of AI into healthcare to enhance woun

Development and validation of a deep learning markerless system for lower-limb kinematics in hip and knee osteoarthritis population.

With the advancement of deep learning technology, markerless systems have emerged as a cost-effective and user-friendly alternative to marker-based systems. However, most existing markerless systems are developed using datasets from healthy individuals, which limits their generalizability to patient populations. Therefore, this study developed a four-camera markerless system using a dataset of patients with osteoarthritis and validated its measurement accuracy in lower-limb kinematics. A total of 150 patients with hip or knee osteoarthritis were allocated to a training set (n = 120) and a test

When size matters: energy allocation and thermogenic mechanisms in very small mammals, African pygmy mice.

Thermoregulation is a major challenge for extremely small mammals such as the African pygmy mice Mus mattheyi and Mus minutoides, weighing less than 12 g. A previous study showed that these tiny mice exhibit different mitochondrial energy efficiency for ATP synthesis, with a higher efficiency in M. mattheyi (∼6 g) than in M. minutoides (∼10 g). This result suggests a lower mitochondrial heat production at rest in M. mattheyi, despite its lower body surface and inevitably greater heat loss from its body surface, than in M. minutoides. Consequently, a compensatory thermoregulatory strategy shoul

Designing a testing kit supply network for suspected COVID-19 cases under mixed uncertainty approach.

Motivated by the COVID-19 (C-19) pandemic and the challenges it poses to global health and the medical communities, this research aims to investigate the factors affecting of reduction health inequalities related to the C-19 to tackle the increasing number of outbreaks and their social consequences in such a pandemic. Hence, we design a COVID-19 testing kit supply network (C-19TKSN) to allocate various C-19 test kits to the suspected C-19 cases depending on the time between the emergence of their first symptoms and the time they are tested. In particular, this model aims to minimize the total

Study protocol for the DEFENDD trial: an RCT on the Dynamic Locking Blade Plate (DLBP) versus the Dynamic Hip Screw (DHS) for displaced femoral neck fractures in patients 65 years and younger.

BACKGROUND: The Dynamic Locking Blade Plate (DLBP) was recently introduced for fixation of displaced femoral neck fractures (FNF) and has been well received. Although the results of this implant in young patients are promising, the DLBP has not yet been compared to a standard device such as the Dynamic Hip Screw (DHS). The aim of this study is to compare the clinical outcome and costs of displaced FNF treated with internal fixation by means of either the DLBP or the DHS in patients up to 65 years of age. We hypothesize that the DLBP is superior compared to the DHS in terms of revision surgery

Can low-dose methotrexate reduce effusion-synovitis and symptoms in patients with mid- to late-stage knee osteoarthritis? Study protocol for a randomised, double-blind, and placebo-controlled trial.

BACKGROUND: Osteoarthritis (OA) is a common chronic disease in older adults. Currently, there are no effective therapies to reduce disease severity and progression of knee OA (KOA), particularly in mid- to late-stages. This study aims to examine the effect of methotrexate (MTX) on knee effusion-synovitis and pain in symptomatic patients with mid- to late-stage KOA. METHODS/DESIGN: This protocol describes a multicentre randomised placebo-controlled clinical trial aiming to recruit 200 participants with mid- to late-stage symptomatic KOA and with effusion-synovitis grade of ≥ 2. Participants wi

Jump starting shared medical appointments for diabetes with weight management: Rationale and design of a randomized controlled trial.

BACKGROUND: Rates of glycemic control remain suboptimal nationwide. Medication intensification for diabetes can have undesirable side effects (weight gain, hypoglycemia), which offset the benefits of glycemic control. A Shared Medical Appointment (SMA) intervention for diabetes that emphasizes weight management could improve glycemic outcomes and reduce weight while simultaneously lowering diabetes medication needs, resulting in less hypoglycemia and better quality of life. We describe the rationale and design for a study evaluating a novel SMA intervention for diabetes that primarily emphasiz

Probiotic Supplementation in Preterm: Feeding Intolerance and Hospital Cost.

We hypothesized that giving the probiotic strain Lactobacillus reuteri (L. reuteri) DSM 17938 to preterm, formula-fed infants would prevent an early traumatic intestinal inflammatory insult modulating intestinal cytokine profile and reducing the onset of feeding intolerance. Newborn were randomly allocated during the first 48 h of life to receive either daily probiotic (10⁸ colony forming units (CFUs) of L. reuteri DSM 17938) or placebo for one month. All the newborns underwent to gastric ultrasound for the measurement of gastric emptying time. Fecal samples were collected for the evaluation o

The Contributions of Onchocerciasis Control and Elimination Programs toward the Achievement of the Millennium Development Goals.

In 2000, 189 member states of the United Nations (UN) developed a plan for peace and development, which resulted in eight actionable goals known as the Millennium Development Goals (MDGs). Since their inception, the MDGs have been considered the international standard for measuring development progress and have provided a blueprint for global health policy and programming. However, emphasis upon the achievement of priority benchmarks around the "big three" diseases--namely HIV, tuberculosis (TB), and malaria--has influenced global health entities to disproportionately allocate resources. Meanw

Preventing preterm birth with progesterone: costs and effects of screening low risk women with a singleton pregnancy for short cervical length, the Triple P study.

BACKGROUND: Women with a short cervical length in mid-trimester pregnancy have a higher risk of preterm birth and therefore a higher rate of neonatal mortality and morbidity. Progesterone can potentially decrease the number of preterm births and lower neonatal mortality and morbidity. Previous studies showed good results of progesterone in women with either a history of preterm birth or a short cervix. However, it is unknown whether screening for a short cervix and subsequent treatment in mid trimester pregnancy is effective in low risk women. METHODS/DESIGN: We plan a combined screen and tre

PROthrombin complex concentrate versus fresh frozen Plasma for bleeding in adults undergoing HEart SurgerY (PROPHESY-2 trial): a phase III, randomised control trial in England and Wales.

BACKGROUND: Fresh frozen plasma (FFP) is the standard treatment for severe bleeding following cardiac surgery. Despite increasing use of prothrombin complex concentrate (PCC) for coagulopathic bleeding in preference to FFP in the UK, the evidence comparing FFP versus PCC in this setting is lacking. HYPOTHESIS: In adults who develop severe bleeding, PCC is superior to FFP in reducing a composite of mortality, organ failure, or infection up to 90 days following cardiac surgery, and is more cost-effective. METHODS: Phase III pragmatic, multicentre, parallel group, superiority, non-blinded, open

Evaluation of the efficacy and safety of endoscopic band ligation in the treatment of bleeding from mild to moderate gastric varices type 1.

BACKGROUND: According to practice guidelines, endoscopic band ligation (EBL) and endoscopic tissue adhesive injection (TAI) are recommended for treating bleeding from esophagogastric varices. However, EBL and TAI are known to cause serious complications, such as hemorrhage from dislodged ligature rings caused by EBL and hemorrhage from operation-related ulcers resulting from TAI. However, the optimal therapy for mild to moderate type 1 gastric variceal hemorrhage (GOV1) has not been determined. Therefore, the aim of this study was to discover an individualized treatment for mild to moderate GO

Selective laser trabeculoplasty versus eye drops for first-line treatment of ocular hypertension and glaucoma (LiGHT): a multicentre randomised controlled trial.

BACKGROUND: Primary open angle glaucoma and ocular hypertension are habitually treated with eye drops that lower intraocular pressure. Selective laser trabeculoplasty is a safe alternative but is rarely used as first-line treatment. We compared the two. METHODS: In this observer-masked, randomised controlled trial treatment-naive patients with open angle glaucoma or ocular hypertension and no ocular comorbidities were recruited between 2012 and 2014 at six UK hospitals. They were randomly allocated (web-based randomisation) to initial selective laser trabeculoplasty or to eye drops. An object

The City of Johannesburg can end AIDS by 2030: modelling the impact of achieving the Fast-Track targets and what it will take to get there.

INTRODUCTION: In 2014, city leaders from around the world endorsed the Paris Declaration on Fast-Track Cities, pledging to achieve the 2020 and 2030 HIV targets championed by UNAIDS. The City of Johannesburg - one of South Africa's metropolitan municipalities and also a health district - has over 600,000 people living with HIV (PLHIV), more than any other city worldwide. We estimate what it would take in terms of programmatic targets and costs for the City of Johannesburg to meet the Fast-Track targets, and demonstrate the impact that this would have. METHODS: We applied the Optima HIV epidem
