Steady-state discharge and bursting of arterial chemoreceptors in the duck.

The steady-state discharge of fourteen arterial chemoreceptor preparations were recorded from the left cervical vagi of unidirectionally ventilated, pentobarbitone anaesthetized ducks. All were excited by both hypoxia and hypercapnia and these stimuli interacted multiplicatively, as they do in mammals. We located the receptive fields of three preparations by observing their responses to i.v. injections of 2,4-dinitrophenol before, during and after occlusion of various arteries. The responses of two preparations were consistent with their location in the ipsilateral carotid body, but the respon

Novel approach of rigid bronchoscopy concurrent with cesarean section and bronchial arterial embolism for patients with massive hemoptysis during pregnancy: case reports and literature review.

Massive hemoptysis can be life-threatening and is frequently encountered in clinical practice, but rare during pregnancy. There have been limited case reports of massive hemoptysis in pregnancy in patients with conditions such as Takayasu's arteritis, bronchiectasis, bronchial carcinoid tumor, and tuberculosis. The most important management is early control of the hemorrhage and airway protection. We report on 2 patients at 33 and 27 gestational weeks who were admitted to the emergency department because of massive hemoptysis. Therapeutic rigid bronchoscopy with the application of high-frequen

Sildenafil for pulmonary hypertension in neonates.

BACKGROUND: Persistent pulmonary hypertension in neonates (PPHN) is associated with high mortality. Currently, the therapeutic mainstay for PPHN is assisted ventilation and administration of inhaled nitric oxide (iNO). However, nitric oxide is costly and may not be appropriate in resource-poor settings. Approximately 30% of patients fail to respond to iNO. High concentrations of phosphodiesterases in the pulmonary vasculature has led to the use of phosphodiesterase inhibitors such as sildenafil or milrinone. OBJECTIVES: To assess the efficacy and safety of sildenafil in the treatment of persi

A Case Report of Posterior Reversible Encephalopathy Syndrome (PRES) in a Nonsevere Case of COVID-19.

Posterior reversible encephalopathy syndrome (PRES) is a rare complication that the exact pathophysiological mechanism of which is still unclear. PRES most often occurs in connection with severe hypertension and autoimmune diseases. It can also appear during chemotherapy or immunosuppressive treatment. A 38-year-old woman with a negative medical history was admitted to the local hospital due to loss of consciousness accompanied by seizures and high values of blood pressure, and a PCR test for COVID-19 was positive. The patient's condition was preceded by weakness, wet cough, runny nose, and lo

Improved Risk Stratification Prior to Major Pulmonary Resection by Combining Peak Oxygen Uptake and Ventilatory Efficiency in a 9-Field Matrix.

BACKGROUND: Cardiopulmonary exercise testing (CPET) has a pivotal role in preoperative evaluation of patients before lung cancer surgery. As surgical and perioperative practice and functional diagnostics continuously evolve, it may be time to reevaluate and refine the use of CPET in this setting. RESEARCH QUESTION: Can risk assessment with CPET before lung cancer surgery be improved by combining 2 established CPET variables (percent predicted peak oxygen uptake [Vo2peak], and ventilatory efficiency, measured by minute ventilation [ V ˙ E ]/carbon dioxide elimination [Vco2] slope) while usin

A methodology to explore ventilatory chemosensitivity and opioid-induced respiratory depression risk.

Reported incidence of postoperative opioid-induced respiratory depression (OIRD) ranges from 0.5-41% and is not reliably predicted by traditional risk factors. This study tests a new methodology to investigate ventilatory chemosensitivity as a new potential risk factor and explore OIRD distribution across sleep and wakefulness. Preoperative patient ventilatory chemosensitivity was quantified by hypercapnic ventilatory responses with (HCVRREMI, effect site concentration 0.7 or 2.0 ng/mL) and without (HCVRBL) remifentanil during hyperoxia and hypoxia. Postoperative opioid consumption was recorde

Delirium in critically ill patients: epidemiology, pathophysiology, diagnosis and management.

Delirium is commonly observed in critically ill patients and is associated with negative outcomes. The pathophysiology of delirium is not completely understood. However, alterations to neurotransmitters, especially acetylcholine and dopamine, inflammatory pathways and an aberrant stress response are proposed mechanisms leading to intensive care unit (ICU) delirium. Detection of delirium using a validated delirium assessment tool makes early treatment possible, which may improve prognosis. Patients at high risk of delirium, especially those with cognitive decline and advanced age, should be ide

[Variability in working systems in intensive care units].

INTRODUCTION: Patients attended in Intensive Medicine Services (IMS) require a high therapeutic support. To do so, each IMS elaborates a work system (monitoring of constants, insertion of catheters,...) aiming to respond to these high necessities of therapy. The objective of this study is to describe the variability among different IMS. METHODS: The study was performed in 15 hospitals in Catalonia between April and May 1995. In the end two of them did not participate in the study. In each hospital 180 pieces of data were collected referring to their material and human resources, graphics of d

Thrombotic and hemorrhagic complications of COVID-19 in adults hospitalized in high-income countries compared with those in adults hospitalized in low- and middle-income countries in an international registry.

BACKGROUND: COVID-19 has been associated with a broad range of thromboembolic, ischemic, and hemorrhagic complications (coagulopathy complications). Most studies have focused on patients with severe disease from high-income countries (HICs). OBJECTIVES: The main aims were to compare the frequency of coagulopathy complications in developing countries (low- and middle-income countries [LMICs]) with those in HICs, delineate the frequency across a range of treatment levels, and determine associations with in-hospital mortality. METHODS: Adult patients enrolled in an observational, multinational

Active surveillance to determine the impact of methicillin resistance on mortality in patients with bacteremia and influences of the use of antibiotics on the development of MRSA infection.

INTRODUCTION: Methicillin-resistant Staphylococcus aureus (MRSA) is among the most important pathogens of nosocomial infections, mainly in intensive care units (ICUs), and accounts for 40-60% of all healthcare-associated S. aureus infections. We evaluated the incidence of nosocomial infection by S. aureus, identified the risk factors for MRSA infection, and evaluated the effect of resistance to methicillin on mortality in patients. METHODS: We conducted MRSA surveillance at a university hospital in Brazil from January 1, 2010, to December 31, 2010, and performed a retrospective case-control m

The difference in respiratory and blood gas values during recovery after exercise with spontaneous versus reduced breathing frequency.

Extrapolation from post-exercise measurements has been used to estimate respiratory and blood gas parameters during exercise. This may not be accurate in exercise with reduced breathing frequency (RBF), since spontaneous breathing usually follows exercise. This study was performed to ascertain whether measurement of oxygen saturation and blood gases immediately after exercise accurately reflected their values during exercise with RBF. Eight healthy male subjects performed an incremental cycling test with RBF at 10 breaths per minute. A constant load test with RBF (B10) was then performed to ex

Anti-IL-8 antibody potentiates the effect of exogenous surfactant in respiratory failure caused by meconium aspiration.

AIM: Meconium aspiration syndrome (MAS) is life-threatening respiratory failure of newborns which can be treated by exogenous surfactant. In response to meconium, increased levels of chemokine IL-8 (CXCL8) stimulate massive neutrophil infiltration of the lungs. Local accumulation and activation of neutrophils, on-going inflammation, lung edema, and oxidative damage contribute to inactivation of endogenous and therapeutically given surfactants. Therefore, we have hypothesized that addition of monoclonal anti-IL-8 antibody into exogenous surfactant can mitigate the neutrophil-induced local injur

Diagnostic criteria for invasive pulmonary aspergillosis in COPD patients.

Over 400 million people have chronic obstructive pulmonary disease (COPD), with exacerbations representing a major health burden. Although the overall incidence of invasive pulmonary aspergillosis (IPA) in patients with COPD with hospitalised exacerbation is only 1 to 4%, certain factors substantially increase this frequency. Risk factors compromising defences against Aspergillus spp in COPD patients include systemic or high-dose inhaled corticosteroids, comorbidities including bronchiectasis, diabetes, and cardiovascular disease, and prolonged courses of antibiotics. An international group of

Clinical determinants of hospital mortality in liver failure: a comprehensive analysis of 62,717 patients.

BACKGROUND: Liver failure (LF) is characterised by a loss of the synthetic and metabolic liver function and is associated with a high mortality. Large-scale data on recent developments and hospital mortality of LF in Germany are missing. A systematic analysis and careful interpretation of these datasets could help to optimise outcomes of LF. METHODS: We used standardised hospital discharge data of the Federal Statistical Office to evaluate current trends, hospital mortality and factors associated with an unfavourable course of LF in Germany between 2010 and 2019. RESULTS: A total of 62,717 h
