Intrahepatic quantification of HBV antigens in chronic hepatitis B reveals heterogeneity and treatment-mediated reductions in HBV core-positive cells.

BACKGROUND & AIMS: Patterns of liver HBV antigen expression have been described but not quantified at single-cell resolution. We applied quantitative techniques to liver biopsies from individuals with chronic hepatitis B and evaluated sampling heterogeneity, effects of disease stage, and nucleos(t)ide (NUC) treatment, and correlations between liver and peripheral viral biomarkers. METHODS: Hepatocytes positive for HBV core and HBsAg were quantified using a novel four-plex immunofluorescence assay and image analysis. Biopsies were analysed from HBeAg-positive (n = 39) and HBeAg-negative (n = 7

Deep learning on histopathological images to predict breast cancer recurrence risk and chemotherapy benefit: a multicentre, model development and validation study.

BACKGROUND: Genomic assays such as Oncotype DX have transformed adjuvant treatment selection for hormone receptor-positive, HER2-negative, early breast cancer but remain inaccessible to many patients because of high cost and logistical barriers. We aimed to develop and validate an artificial intelligence (AI) model that estimates Oncotype DX 21-gene recurrence scores directly from routine histopathology slides and clinicopathological variables. METHODS: In this multicentre, model development and validation study, a multimodal deep-learning model was trained on digital whole-slide images and c

Scalable molecular representations enabled by multimodal fusion and sequence distillation.

Molecular prediction depends on how chemical structures are represented, yet descriptors capture only partial aspects of chemical information. Here, we show that the Chemical Dice Integrator combines six complementary molecular views spanning physicochemical properties, molecular topology, two-dimensional structural images, bioactivity profiles, quantum properties, and molecular language into a unified latent space. This multimodal representation is distilled into a sequence-based model that generates the embedding directly from molecular strings. Across classification and regression benchmark

Standardization of Collected Biobank Data to Facilitate Cardiovascular Research: A Scientific Statement From the American Heart Association.

Cardiovascular disease has been the focus of extensive biomedical research, yet it remains a leading cause of morbidity and mortality worldwide. One recent approach to support scientific discovery has been the development of large-scale population-based prospective studies that couple phenotypic data, including outcomes, to collection and analysis of biological samples. Large, data-dense biobanks, particularly those linked to electronic health records (including laboratory, electrocardiographic, imaging, and therapeutic data) and genomic data, have demonstrated value for interdisciplinary rese

A cross-species AAV platform for precise microglial interrogation.

Targeting microglia with adeno-associated viruses (AAVs) remains challenging because of inefficient transduction, off-target expression, and inconsistent payload performance. Here, we develop an integrated, Cre-independent AAV platform that combines a compact 446-bp microglia-specific regulatory element (MSRE), a high-yield AAV2/9.m capsid, microRNA-mediated neuronal de-targeting, and a microglia-tailored codon-optimization strategy. Following direct intracranial delivery, the optimized vector achieves robust, long-lasting transgene expression with greater than 90% microglial specificity acros

Infantile giant intracranial-extracranial communicating mixed germ cell tumor with ARID1B gene mutation: a case report.

Congenital intracranial-extracranial communicating mixed germ cell tumors (MGCTs) are extremely rare in infants, with no standardized treatment protocol. We report a 2-month-old female infant with giant congenital intracranial-extracranial communicating MGCT accompanied by typical "crater-like" skull defect. Preoperative serum alpha-fetoprotein (AFP) was significantly elevated, while β-human chorionic gonadotropin (β-HCG) was within normal range. Maximal safe resection was performed, and pathology confirmed mixed germ cell tumor (immature teratoma WHO grade 3 + yolk sac tumor). Next-generation

Olduvai Domains Downregulate Mitochondrial Pathways to Promote Human Brain Evolution and Neoteny.

Olduvai (formerly DUF1220) protein domains, encoded by the NBPF gene family, have undergone the greatest human lineage-specific copy-number expansion of any coding sequence in the genome and strongly correlate with brain size and neuron number across primates. Here we show that Olduvai domains act in a dosage-dependent manner to suppress mitochondrial metabolism. Transcriptomic, proteomic, and live-cell imaging analyses of cells overexpressing NBPF1 (which encodes seven Olduvai domains) reveal pronounced downregulation of mitochondrial pathways, including electron transport chain components an

Insulators As Dynamic, Tunable Regulators of Enhancer-Promoter Coordination in Living Drosophila Embryos.

The three-dimensional organization of the genome enables enhancers and promoters to interact across vast distances and direct transcription. Yet whether architectural elements, such as insulators, serve as rigid, passive barriers or as dynamic, active organizers of this communication remains unclear. Here, using single-cell, live imaging of a Drosophila transgene in which a single enhancer regulates two equidistant promoters, we confirm that the enhancer engages both promoters simultaneously and show that coordinated bursting is intrinsically more productive than uncoordinated activity. Spatiotemporal brain transcriptomics reveal risk gene hot-spots in major neuropsychiatric disorders.

Brain development is guided by dynamic gene expression programs that vary across brain regions and developmental stages. Although numerous risk genes for neuropsychiatric disorders have been identified, the spatiotemporal contexts in which they act remain unclear. Here we show the spatiotemporal expression patterns of genome-wide risk gene sets across 15 neuropsychiatric traits, including autism, attention deficit hyperactive disorder, obsessive compulsive disorder, major depression, bipolar disorder, schizophrenia, epilepsy, Alzheimer's disease and Parkinson's disease, using bulk and single-c

Advanced Molecular and Microscopic Diagnostics Suggest Congenital Borrelia Transmission: A Case Report.

Lyme disease is by far the most common arthropod-borne disease in the Northern Hemisphere. It is caused by certain Borrelia species that are primarily transmitted to hosts by Ixodid ticks; however, transplacental transmission of the spirochete in both animals and humans has been reported. Here, we report imaging of intact spirochetes in an archived placental tissue sample that is immunoreactive to Borrelia antibodies and from which Borrelia DNA was isolated. Both mother and child showed evidence of seroreactivity to Borrelia spp. in the years following the child's birth, although neither indiv

Genetic insights into foveal morphology and its associations with pigmentation and age-related macular degeneration.

The fovea is the small depression at the neurosensory retina that underlies high-resolution central vision. It is vulnerable to disease and disruption of its architecture causes visual disability. Foveal morphology varies significantly across individuals. The molecular causes and functional consequences of this anatomical diversity are incompletely understood. Here, we extracted six foveal morphological parameters from Optical Coherence Tomography (OCT) images of 55,604 UK Biobank participants. We found notable variability in foveal morphology, and detected significant links with sex and genet

Systemic Consequences of Inflammatory Bowel Disease Beyond Immune-Mediated Manifestations.

Inflammatory bowel disease (IBD) management traditionally focuses on intestinal inflammation, yet extraintestinal manifestations can substantially impair patient quality of life. In this perspective, we emphasize the broad systemic impact of IBD-from highly prevalent conditions such as anemia, metabolic dysfunction-associated steatotic liver disease, or fatigue to rare but severe complications like interstitial lung disease and drug-induced glomerulonephritis. We review underlying mechanisms linking gut inflammation to distant organs, including immune dysregulation, microbial translocation, an

Disseminated Hypervirulent Klebsiella pneumoniae Infection Following Travel: A Case of Cavitating Pneumonia, Hepatic and Renal Abscesses, and Thrombosis.

Hypervirulent Klebsiella pneumoniae (hvKP) is a recently emerging pathogen that causes severe community-acquired infections in immunocompetent patients, in contrast to classical K. pneumoniae, which is found in nosocomial settings. We report the case of a healthy 55-year-old woman who, following recent travel to Singapore, presented with diabetic ketoacidosis (DKA) and septic shock. She presented with fever, cough, myalgias, and confusion, imaging demonstrating bilateral cavitating pneumonia, hepatic and perinephric abscesses, and renal vein and inferior vena cava thrombosis. Genetic architecture of bone marrow fat fraction implies its involvement in osteoporosis risk.

Bone marrow adipose tissue, as a distinct adipose subtype, has been implicated in the pathophysiology of skeletal, metabolic, and hematopoietic disorders. To identify its underlying genetic factors, we utilized a deep learning algorithm capable of quantifying bone marrow fat fraction (BMFF) in the vertebrae and proximal femur using magnetic resonance imaging data of over 38,000 UK Biobank participants. Genome-wide association analyses uncovered 373 significant BMFF-associated variants (P-value < 5 × 10-9), with enrichment in bone remodeling, metabolism, and hematopoiesis pathway. Furthermore,

A clonally expanded nodal T-cell population diagnosed as T-cell lymphoma after CAR-T therapy.

Reports of secondary malignancies after chimeric antigen receptor (CAR)-T and possible CAR-T derived malignant transformation necessitate caution. Here we describe a patient with diffuse large B-cell lymphoma who developed new lymphadenopathy 2.5 years after CAR-T in the context of COVID-19 infection with histopathologic features consistent with T-cell lymphoma (TCL). Deep molecular interrogation with genomic sequencing and single-cell spatial transcriptomics reveals a highly proliferative clonal T-cell population co-expressing CD4 and CD8 with biallelic TCR rearrangement and no evidence of th

A report of progressive multifocal leukoencephalopathy during adjuvant abemaciclib for breast cancer.

Drugs inhibiting the cyclin-dependent kinases (CDK) 4 and 6 are widely used for the treatment of luminal breast cancer, in the adjuvant and in the metastatic setting, and adverse events are well-known. Progressive multifocal leukoencephalopathy (PML) is classically linked to immunosuppression. We present a case of progressive multifocal leukoencephalopathy that developed during adjuvant abemaciclib therapy in a seventy-year-old woman diagnosed with locally advanced ductal carcinoma of the breast. Neurological impairment developed after 18 months of initiating abemaciclib and in imaging tests a

Astrocytic Neuroligin-3 influences gene expression and social behavior, but is dispensable for synapse number.

Neuroligin-3 (Nlgn3) is an autism-associated cell-adhesion molecule that interacts with neurexins and is robustly expressed in both neurons and astrocytes. Neuronal Nlgn3 is an essential regulator of synaptic transmission but the function of astrocytic Nlgn3 is largely unknown. Given the high penetrance of Nlgn3 mutations in autism and the emerging role of astrocytes in neuropsychiatric disorders, we here asked whether astrocytic Nlgn3 might shape neural circuit properties in the cerebellum similar to neuronal Nlgn3. Imaging of tagged Nlgn3 protein produced by CRISPR/Cas9-mediated genome editi

A host-directed oxadiazole compound potentiates antituberculosis treatment via zinc poisoning in human macrophages and in a mouse model of infection.

Antituberculosis drugs, mostly developed over 60 years ago, combined with a poorly effective vaccine, have failed to eradicate tuberculosis. More worryingly, multiresistant strains of Mycobacterium tuberculosis (MTB) are constantly emerging. Innovative strategies are thus urgently needed to improve tuberculosis treatment. Recently, host-directed therapy has emerged as a promising strategy to be used in adjunct with existing or future antibiotics, by improving innate immunity or limiting immunopathology. Here, using high-content imaging, we identified novel 1,2,4-oxadiazole-based compounds, whi

[Advancements in Radiomics for Immunotherapy of Non-small Cell Lung Cancer].

Lung cancer is the main cause of cancer-related deaths, with non-small cell lung cancer (NSCLC) being the predominant subtype. At present, immunotherapy represented by immune checkpoint inhibitors (ICIs) of programmed cell death receptor 1 or its ligand has been widely used in the clinical diagnosis and treatment of patients with NSCLC. However, only a few patients can benefit from it, and reliable predictive markers for immunotherapy are lacking. Radiomics is a tool that uses computer software and algorithms to extract a large amount of quantitative information from biomedical images. A large

Clinical features of a novel compound heterozygous genotype of the BBS2 gene: a case report.

Bardet-Biedl syndrome is a rare autosomal recessive genetic disorder with heterogenous clinical manifestations. The present study reports the clinical features of a novel compound heterozygous genotype of the BBS2 gene in a 14-year-old girl and her 6-year-old sister who had complaints of early-onset low vision. Fundus images revealed retinitis pigmentosa-like changes, and full-field electroretinograms showed no amplitude for the rod or cone response in both patients. Interestingly, nystagmus was observed in the older sister. On physical examination, the sisters had moderate obesity without pol
