Open versus closed lateral sphincterotomy performed as an outpatient procedure under local anesthesia for chronic anal fissure: prospective randomized study of clinical and manometric longterm results.

BACKGROUND: The aim of this prospective randomized trial was to compare the effectiveness and morbidity of open and closed sphincterotomy performed as an outpatient procedure under local anesthesia in the treatment of chronic anal fissure. STUDY DESIGN: Eighty patients with chronic anal fissure were treated by open (group 1) or closed lateral internal sphincterotomy (group 2). Clinical and manometric results were recorded at the time of admission and at 1-week, 2-month, 6-month, 1-year, and 2-year followup visits. RESULTS: Overall healing after 2 years was 92.5% in the open sphincterotomy gr

Modified soave pull-through for Hirschsprung's disease: intraoperative internal sphincterotomy.

BACKGROUND/PURPOSE: Anorectal achalasia (AA) may persist after pull-through (PT) for Hirschsprung's disease (HD), which may cause postoperative enterocolitis (POE) and constipation. The authors modified the Soave PT (modified Soave PT, MSPT) to eliminate AA, and present their results. METHODS: This was a 16-year retrospective review of 43 patients with histologically proven HD of the rectosigmoid or sigmoid colon treated by MSPT. The MSPT involves excision of the posterior rectal cuff and an intraoperative internal sphincterotomy, allowing the PT colon to fit nicely. RESULTS: Mean age at MSP

Hepatic parenchymal perfusion abnormalities after pancreaticobiliary surgery. Evaluation with dynamic helical CT.

PURPOSE: To evaluate perfusion abnormalities of the liver after pancreaticobiliary surgery. MATERIAL AND METHODS: We retrospectively evaluated 128 patients with pancreaticobiliary malignant tumors who had been examined both before and after surgery by means of helical CT of the liver. An infusion of 3 ml/s of 60% nonionic contrast material was followed by helical CT of the liver in a sequential arterial phase, portal venous phase, and equilibrium phase. RESULTS: Of 128 patients, we followed 97. In 21 patients (22%) we found 47 lesions with perfusion abnormalities that were detected 1-33 mont

Perioperative outcomes of robotic-assisted gastrointestinal surgery during early program implementation: a retrospective single-center study.

Robotic-assisted gastrointestinal (GI) surgery offers technical advantages over conventional minimally invasive approaches; however, evidence from low- and middle-income countries remains limited. This study evaluated the safety, feasibility, and early perioperative outcomes of implementing a robotic-assisted GI surgery program in a resource-constrained setting. A retrospective observational cohort study was conducted at a tertiary care center in Pakistan. Consecutive adult patients who underwent robotic-assisted GI procedures between January 2022 and October 2025 were included. Clinical, oper

In experienced hands, does the robotic platform impact operative efficiency? Comparison of the da Vinci Si versus Xi robot in colorectal surgery.

Robotic surgical platforms have aimed to assist surgeons in meeting the challenges accompanying minimally invasive colon and rectal surgical procedures. Since early adoption over decade ago, there has been an evolution of the robotic platforms aimed at improved usability. We hypothesize, when compared to da Vinci Si, the Xi will allow for greater efficiency and result in shorter operative times. A case log review was completed for a single colorectal surgeon with 7 years' robotic experience, totaling over 800 cases. Patients were identified using CPT code S2900 (robot) for the most recent 15 m

Additional secure circular suture during sphincteroplasty - preliminary results on the efficacy of fecal incontinence surgery in urogynecological patients.

OBJECTIVES: The paper is a ten case series study presenting women with complex pelvic floor disorders involving fecal incontinence (FI) with stress urinary incontinence or pelvic organ prolapse. Our study aimed at ascertaining whether FI-induced sphincteroplasty with an additional secure circular suture around the external anal sphincter muscle (EAS) may improve long term success rates. MATERIALS AND METHODS: Twelve patients had scheduled urogynecological surgery and overlapping sphincteroplasty with the placement of an additional circular suture around the EAS. Of these, the status of ten wo

Hybrid abdominal robotic approach with conventional transanal total mesorectal excision (TaTME) for rectal cancer: feasibility and outcomes from a single institution.

Total mesorectal excision (TME) is currently recognised as the standard of care for patients with rectal cancer. Complete TME is known to be associated with lower rates of recurrence. Robotic and endoscopic TaTME approaches are reported to offer excellent proximal and distal rectal dissection into the TME plane, however, combining both approaches in a hybrid procedure could potentially optimise visualisation of the dissection plane and confer improved circumferential and distal margin rates. The aim of this study was to analyse the feasibility of a hybrid robotic abdominal approach with conven

Insight into the da Vinci® Xi - technical notes for single-docking left-sided colorectal procedures.

BACKGROUND: The adoption of robot-assisted laparoscopic colorectal surgery has been hampered by issues with docking, operative duration, technical difficulties in multi-quadrant access, and cost. The da Vinci® Xi has been designed to overcome some of these limitations. We describe our experience with the system and offer technical insights to its application in left-sided colorectal procedures. METHODS: Our initial series of left-sided robotic colorectal procedures was evaluated. Patient demographics and operative outcomes were recorded prospectively using a predefined database. RESULTS: Bet

Perioperative fluid prescription, complications and outcomes in major elective open gastrointestinal surgery.

Perioperative fluid therapy and associated outcomes of patients undergoing major elective open gastrointestinal surgery are poorly understood. This study measured perioperative fluid therapy, complication rates and outcomes for major elective open gastrointestinal surgery in a tertiary care hospital. We obtained demographic data, operative details, fluid prescription, complications and outcomes in 100 patients. Patients were elderly and had multiple comorbidities. Median delivered intraoperative fluid volume was 4.2 litres, followed by 6.3 litres over the subsequent 24 hours. Perioperative flu

Laparoscopic Ladd procedure: a minimally invasive approach to malrotation without midgut volvulus.

The management of intestinal malrotation without midgut volvulus is controversial. Some advocate the Ladd procedure in all patients with malrotation, whereas others propose a more selective approach. We attempted the laparoscopic Ladd procedure on nine patients who were diagnosed with intestinal malrotation without volvulus. Patient records were retrospectively reviewed. Data were collected on patient presentation, operative procedure, hospital course, and outcome. The laparoscopic Ladd procedure was successfully completed in eight patients (aged 10 weeks to 25 years). One patient required con

Anal canal duplication: experience at a single institution and literature review.

PURPOSE: Anal canal duplication (ACD) is an extremely rare congenital intestinal anomaly. There are not many reports in the English literature, with just a few from each institution. The aim of this study was to describe the clinical characteristics, surgical treatment, and outcome of ACD in pediatric cases at a single institution. METHOD: The medical records of ACD patients we treated from 1988 to 2009 were reviewed retrospectively. RESULTS: There were ten cases, all females. Nine were asymptomatic without any history of perianal infection, identified only incidentally at routine check-up a

Comparison of the clinical presentations of ingested foreign bodies requiring operative and nonoperative management.

Foreign body ingestion is commonly seen in emergency departments. Although most cases have a clear history, foreign bodies in the gastrointestinal tract can be an unexpected finding after operations for other conditions. This study compares the clinical presentations and outcomes for patients requiring or not requiring operations. Between January 1998 and December 2001, 80 patients with foreign body ingestion were included and divided into two groups. Specifically, group 1 patients were managed nonoperatively, and group 2 patients were managed operatively. The patient demographics, symptoms, f

Surgical intervention of intestinal malrotations in paediatric patients without other congenital anatomical abnormalities: overview from a single center.

The subject of malrotation in infants and children without other congenital anatomical abnormalities is reviewed from the perspective of experience with 97 patients operated in 11 years. Fifty-five patients were younger than 6 weeks at operation. They often presented with bilious vomiting, in contrast to older children who presented with non-bilious vomiting or feeding problems. Patients younger than 6 weeks were operated more often acutely than older patients. Volvulus was more common in infants younger than 6 weeks. Two patients with a resulting short bowel syndrome died. In 73 of the surviv

Minimally Invasive Surgery in the Management of Anorectal Malformations.

Imperforate anus, a variant of anorectal malformation (ARM), is a common congenital anomaly requiring surgical attention in the newborn period. It may present with a variety of anatomic configurations, largely dependent on the presence and location of a fistula. The location (or characteristics) of a fistula, which usually lies between the gastrointestinal tract and the genitourinary tract or perineum, is often used in determining the type and timing of operative management. This article discusses the work-up and management, modes of treatment and their postoperative outcomes, and continued co
