High-velocity gunshot-related humeral fractures managed with circular external fixation in a resource-limited setting, Samburu County, Kenya: Surgical technique highlights from a case series.

INTRODUCTION: High-velocity gunshot-related humeral fractures are complex injuries often associated with a high rate of nonunion, infection, and poor functional outcome. In low- and middle-income countries these injuries are further complicated by delayed presentation and implant unavailability. Staged management with spanning external fixation followed by internal fixation with intramedullary nail or plate has been well described in the literature; however, the use of a circular frame as a definitive treatment has been very seldom mentioned. MATERIALS AND METHODS: Six high-velocity gunshot-r

Proximal Humerus Internal Locking System (PHILOS) Plating Versus External Fixation Using Kirschner Wires (K-Wires) With Frames for Proximal Humerus Fractures: A Prospective Comparative Study.

BACKGROUND: Proximal humerus fractures are among the most common fractures involving the shoulder and frequently require surgical intervention when displaced or unstable. Among the available fixation techniques, open reduction and internal fixation using proximal humerus internal locking system (PHILOS) plates and external fixation using Kirschner wires (K-wires) and frames are commonly employed. However, evidence comparing their clinical and functional outcomes remains limited, particularly in the Indian population. METHODS: This prospective comparative observational study was conducted at G

Functional and radiological outcome in patients with acute Monteggia fracture treated surgically: a comparison between closed reduction and external fixation versus closed reduction and elastic stable intramedullary nailing.

The aim of this study was to compare the clinical and radiological outcomes in children with acute Monteggia fracture treated by two different surgical techniques. Twenty-six children treated by either closed reduction and external fixation (group A) or elastic stable intramedullary nails (group B) were retrospectively reviewed. Clinical outcome was evaluated with the Quick DASH questionnaire. Overall, both surgical techniques provided similar excellent healing rates and good clinical outcomes (P > 0.05), as well as radiological outcomes (P > 0.05). Radiographically, no secondary displacement,

Repair of Tibiotarsal Rotation in 7 Chukar Partridges (Alectoris chukar) and 12 Domestic Pigeons (Columba livia domestica) with Type-2 External Skeletal Fixator Intramedullary Pin Tie-in.

Rotational deformities of the long bones affect various avian species. Tibiotarsal rotation may cause the leg to deviate up to 180° from the dorsoplantar axis in a matter of days, thus preventing the birds from walking freely and leading to the inability to stand. In this study, tibiotarsal rotation observed in pigeons and partridges was managed by creating a closed fracture in the tibiotarsus and then, following reduction, stabilizing it with an intramedullary tie-in Type 2 external skeletal fixation system. Functional healing was achieved in 12 pigeons (Columba livia domestica; mean healing

Operative management of supination-external rotation type trimalleolar equivalent ankle fractures: reassessing the role of deltoid ligament repair and impact of posterior malleolar fixation on postoperative syndesmotic widening.

PURPOSE: The role of deltoid ligament (DL) repair in supination-external rotation (SER)-type trimalleolar equivalent ankle fractures (TEAFs) remains controversial. Additionally, it remains unclear whether posterior malleolar fixation alone can prevent postoperative syndesmotic widening. Therefore, this study aimed to determine the optimal combination of surgical treatments for SER-type TEAFs involving the lateral and posterior malleoli and the DL. METHODS: We retrospectively evaluated 77 patients who underwent surgical treatment for SER-type TEAFs involving Weber type B lateral malleolar frac

Effect of closed reduction and external fixation in elderly patients with anterior shoulder dislocation combined with humeral surgical neck fracture: a retrospective cohort study based on fracture-dislocation classification.

BACKGROUND: Anterior shoulder dislocation combined with humeral surgical neck fracture is a severe injury that often requires emergency surgery. However, for some patients, such as those of advanced age or with surgical contraindications, closed reduction can be considered. This retrospective cohort study aimed to confirm the feasibility and clinical outcomes of the closed reduction technique with effective external fixation for the treatment of anterior shoulder dislocation combined with humeral surgical neck fracture in elderly patients. METHOD: From January 2010 to March 2024, a total of 5

Comparative study of the results of compound tibial shaft fractures treated by Ilizarov ring fixators and limb reconstruction system fixators.

PURPOSE: Ilizarov ring fixator and limb reconstruction system (LRS) fixators have been used in the management of complex tibial fractures with severe soft tissue injuries, compound tibial fractures, and infected tibial nonunion for which conventional internal fixation cannot be contemplated. Fracture union and distraction osteogenesis can be done simultaneously with these external fixators, allowing early weight bearing. Several previous studies have shown almost equal results of rail and ring fixators for the compound tibial shaft fractures. Thus we performed a prospective study to evaluate t…

A finite element study: Finding the best configuration between unilateral, hybrid, and ilizarov in terms of biomechanical point of view.

In an open fracture, the external fixator is one of the definitive treatment options as it could provide the initial stabilisation of the fractured bone. Limited literature discussing on the biomechanical stability between unilateral, hybrid and Ilizarov configurations, principally in treating a femoral fracture. Thus, this study aims to analyse the biomechanical stability of different external fixators via the finite element method (FEM). The present study portrays that different configurations of fixators possess different biomechanical stability, hence leading to different healing rates and…

INTRODUCTION: Management of infected extra-articular long bone fractures without bone loss remains a challenging orthopedic problem. Traditional methods involving external fixators can be associated with discomfort, pin site infections, and delayed union. Supercutaneous plating using metaphyseal locking plates offers a promising alternative. To evaluate the clinical and radiological outcomes of locking plate fixators as a definitive treatment for infected extra-articular tibial and femoral fractures without bone loss. MATERIALS AND METHODS: This prospective study included 15 patients treated …

Male sex, Gustillo-Anderson type III open fracture and definitive external fixation are risk factors for a return to the or following the surgical management of geriatric low energy open ankle fractures.

INTRODUCTION: Open ankle fractures in geriatric (age > 60 years) patients are a source of significant morbidity and mortality. Surgical management includes plate and screw fixation (ORIF), retrograde hindfoot nail (HFN), definitive external fixation (ex-fix) and below knee amputation. However, each modality poses unique challenges for this population. We sought to identify predictors of unplanned OR and short-term mortality after geriatric open ankle fractures managed by our service. MATERIALS AND METHODS: In an IRB-approved protocol, we evaluated patients over 60 years of age managed for a l
