[Impact of therapeutic education on adherence in patients treated with anti-vitamins K].

INTRODUCTION: Therapeutic education of the patient (TPE) treated with AVK has improved the safety and efficiency of this treatment by establishing a new doctor-patient relationship. The objective of our study was to assess the impact of a TPE program on adherence in patients treated with AVK. METHODS: We conducted a prospective interventional study between January and September 2020, including two groups of patients hospitalized for a first thromboembolic: a G1 group with conventional service support and a G2 group with an ETP program created during this work. Each patient was assessed after

Vascular access for chronic hemodialysis in children: arteriovenous fistula or central venous catheter?

BACKGROUND: The choice of vascular access (VA) for hemodialysis (HD) in end-stage renal disease (ESRD) is arteriovenous fistula (AVF) or central venous catheter (CVC). Whereas clinical practice guidelines suggest AVF to preserve the vascular bed, pediatric nephrologists tend to favor CVC for shorter-term dialysis. Our objective was to determine whether pediatric priority allocation policies for deceased-donor kidneys affect VA planning. METHODS: Pediatric priority for deceased-donor kidneys was instituted in Quebec in 2004. We retrospectively compared clinical practice on AVF, CVC, wait time

Role of operator experience in ultrasound-guided fine-needle aspiration biopsy of the thyroid.

PURPOSE: Thyroid nodules are commonly encountered in clinical practice, and ultrasound (US)-guided fine-needle aspiration biopsy (FNAB) is the gold standard in diagnosing the pathological nature of undetermined thyroid nodules. The aim of this study was to evaluate whether operator experience influences the diagnostic accuracy of US-guided FNAB. MATERIALS AND METHODS: A total of 700 consecutive US-guided FNAB done by a single radiologist between 2000 and 2007 were retrospectively analysed. The same freehand technique and capillary-action technique with 22- or 25-gauge needles was used for all

Dyad motor learning in a wrist-robotic environment: Learning together is better than learning alone.

OBJECTIVE: Dyad motor practice is characterized by two learners alternating between physical and observational practice, which can lead to better motor outcomes and reduce practice time compared to physical practice alone. Robot-assisted therapy has become an established neurorehabilitation tool but is limited by high therapy cost and access. Implementing dyad practice in robot-assisted rehabilitation has the potential to improve therapeutic outcomes and/or to achieve them faster. This study aims to determine the effects of dyad practice on motor performance in a wrist-robotic environment to e

A Korean Clinical Practice Framework for Thyroid Eye Disease: Integrating International Recommendations with Survey-Informed Consensus.

Thyroid eye disease (TED), the most common extrathyroidal manifestation of Graves' disease, requires individualized management that integrates medical, radiotherapeutic, and surgical interventions. In this article, we present a society-endorsed, survey-informed Korean clinical practice framework, including consensus elements, for TED management. This framework integrates the 2022 American Thyroid Association/European Thyroid Association consensus, the 2021 European Group on Graves' Orbitopathy (EUGOGO) guideline, and contemporary Korean specialist practice patterns. A 2025 nationwide survey of

Involvement of human left frontoparietal cortices in neural processes associated with task-switching between two sequences of skilled finger movements.

In this study, we demonstrate the involvement of left frontoparietal cortices in neural processes for task-switching between skilled movements. Functional magnetic resonance imaging was conducted while thirty-two right-handed healthy participants performed two sequential finger-movement tasks with their left hands. One group (n = 16) trained these tasks through random-practice (tasks were either switched or repeated trial by trial) on one day and blocked-practice (successive intensive practice of each task) on the next day, while the remaining participants practiced in the reverse order. On th

Family physician practice visits arising from the Alberta Physician Achievement Review.

BACKGROUND: Licensed physicians in Alberta are required to participate in the Physician Achievement Review (PAR) program every 5 years, comprising multi-source feedback questionnaires with confidential feedback, and practice visits for a minority of physicians. We wished to identify and classify issues requiring change or improvement from the family practice visits, and the responses to advice. METHODS: Retrospective analysis of narrative practice visit reports data using a mixed methods design to study records of visits to 51 family physicians and general practitioners who participated in PA

Supplementary VR batting training with slow-replay augmented feedback improves bat-ball contact precision.

Fast interceptive sport actions may provide too little usable error information for performance refinement, because performance-critical events unfold too quickly to be inspected directly. We tested whether making such information visible through slow-replay augmented feedback, as part of supplementary VR batting training added to usual practice, would support improvement in bat-ball contact precision. Thirty-seven collegiate batters were randomly assigned to a Training or Control group. Over four weeks, both groups continued usual practice; the Training group also completed eight VR batting s

Comparing prevalence and types of potentially inappropriate medications among patient groups in a post-acute and secondary care hospital.

Reducing potentially inappropriate medications (PIMs) is a challenge in post-acute care hospitals. Some PIMs may be associated with patient characteristics and it may be useful to focus on frequent PIMs. This study aimed to identify characteristic features of PIMs by grouping patients as in everyday clinical practice. A retrospective review of medical records was conducted for 541 patients aged 75 years or older in a Japanese post-acute and secondary care hospital. PIMs on admission were identified using the Screening Tool for Older Person's Appropriate Prescriptions for Japanese. The patients

General practitioners' management of mental disorders: a rewarding practice with considerable obstacles.

BACKGROUND: Primary care improvement is the cornerstone of current reforms. Mental disorders (MDs) are a leading cause of morbidity worldwid … key to good MD management. Limited access to specialists, system fragmentation, and underdeveloped group practice and shared-care models were impediments. MD management was seen as burdensome because it required more time, flexibility, and emotional investment. Strategies exist to reduce the burden (one-problem-per-visit rule; longer time slots). GPs found MD p …

The time course of hearing aid benefit.

OBJECTIVE: The present study sought to determine whether benefit derived from hearing aid use increases with hearing aid experience. This question is of considerable interest to both researchers and clinicians. Several previous investigations recently published have obtained mixed results. DESIGN: Control conditions were provided to address the potential influences of practice effects and changes in preferred hearing aid gain. The experimental group consisted of 13 hearing-impaired listeners about to be fit with their first hearing-aid. The control group consisted of 13 hearing aid users with

Maximising living donation with paediatric blood-group-incompatible renal transplantation.

Long-term outcomes for paediatric renal transplant recipients have improved over the last 20 years, with better patient and renal allograft survival. As paediatric renal transplantation programmes have increased over this timeframe, living donation has become the favoured modality for renal replacement therapy and is advocated pre-emptively in as many cases as possible. However, one of the main barriers historically to living donation has been ABO blood-group incompatibility, with the result that patients were listed to be on call for a deceased donor renal transplant. The clinical scenario ha

Hand movement span after mild traumatic brain injury: a longitudinal study.

This study examined whether memory span was impaired during the acute and post-acute phases following mild traumatic brain injury (mTBI). Twenty-two adults with mTBI were compared with 22 controls on computerized tasks of immediate memory for verbal, spatial, and hand movement sequences under no interference (baseline) and articulatory suppression conditions. Groups were assessed within a month and followed up 3-12 months post-injury. In the acute phase, there were no group differences across tasks under either condition. At follow-up, all spatial and verbal span scores and associated practice
