Concomitant injuries and its influence on functional outcome after traumatic brain injury.

PURPOSE: This study aims to describe the presence and severity of extracranial concomitant injuries in traumatic brain injury (TBI) patients and to ascertain their effect on long-term functional outcome. METHOD: A retrospective cohort study was performed in a single-center recruiting patients with first episode of TBI. The types and severity of extracranial concomitant injuries were documented for patients who fulfilled the inclusion criteria. Injuries with Abbreviated Injury Scale (AIS) grade 3-6 were considered to be high grade. A cross-sectional functional assessment was conducted at 18 mo

Delayed Facial Nerve Palsy After Open Reduction of an Isolated Zygomaticomaxillary Complex Fracture.

Facial nerve paralysis is a devastating complication which can occur after a variety of otolaryngic procedures, including facial trauma repair. The frontal and marginal branches are most often placed at risk. However, facial nerve paralysis is not typically described as a risk in most uncomplicated facial trauma repairs of the zygomaticomaxillary complex (ZMC). In particular, buccal branch injury has not been described in a delayed setting following repair of the ZMC. The authors present a patient of delayed buccal branch paralysis following a simple ZMC repair which has not been previously re

Surgical exposure of the facial nerve: contemporary transtemporal techniques.

Contemporary surgical exposure of the facial nerve has as its aims adequate exposure of the facial nerve at any point in its course, with preservation of hearing and vestibular function, and without further injury to the facial nerve and the necessity for producing a mastoid cavity. Today it is surgically possible to expose the facial nerve from the brainstem to its terminal branches in the face. A decade of experience has shown that if the operator is armed with intimate anatomic knowledge of the temporal bone and middle and posterior fossae, and if the principles of facial nerve surgery outl

Two-Point Versus Three-Point Fixation in Zygomaticomaxillary Complex (ZMC) Fractures: A Narrative Review.

Zygomaticomaxillary complex (ZMC) fractures are among the most prevalent facial injuries encountered during maxillofacial surgery because of the anatomical prominence and structural significance of the zygoma. These fractures often result from high-energy impacts, such as road traffic accidents, assaults, falls, and sports injuries, predominantly affecting males in their third to fourth decades of life. The management of ZMC fractures requires restoring facial aesthetics, orbital integrity, and masticatory function while minimizing surgical morbidity. Open reduction and internal fixation (ORIF…

Overexpression of c-Jun inhibits erastin-induced ferroptosis in Schwann cells and promotes repair of facial nerve function.

Myelin undergoes various changes after nerve injury, and c-Jun has a close relationship with Schwann cells (SCs). However, it remains unclear whether c-Jun can be involved in nerve repair by regulating ferroptosis. To explore this, we first set up a facial nerve injury model and detected the changes of ferroptosis-related proteins and c-Jun by immunofluorescence and Western blot. Then, we cultured RSC 96 and pSCs, and studied the potential regulatory relationships by a combination of experimental methods such as CCK-8, ELISA, immunofluorescence, qRT-PCR, Western blot and viral transfection. Nerve injury-induced pain in the trigeminal system.

This article reviews some recent findings on peripheral mechanisms related to the development of oro-facial pain after trigeminal nerve injury. Chronic injury-induced oro-facial pain is not in itself a life-threatening condition, but patients suffering from this disorder undoubtedly have a reduced quality of life. The vast majority of the work on pain mechanisms has been carried out in spinal nerve systems. Those studies have provided great insight into mechanisms of neuropathic spinal pain, and much of the data from them is obviously relevant to studies of trigeminal pain. However, it is now

Fractures of the temporal bone--chain incongruencies.

PURPOSE: The sequela of a temporal bone fracture include hearing loss, vestibular dysfunction, and facial nerve injury. A large series of patients with temporal bone fractures are reviewed to correlate mechanism of injury, fracture location, and outcome. PATIENTS AND METHODS: A retrospective analysis of patients evaluated between 1978 and 1987 for temporal bone fractures at two Swedish clinics was undertaken. Assessment of auditory, vestibular, and facial nerve function is reported. Follow up varied from 3 months to 12 years. RESULTS: A total of 72 temporal bone fractures were encountered. E

Fractures of the facial skeleton in children.

Fractures of the facial skeleton in children are uncommon. This study presents the results of 139 children who sustained a total of 161 such fractures and were admitted to the Royal Hospital for Sick Children, Edinburgh, between January 1983 and December 1992. The male to female ratio was 3:1 and the highest incidence was at age 10 years. Analysis of fracture patterns showed that despite differences in anatomy, the fracture patterns were similar to those occurring in adults, but the relative proportion of each fracture type was different in children. Nasal fractures occurred most frequently (5

[Facial nerve identification in the translabyrinthine approach: an alternative method].

Since the abrupt drop in the mortality rate as a result of the introduction of microsurgical dissection techniques in the treatment of acoustic neuromas, surgeons have concentrated their efforts on preserving hearing and facial nerve function. In the translabyrinthine approach, identification of the facial nerve at the fundus of the internal auditory canal is an important step for subsequent dissection. However, the identification techniques available to date carry with them some potential risk of facial nerve injury when performed by inexperienced surgeons. In addition, they are time-consumin

Total Transcanal Endoscopic Facial Nerve Decompression for Traumatic Facial Nerve Palsy.

A few approaches can be used to decompress traumatic facial nerve paralysis including the middle cranial fossa approach or transmastoid approach depending on the site of injury. In some specific situation of treating traumatic facial nerve palsy whose injured site was confined from the geniculate ganglion to the second genu, transcanal endoscopic approach for facial nerve decompression can be used. We performed two cases of total endoscopic transcanal facial nerve decompression in patients with traumatic facial nerve palsy. After a six month follow-up, both patients showed improvement in facia

Effects of strength training on neuromuscular facial rehabilitation.

OBJECTIVE: Physical trauma is the third leading cause of facial nerve damage, which can disrupt communication, social interaction and emotional expression. The objective of this report was to investigate the effects of facial muscle exercise as a stand-alone treatment in a young adult with unilateral facial nerve damage 13-years post-onset. METHOD: This single case study examines the long-term results of a 7-week intensive facial exercise programme followed by a 16-week moderate facial exercise programme. RESULTS: Intensive exercise increased facial strength and upper lip elevation on the af

Patient Perception of Education, Care Coordination, and Psychological Distress After Developing Facial Paralysis: A Qualitative Study.

IMPORTANCE: The management of vestibular schwannoma may include observation, microsurgical resection, or radiation of a tumor near the facial nerve. Injury to the facial nerve can result in facial paralysis with major functional, social, and psychological sequelae, and the experiences of patients after paralysis are not well studied. OBJECTIVE: To (1) identify patient preparedness for developing facial paralysis and how well their care is coordinated following its development and (2) present in their own words outcomes of facial paralysis in terms of physical health, emotional health, self-pe

Decision making in head injury management in the Edwin Smith Papyrus.

The Edwin Smith Papyrus (circa 1650-1550 BC) is a didactic trauma treatise of major interest to neurosurgery, as it deals primarily with cranial and spine injuries. Information regarding the patient's condition is conveyed in the papyrus with sufficient clarity to allow a clinical assessment of each injury. The ancient Egyptian physician/teacher lists the key diagnostic elements in each case, and then pronounces his opinion of the treatment potential in one of three verdicts: 1) "a medical condition I can treat;" 2) "a medical condition I can contend with;" or 3) "a medical condition you will

Facial Trauma in Sports.

Injuries resulting from facial trauma are common in all sports. Athlete-to-athlete contact, falls, and blows from equipment account for the majority of these events. Appropriate knowledge of basic science, relevant anatomy, and clinical skills is required to provide the correct medical care. While true medical emergencies are infrequent, a prompt accurate diagnosis is essential in developing targeted management and return to play options.
