A survey of doctors' and nurses' knowledge, attitudes and compliance with infection control guidelines in Birmingham teaching hospitals.

This study investigated knowledge about infection control amongst doctors and nurses through a cross-sectional survey conducted between March and May 2001 in three Birmingham, UK teaching hospitals. Seventy-five doctors and 143 nurses, representing 7% and 4%, respectively, of potential respondents, participated in the study measuring knowledge of, attitudes towards, and compliance with universal precautions. Overall knowledge of risks of blood-borne virus (BBV) transmission from an infected patient after needlestick injury was low [44.0% for hepatitis B virus (HBV), 38.1% for hepatitis C virus…

Reduced rates of hospital-acquired UTI in medical patients. Prevalence surveys indicate effect of active infection control programmes.

Prevalence surveys have been part of the Danish infection control programme since 1974. Nationwide surveys were carried out in 1978, 1979, 1980, 1991 and 1999. The results indicate a net reduction of approximately 25%, mainly due to a reduction of urinary tract infections (UTIs) in medical patients. Results from the nationwide surveys are validated by results of occasional independent surveys performed in regions or single hospitals. The reduction coincides with preventive programmes focused on catheter policy and UTI. We conclude that the prevalence survey is an important part of our strategi…

Expression of IL-15 and IL-4 in IFN-gamma-independent control of experimental human Cryptosporidium parvum infection.

We have previously demonstrated interferon gamma (IFN-gamma) in intestinal mucosa after experimental human Cryptosporidium parvum infection, but expression was limited to sensitized volunteers. To characterize IFN-gamma-independent mechanisms in control of infection, jejunal biopsies from immunocompetent volunteers experimentally challenged with C. parvum were examined by in situ hybridization for interleukin (IL-)15 and IL-4 mRNA with confirmation by immunohistochemistry. Cytokine expression was correlated with prechallenge anti- C. parvum IgG, symptoms, oocyst shedding, and prior IFN-gamma e

Helicobacter pylori infection and risk of cardia cancer and non-cardia gastric cancer. A nested case-control study.

BACKGROUND: Helicobacter pylori infection is an established risk factor for gastric adenocarcinoma. Potential confounding by socioeconomic factors has not been adequately assessed, and the magnitude of the relative risk in relation to gastric subsites, morphologic subtypes, sex, age, and follow-up time need further study. METHODS: We conducted a serologic case-control study nested within the Norwegian JANUS cohort. Between 1972 and 1986 serum was collected from 101,601 subjects who were followed up with regard to cancer development through 1992. RESULTS: Among 208 gastric adenocarcinoma case

Role of Helicobacter pylori infection in perforation of peptic ulcer: an age- and gender-matched case-control study.

Evidence showed a marked decrease in recurrence rate of peptic ulcer after eradication of Helicobacter pylori infection. However, whether H. pylori infection is etiologically related to perforation of peptic ulcer remains to be clarified. We therefore conducted an age- and gender-matched case-control study between perforated and nonsurgical peptic ulcers in H. pylori infection and examined differences in the cytotoxin genes cagA and vacA. Serum H. pylori IgG antibody (ELISA) was positive in 20/21 (95%) of perforated vs. 37/40 (93%) of nonsurgical duodenal ulcers and in 5/5 (100%) of perforated

Efficacy of delayed administration of benzylpenicillin in the control of infection in penetrating soft tissue injuries in war.

In war, uncomplicated penetrating injuries to limbs require evacuation to a surgical facility. A delay is inevitable between injury and definitive surgical treatment. This paper describes an experimental model that has been developed to assess the efficacy of antibiotics in such war wounds; the aim is to develop a treatment protocol to prevent the development of infection before casualties reach a surgical facility. The model is described in detail. Its use to determine efficacy of an antibiotic regimen, and the consequences of delay in the initial administration of antibiotic, are summarized.

Inhibition of group B streptococci by amniotic fluid from patients with intra-amniotic infection and from control subjects.

This study describes the inhibition of group B streptococci by amniotic fluid from 50 patients with intra-amniotic infection and 50 matched, noninfected control subjects. Patients were matched for gestational age, time from rupture of membranes to delivery, and time from rupture of membranes to collection of the sample of fluid. Study patients had the clinical diagnosis of intra-amniotic infection and greater than or equal to 10(2) colony-forming units of a high-virulence organism per milliliter. None of the control patients became infected or received antibiotics. We collected the amniotic fl

A case-control neuroimaging investigation of chronic Zika virus-infected adults.

BACKGROUND: Systemic viral infections with neurotropic potential pose significant global health challenges. The Zika virus (ZIKV) is known for its pronounced neurotropism, with recent infectious clusters raising renewed public health concerns. While research has predominantly focused on congenital populations, growing evidence suggests that the mature central nervous system (CNS) is also vulnerable. However, no study has examined the long-term impact of ZIKV infection on the adult human brain. METHODS: To address this gap, we studied a rare group of adult ZIKV patients presenting with both pe

Innate lymphoid cells integrate sensing and plasticity to control fungal infections.

Innate lymphoid cells (ILCs) are crucial regulators of tissue immunity. Here, we demonstrate that pulmonary ILCs can sense fungal components, leading to their activation. Mechanistically, we identify Syk/p38-dependent signaling as one of the key drivers of ILC activation following fungal challenges. Aspergillus fumigatus infection reshaped this response in vivo, creating a cytokine milieu that promoted ILC3 induction. We identified interleukin (IL)-23, IL-1β, and TGF-β as drivers of ILC2 conversion and IL-23 and IL-1β as triggers for ILC1s obtaining an ILC3 phenotype in vitro. Moreover, adopti

Controlling respiratory infections in long-term care facilities: A systematic review.

BACKGROUND: Residents of long-term care facilities are vulnerable to respiratory infections, yet infection prevention and control strategies for these settings remain limited. While extensive infection control guidelines for hospitals are well established, few address long-term care facilities, where prevention efforts have largely prioritized pharmacological over non-pharmacological measures. Non-pharmacological strategies are therefore essential to prevent respiratory infection transmission, particularly during early outbreak stages or in settings where pathogen-specific treatments are limit…

Controlled human infection with Mycobacterium tuberculosis: practical considerations for clinical trials.

Controlled human infection models (CHIMs) can accelerate vaccine development for infectious diseases. Mycobacterium tuberculosis is a human-adapted pathogen that is the leading infectious cause of death worldwide. M tuberculosis infection results in a spectrum of clinical outcomes that are incompletely modelled in animals. To date, the risks of infection, prolonged treatment, and sequelae related to CHIMs with M tuberculosis have been considered ethically unacceptable. However, recent advances in bacterial engineering have resulted in safe strains that could permit M tuberculosis CHIM studies

Knowledge gaps in research and control of porcine circovirus 2 (PCV2) infections.

Porcine circovirus type 2 (PCV2) remains one of the most important pathogens in swine production, associated with a spectrum of clinical conditions collectively termed PCV2-associated diseases. Despite the remarkable success of vaccination programs, which have drastically reduced the incidence of systemic disease and reproductive disorders, PCV2 continues to circulate globally in both domestic and wild swine populations. Its high evolutionary rate, capacity for recombination, and broad host plasticity raise ongoing concerns regarding viral persistence and long-term control. This review synthes

How community members in Uganda perceive controlled human infection studies.

Controlled Human Infection studies are a new and uncommon way of conducting clinical research in low resource settings. Regardless of the supposed importance of human infection studies in advancing clinical research, there is still limited literature on community perceptions of this field of research, particularly in low-resource settings. An exploratory qualitative study involving 107 community members organized into 16 focussed groups was conducted in Kampala city and Buikwe district in central Uganda between September 2023 and March 2024. The groups comprised market vendors, secondary schoo

In vitro and in vivo efficacy of PI3Kγ inhibitor AS605240 in controlling mycobacterial infections.

Inhibition of PI3Kγ is an attractive therapeutic target for the development of novel host-directed modulating strategies for the treatment of infectious diseases. This work investigated the antimicrobial potential of AS605240, a selective inhibitor of PI3Kγ, in pre-clinical models of mycobacterial infections. Of note, we observed that treatment with AS605240 effectively reduced both intracellular M. smegmatis and M. tuberculosis counts in RAW 264.7 cells. Moreover, treatment of M. tuberculosis-infected cells with AS605240 increased TNF-α and decreased IL-1β levels compared to the infected grou

Simultaneously Controlling Inflammation and Infection by Smart Nanomedicine Responding to the Inflammatory Microenvironment.

The overactivated immune cells in the infectious lesion may lead to irreversible organ damages under severe infections. However, clinically used immunosuppressive anti-inflammatory drugs will usually disturb immune homeostasis and conversely increase the risk of infections. Regulating the balance between anti-inflammation and anti-infection is thus critical in treating certain infectious diseases. Herein, considering that hydrogen peroxide (H2O2), myeloperoxidase (MPO), and neutrophils are upregulated in the inflammatory microenvironment and closely related to the severity of appendectomy pati
