Reconstructing tumor history in breast cancer: signatures of mutational processes and response to neoadjuvant chemotherapy⋆.

BACKGROUND: Different endogenous and exogenous mutational processes act over the evolutionary history of a malignant tumor, driven by abnormal DNA editing, mutagens or age-related DNA alterations, among others, to generate the specific mutational landscape of each individual tumor. The signatures of these mutational processes can be identified in large genomic datasets. We investigated the hypothesis that genomic patterns of mutational signatures are associated with the clinical behavior of breast cancer, in particular chemotherapy response and survival, with a particular focus on therapy-resi

A novel frameshift variant in MSH2 (p.Q170Rfs4) associated with suspected Lynch syndrome in a Chinese family.

OBJECTIVE: This study aimed to characterize the pathogenic variants in three colon cancer families suspected of Lynch syndrome (LS), providing experimental evidence for precision screening and genetic counseling of the disease. METHODS: Three suspected LS families were first identified, and subsequently, immunohistochemical analysis was performed on colon tissue samples from probands to assess the expression of four DNA mismatch repair proteins. Whole-exome sequencing was conducted to screen for potential pathogenic variants within the families. The SWISS-MODEL online platform was used to pre…

Molecular characterization of mismatch repair deficient tumors in young Jordanian patients.

Mismatch repair deficiency (MMRD) contributes substantially to early-onset colorectal carcinoma (CRC) and pediatric malignancies in Jordan. Approximately 19% of CRCs diagnosed in adults younger than 45 years exhibit MMRD by immunohistochemistry (IHC). In pediatric tumors, MMRD accounts for 39% of high-grade gliomas (HGGs) and 44% of CRCs, likely reflecting the increased prevalence of constitutional MMRD (CMMRD) in this highly consanguineous population. However, the molecular characterization of MMRD-associated tumors in the Middle East remains limited. We analyzed 14 Jordanian patients (< 45 y

Targeting molecular residual disease and immune escape in colorectal cancer recurrence: biomarker-guided immunotherapy and cell therapies.

Colorectal cancer (CRC) recurrence after curative-intent treatment remains a major clinical challenge. Conventional postoperative risk stratification relies on clinicopathological variables, whereas recurrence is increasingly understood as a dynamic biological process shaped by molecular residual disease (MRD), immune editing, tumor microenvironmental remodeling and therapy-resistant cellular states. Circulating tumor DNA (ctDNA) has emerged as a clinically relevant MRD biomarker for identifying patients at high risk of relapse and for refining adjuvant-treatment decisions. At the same time, i

Temporal transcriptional regulation and genetic adaptation strategies in Nannochloropsis oceanica during long-term CO2 acclimation.

To overcome the limited tolerance of microalgal strains to high CO2 levels and the non-directional nature of traditional physicochemical mutagenesis, long-term multi-cycle acclimation under 10%-100% CO2 was employed to domesticate Nannochloropsis oceanica with the aim of elucidating its temporal transcriptional regulation and genetic adaptation strategies. Under multi-cycle acclimation at 55% CO2, chloroplast-associated genes underwent mutations that promoted accelerated chloroplast development, enhanced photosystem II efficiency, enhanced Calvin cycle activity, and reduced photodamage. The te

Optimizing immunotherapy in advanced gastric cancer: established and emerging biomarkers for precision patient selection.

mmune checkpoint inhibitors have reshaped the therapeutic landscape of advanced gastric and gastroesophageal junction cancer, yet durable benefit remains restricted to biologically selected patient subgroups. Optimizing immunotherapy, therefore, requires a disease-specific, clinically practical, and critically interpreted biomarker framework rather than reliance on any single predictive marker. This review synthesizes established and emerging biomarkers for immune checkpoint inhibitor selection in advanced, metastatic, and perioperative gastric cancer, with emphasis on programmed death-ligand

Establishment of a human ovarian endometrioid carcinoma cell line by constitutive expression of cyclin-dependent kinase 4, cyclin D1 and telomerase reverse transcriptase.

Only a few human ovarian endometrioid carcinoma cell lines are currently available, partly due to the difficulty of establishing cell lines from low-grade cancers. Here, using a cell immortalization strategy consisting of i) inactivation of the p16INK4a-pRb pathway by constitutive expression of mutant cyclin-dependent kinase 4 (R24C) (CDK4R24C) and cyclin D1, and ii) acquisition of telomerase reverse transcriptase (TERT) activity, we established a human ovarian endometrioid carcinoma cell line from a 46-year-old Japanese woman. That line, designated JFE-21, has proliferated continuously for ov

First Middle East and North Africa report of an EPCAM-MSH2 deletion in two Iranian Lynch syndrome families: a case report.

Lynch syndrome is an inherited autosomal-dominant cancer-predisposition condition that results from germline defects in the DNA mismatch-repair (MMR) pathway or inactivation of the EPCAM gene. Variants involving EPCAM are detected in roughly 1-3 % of Lynch syndrome cases, whereas combined EPCAM-MSH2 deletions remain very uncommon. We investigated two Iranian families from the same village, each with multiple members affected by colorectal cancer (CRC), to identify the underlying genetic causes. In Family A, colon tumor immunohistochemistry showed loss of MSH2 and MSH6 expression in formalin-fi

Cancer risks for MSH6 pathogenic variant carriers.

INTRODUCTION: Lynch syndrome (LS) is a hereditary cancer syndrome caused by pathogenic variants (PVs) in DNA mismatch repair genes, including MSH6. Although MSH6-associated LS (MSH6-LS) is known to increase the risk of several cancers, precise risk estimates-particularly for non-colorectal cancers-remain uncertain. This limits personalised clinical guidance for MSH6-LS. This study aims to refine cancer risk estimates for individuals with pathogenic or likely pathogenic MSH6 variants. METHODS: A retrospective cohort study was conducted using data from 360 Dutch families, comprising 1117 MSH6 P…

High Prevalence of Potential Molecular Therapeutic Targets in Poorly Differentiated Thyroid Carcinoma.

Poorly differentiated thyroid carcinoma (PDTC) is a rare thyroid cancer with aggressive clinical course and peculiar clinical/pathological characteristics but lacking effective therapeutic options, when surgery is not curative. We aimed at the molecular characterization of PDTC with a specific focus on the identification of potential therapeutic targets. A series of PDTC cases was selected from a multi-institutional network. Fifty-nine samples underwent wide targeted DNA and RNA next-generation sequencing (NGS) testing and immunohistochemical analysis for mismatch repair (MMR) proteins. Gene f

The Role of Microsatellite Instability in Endometrial Hyperplasia and Risk of Carcinoma Development.

BACKGROUND: Endometrial hyperplasia (EH) represents a precursor lesion in the development of endometrial carcinoma, particularly the endometrioid subtype. Among the molecular pathways involved, microsatellite instability (MSI) resulting from DNA mismatch repair (MMR) deficiency has gained increasing attention as an early event in endometrial carcinogenesis. OBJECTIVE: This study aimed to evaluate the expression of key MMR proteins (MLH1, PMS2, MSH2, and MSH6) in endometrial hyperplasia without atypia and endometrial atypical hyperplasia/endometrioid intraepithelial neoplasia (EAH/EIN) to dete…

NucS Thr74Ile confers mutator phenotype in a globally disseminated clinical Mycobacterium abscessus subsp. abscessus clone.

OBJECTIVE: The emergence of mutations conferring resistance to first-line drugs, macrolides and amikacin, is a major cause of treatment failure in Mycobacterium abscessus complex (MABC) infections; however, mutator strains predisposed to developing drug-resistance mutations remain uncharacterized. NucS, a key DNA mismatch repair protein, maintains mycobacterial genomic stability. This study investigated whether naturally occurring NucS polymorphisms contribute to mutator phenotypes in clinical MABC strains. METHODS: The whole genome data of clinical MABC isolates obtained from public database…

Evaluation of methylated DCR1 as a biomarker for response to adjuvant irinotecan-based therapy in stage III colon cancer: cancer and leukaemia Group B 89803 (Alliance).

Aberrantly methylated genes contribute to the landscape of epigenetic alterations in colorectal adenocarcinoma. The global CpG Island methylator phenotype (CIMP) and individually methylated genes are potential prognostic/predictive biomarkers. Research suggests an association between methylated DCR1 (mDCR1) and lack of benefit with irinotecan (IFL) treatment. We assessed the association between DCR1 methylation status and survival in patients receiving adjuvant fluorouracil/ leucovorin (5-FU/LV) or IFL. We analysed data from patients with stage III colon adenocarcinoma randomly assigned to adj

Genomic profile of pseudomyxoma peritonei analyzed using next-generation sequencing and immunohistochemistry.

Pseudomyxoma peritonei (PMP) is a relatively rare clinical syndrome characterized by neoplastic epithelial cells growing in the peritoneal cavity and secreting mucinous ascites. Our aim was to explore the molecular events behind this fatal but under-investigated disease. We extracted DNA from 19 appendix-derived PMP tumors and nine corresponding normal tissues, and analyzed the mutational hotspot areas of 48 cancer-related genes by amplicon-based next-generation sequencing (NGS). Further, we analyzed the protein expression of V600E mutated BRAF, MLH1, MSH2, MSH6 and p53 from a larger set of PM
