Redox protection by bacterial glutathione peroxidase drives virulence in Pseudomonas aeruginosa.

BACKGROUND: Pseudomonas aeruginosa relies on antioxidant enzymes to withstand host-derived oxidative stress during infection. Here, we investigated the role of bacterial glutathione peroxidase (GPx) in survival and virulence of the PA14 strain under inflammatory conditions associated with hyperuricemia. METHODS: Wild-type (WT) and Δgpx strains were exposed to isolated hypochlorous acid (HOCl), hydrogen peroxide (H₂O₂), tert-butyl hydroperoxide, and urate hydroperoxide (HOOU) or challenged with human neutrophils in the presence or absence of oxidative burst modulators. In vivo, normouricemic a…

Diagnosis and follow-up of glycogen storage disease (GSD) type VI from the largest GSD center in China.

Glycogen storage disease (GSD) Type VI is a glycogenolysis disorder caused by variants of PYGL. Knowledge about this disease is limited because only approximately 50 cases have been reported. We investigated the clinical profiles, molecular diagnosis, and treatment outcomes in patients with GSD VI from 2000 to 2021. The main initial clinical features of this cohort include hepatomegaly, short stature, elevated liver transaminases, hypertriglyceridemia, fasting hypoglycemia, and hyperuricemia. After uncooked cornstarch treatment, the stature and biochemical parameters improved significantly (p …

Pharmacological basis for use of madecassoside in gouty arthritis: anti-inflammatory, anti-hyperuricemic, and NLRP3 inhibition.

Objectives: Gouty arthritis is caused by the deposition of monosodium urate (MSU) crystals in joints, which is associated with the rise of serum urate content. This study aims to investigate the therapeutic effect of Madecassoside on gouty arthritis and hyperuricemia. Methods: DBA/1 mice were intradermally injected with MSU to stimulate joint inflammation or intraperitoneally injected with MSU to trigger peritonitis. Moreover, ICR mice were exposed to potassium oxonate to stimulate hyperuricemia. Results: Madecassoside repressed MSU-triggered pad swelling, joint 99mTc uptake, and joint inflamm…

Dietary Interventions for Gout and Effect on Cardiovascular Risk Factors: A Systematic Review.

Gout is one of the most prevalent inflammatory rheumatic disease. It is preceded by hyperuricemia and associated with an increased risk for cardiovascular disease, both related to unhealthy diets. The objective of this systematic review is to better define the most appropriate diet addressing both disease activity and traditional cardiovascular risk factors in hyperuricemic patients. We included clinical trials with patients diagnosed with hyperuricemia or gout, investigating the effect of dietary interventions on serum uric acid (SUA) levels, gout flares and-if available-cardiovascular risk f…

Coexistence of Sarcoidosis and Gouty Arthritis.

Sarcoidosis is an inflammatory disease with unknown cause characterized by non-caseating granuloma formations. It may present with bilateral hilar lymphadenopathy, skin lesions, the involvement of eye and symptoms on the locomotor system. Gouty arthritis is an autoinflammatory disease characterized by hyperuricemia, recurrent arthritis attacks and the deposition of monosodium urate crystals in the joints and the surrounding tissues. We reported the coexistence of sarcoidosis and gouty arthritis in this paper.

Urine alkalinization may be enough for the treatment of bilateral renal pelvis stones associated with Lesch-Nyhan syndrome.

Lesch-Nyhan syndrome is a rare sex-linked disorder of purine metabolism that is caused by a mutation in the hypoxanthine-guanine phosphoribosyltransferase (HPRT) gene which causes marked hyperuricemia and hyperuricosuria, with signs of gouty arthritis and uric acid stone disease in early childhood. We report a case of renal pelvis calculi which was dissolved within 10 days of urine alkalinization and hydration.

Gout.

Characteristic imaging finding of chronic gout include eccentric soft-tissue abnormalities, sometimes calcified, that have overly well defined erosions with sclerotic margins and overhanging edges, and preservation of bone density. While the triad of acute monoarticular arthritis, hyperuricemia, and prompt response to drug therapy is highly suggestive of acute gouty arthritis, the radiographic findings during an acute attack are nonspecific.

Metabolic consequences of a sorbitol overdose during neurosurgery.

The authors present a case of severe electrolyte and acid-base disturbances after administration of sorbitol. The rate of sorbitol infusion was in excess of the stated maximum safe infusion rate in parenteral nutrition. Sorbitol was used for treating raised intracranial pressure. The mechanisms of sorbitol metabolism leading to hyperlactacemia, lactic acidosis, hyperuricemia, depletion of intracellular ATP and ADP and depletion of inorganic phosphates are described.

Lesch-Nyhan syndrome: successful prevention of lower lip ulceration caused by self-mutilation by use of mouth guard.

Lesch-Nyhan syndrome (LNS) is a disorder caused by congenital absence of a purine metabolic enzyme, hypoxanthine guanine phosphoribosyl-transferase (HGPRT). This syndrome is characterized clinically by hyperuricemia and neurologic features including choreoathetoid spasticity, self-mutilation, and mental retardation. We report on a patient in whom self-mutilation of the lower lip was suppressed with the help of a mouth guard made from soft resin.

Electrophoretic variation in the partial deficiency of hypoxanthine-guanine phosphoribosyltransferase.

Mutant hypoxanthine-guanine phosphoribosyltransferase from four patients with a partial deficiency of this enzyme has been studied by isoelectric focusing. The isoenzymes found in these hemolysates were different from the normal isoenzymes and were different from each other. These observations suggest that electrophoretic variation is a common occurrence in this disorder and they support the existence of structural gene mutations with genetic heterogeneity in this X-linked hyperuricemia.

Safety Evaluation of Human-Derived Uric Acid Degrading Lacticaseibacillus paracasei M2a and Its Impact on Gut Microbiota.

We report the identification of a human-derived uric acid (UA)-degrading bacterial strain, analyzed its degradation efficiency, and assessed its safety to provide a scientific basis for future clinical applications in the treatment of hyperuricemia (HUA) and gout. Here, we isolated the M2a strain from feces of healthy young men. The strain was identified as Lacticaseibacillus paracasei (formerly Lactobacillus paracasei) via 16S rRNA and biochemical analyses. The in vitro and in vivo efficiencies of M2a uric acid degradation were found to be 45.53% and up to 47.88%, respectively. Strain M2a exh…

From suspected joint tuberculosis to gouty arthritis: a diagnostic journey.

Gout is a metabolic disorder characterized by hyperuricemia, leading to the deposition of monosodium urate crystals in joints and soft tissues. It commonly affects the first metatarso-phalangeal (MTP) joint, but atypical presentations can pose significant diagnostic challenges. In this report, we describe a rare case of gouty arthritis affecting the proximal interphalangeal (PIP) joint of the second toe, which was initially suspected to be a case of joint tuberculosis. A 38-year-old male presented with a painful swelling over the second toe for two months, with imaging and laboratory findings …

Dietary and Lifestyle-Centered Approach in Gout Care and Prevention.

PURPOSE OF REVIEW: We aim to provide a comprehensive review of the available literature to inform dietary recommendations for patients with gout and hyperuricemia that have the potential to simultaneously lower serum urate and reduce gout morbidity while addressing gout's cardiometabolic comorbidities holistically. RECENT FINDINGS: The global burden of gout is rising worldwide, particularly in developed nations as well as in women. Patients with gout are often recommended to follow a low-purine (i.e., low-protein) diet to avoid purine-loading. However, such an approach may lead to increased c…

Structure-Based Immunogenicity Prediction of Uricase from Fungal (Aspergillus flavus), Bacterial (Bacillus subtillis) and Mammalian Sources Using Immunoinformatic Approach.

Gout is a common rheumatic condition caused due to increase in serum uric acid level (hyperuricemia). Uricase is for lowering the level of uric acid but unfortunately, it is not produced in humans due to evolutionary changes. Therefore, it is administered to humans from outside in case of the high uric acid level in blood. A different formulation of uricase from bacterial, fungal, and mammalian sources is present in the market for the treatment of hyperuricemia conditions. Uricase formulation showed immunogenic response due to the occurrence of hypersensitivity reaction during the treatment th…
