Artificial acrylic nails may induce side effects such as fingertip dermatitis, periungual dermatitis, onycholysis, paresthesiae, Raynaud's phenomenon, ectopic facial involvement, and allergic contact dermatitis. We present a patient who developed allergic onycholysis from a cyanoacrylate used in a nail adhesive. She was able to use photobonded sculptured nails because they contain methacrylates that do not cross-react with cyanoacrylates.

Pyoderma gangrenosum is an unusual neutrophilic dermatosis associated to different rheumatologic, haematologic and inflammatory bowel diseases which generally responds to the therapy of the underlying disease. We report a case of pyoderma in a 44-years-old woman with paucisymptomatic, distal, ulcerative colitis which appeared simultaneously in her forehead and hands. We think it of general interest because of its localization, its importance, the failure of response to steroids and the need of cyclosporine A for regression.

A case of erythema elevatum diutinum showing an unusual distribution is reported. Hard nodules were located on the patient's fingertips and pulps while the extensor surfaces were not involved. The disease was initially misinterpreted based on both clinical and histologic examination. Dapsone therapy was followed by a remarkable recovery. The clinical and histologic features of erythema elevatum diutinum are summarized.

During a 15-month period, 536 patients being investigated for suspected contact dermatitis were patch tested with the European standard series and palladium chloride 1% pet. 13 patients (2.4%) had a positive allergic response to palladium chloride and all 13 were also allergic to nickel. 12 of these 13 patients consented to further patch testing with discs of pure palladium metal foil, and none reacted. The authors report 3 patients affected by psoriasiform acral dermatitis, a distinctive clinical entity characterized by a chronic dermatitis of the terminal phalanges, associated with marked shortening of the nail beds of the affected fingers. The skin biopsy showed in all cases the pathological features of a subacute spongiotic dermatitis. X-ray examination of affected fingers showed no bone or soft tissue changes. Differential diagnosis of psoriasiform acral dermatitis included psoriasis, atopic or contact dermatitis and corticosteroid-induced distal phalangeal atrophy.

Irritant contact dermatitis caused by occupational contact with nitroglycerin has been known since the end of the last century. Nitroglycerin is an allergen, and the transdermal drug delivery systems for nitroglycerin recently used to treat angina pectoris have sensitized. 4 patients with allergic contact dermatitis caused by nitroglycerin from explosives are described, and 1 patient who was sensitized by transdermal nitroplaster. Delayed-type allergies account for most reactions to gloves. These have been found in 32 of 39 patients (82%) attending our department with occupationally induced contact dermatitis to gloves. Accelerators, mainly of the thiuram group, antioxidants, vulcanizers, organic pigments, and, presumably, glove powder ingredients are known responsible allergens. A 2-week-old infant had grouped pustules on the right hand, wrist, and antecubital flexure that appeared after the first week of life. The infant was seen repeatedly to suck the involved areas of the hand and wrist. Clinically typical candidiasis was noted in the oral cavity and the diaper area. Wright's-stained smears of pustular contents, potassium hydroxide preparations of pustular material, and scale from the diaper dermatitis confirmed the presence of pseudohyphae and budding yeasts in all the involved sites. A personal series of patients with eczema on the hands arising in non-occupational conditions was examined (students, housewives, clerical workers, teachers, farm labourers). The data obtained indicate the presence of allergic sensitisation in 70% of the cases and especially among females (3 : 1). The most frequently encountered allergens were difenylguanadine (used in rubber manufacture) and nickel. It is therefore suggested that patch tests should be performed immediately on these patients so that contact with allergen can be prevented in addition to the usual topical treatment.

The exotic mould Hendersonula toruloidea may occasionally produce a ringwormlike, non-inflammatory, therapy-resistant mycosis of the skin and nails simulating chronic dermatophytosis. We report on a 30-year-old Nigerian male who, after two years' sojourn in Sweden, developed finger-nail dystrophy and mild scaling of the palm and sole skin due to H. toruloidea infection. Nail avulsion followed by long-term occlusive application of ciclopiroxolamine cleared the onychomycosis, whereas the sole and palm infection was resistant to ciclopiroxolamine and glutaraldehyde treatment.

Contact allergy to nickel could be found in 44.7% of 247 Austrian hairdressers with hand eczema. Two-thirds had become sensitive to nickel during their apprenticeship. The hairdressers' occupational exposure to nickel, chromium and cobalt was screened and different materials were analysed by a flame atomic absorption spectrophotometer. Hairdressers' nails (means = 54.22 ppm) contained ten times more nickel than those of controls (means = 6.5 ppm), but the differences were not statistically significant.
