Lenvatinib plus hepatic arterial infusion chemotherapy of oxaliplatin, fluorouracil, and leucovorin versus lenvatinib alone for advanced hepatocellular carcinoma: a multicenter retrospective cohort study.

BACKGROUND: Lenvatinib stands out as a first-line therapy for individuals with advanced hepatocellular carcinoma (HCC). Concurrently, hepatic arterial infusion chemotherapy comprising oxaliplatin, fluorouracil, and leucovorin (FOLFOX-HAIC) has emerged as a potential option for those with advanced HCC. It is necessary to investigate the efficacy and safety of lenvatinib plus FOLFOX-HAIC (lenvaHAIC) for advanced HCC in real-world situations. METHODS: In this retrospective analysis, 127 consecutive patients underwent lenvaHAIC, while 184 patients received lenvatinib alone as first-line treatment

Treatment Intensification with Hepatic Arterial Infusion Chemotherapy in Patients with Liver-Only Colorectal Metastases Still Unresectable After Systemic Induction Chemotherapy: Exploratory Findings From a Prematurely Closed Multicenter Randomized Phase II Study: SULTAN UCGI 30/PRODIGE 53 (NCT03164655).

BACKGROUND: Hepatic arterial infusion (HAI) oxaliplatin represents a promising treatment option in patients with unresectable liver-only colorectal metastases (CRLM). METHODS: In this randomized phase II study, we evaluated the efficacy of an intensification strategy based on HAI oxaliplatin combined with systemic chemotherapy (sys-CT) as a salvage treatment in patients with CRLM still unresectable after first-line induction sys-CT. The primary objective was conversion to resection/ablation (CTR). A real-life retrospective cohort of consecutive patients treated with HAI oxaliplatin + sys-CT i

Placement of an arterial hepatic catheter after a major hepatectomy for colorectal liver metastases: is this safe?

BACKGROUND: Studies have suggested that hepatic arterial infusion of chemotherapy (HAI) after resection of colorectal liver metastasis (CRLM) may improve patient's survival. The placement of a catheter in the hepatic artery at the time of hepatic surgery should therefore be considered in patients at high risk of hepatic recurrence. The aim of this study was to compare post-operative outcomes in patients who underwent at least a major hepatectomy (≥3 segments) for CRLM with or without catheter placement. METHODS: From 2000 to 2010, 57 patients who underwent at least a major hepatectomy for CRL

Inhibition of serotonin-induced increase in canine external carotid blood flow by drugs that decrease the sympathetic outflow.

1. The present study was designed to analyse the effect of the centrally-acting sympatho-inhibitory drugs, prazosin and ketanserin, on the increase in external carotid blood flow (external CBF) produced by 5-hydroxytryptamine (5-HT) in pentobarbital-anaesthetized dogs. 2. Intracarotid (i.c.) infusions of 5-HT (10 micrograms min-1 during 1 min) produced an increase in external CBF without changes in mean arterial blood pressure or heart rate. This response to 5-HT was dose-dependently blocked by intravenous (i.v.) administration of prazosin (1, 3, 10, 30 and 100 micrograms kg-1) or ketanserin (

Transcatheter arterial infusion chemotherapy combined with lipiodol chemoembolization for advanced gastric fundus and cardia cancer with obstruction.

PURPOSE: Gastric fundus and cardia cancer are increasingly common, diagnosed at a late stage, often with severe obstruction, and have a poor prognosis. Transcatheter arterial lipiodol chemoembolization (TACE) is rarely used for gastric cancer. This single-center retrospective study aimed to analyze the efficacy and safety of transcatheter arterial infusion chemotherapy (TAI) combined with TACE in advanced gastric fundus and cardia cancer with obstruction and compare it to bland embolization (BE). MATERIALS AND METHODS: The clinical efficacy of TAI-TACE was evaluated through technical and clin

Sequential HAIC-TACE combined with targeted and immunotherapy in hepatocellular carcinoma: a quality improvement study.

BACKGROUND: Hepatocellular carcinoma (HCC) remains a leading cause of cancer mortality. Emerging evidence suggests that quadruple therapy - combining hepatic artery infusion chemotherapy (HAIC), transcatheter arterial chemoembolization (TACE), targeted therapy, and immunotherapy - improves outcomes. However, few studies have directly compared this regimen with triple therapy (TACE plus targeted therapy and immunotherapy), and the optimal sequence of locoregional treatments within this regimen remains unclear. OBJECTIVE: To compare the efficacy and safety of HAIC-first quadruple therapy, TACE-

Acute carotid stent thrombosis: review of the literature and long-term follow-up.

BACKGROUND: Carotid artery stenting (CAS) is a reasonable alternative to carotid endarterectomy, especially in patients at high risk for surgery. Carotid stent thrombosis can cause thrombembolic events, but fortunately, it is a very rare complication. We present two cases of carotid stent thrombosis and their long-term follow-up. CASE REPORTS: One patient had severe bilateral carotid stenosis and the other had contralateral carotid occlusion. Both patients were on correct antithrombotic treatment and received balloon expandable stents (bare metal stent and drug-eluting stent). During CAS, lar

Salivary gland carcinoma treated with concomitant chemoradiation with intraarterial cisplatin and docetaxel.

Malignant neoplasms of the salivary gland are uncommon entities in which surgical resection of the primary lesion has been accepted as a standard therapeutic option. The efficacy of radiation and systemic chemotherapy has been limited for patients with recurrent, metastatic, or unresectable disease because of unfavorable response rates and the short duration of the response. We treated one patient with recurrent adenoid cystic carcinoma arising from the sublingual gland and one patient with primary adenocarcinoma arising from the parotid gland with transfemoral intraarterial chemotherapy, base

Cerebrovascular events associated with infusion through arterially malpositioned triple-lumen catheter: report of three cases and review of literature.

Analysis of 10 adult patients treated from January 1998 to November 2004 for arterial misplacement of triple-lumen catheter (TLC) during internal jugular vein cannulation was performed. Three cases that developed neurologic symptoms occurring in the context of infusion through a TLC that was arterially malpositioned are presented, along with the review of literature. In 7 patients, the diagnosis of arterial misplacement was suspected by the color or flow characteristics of blood and confirmed by a combination of blood gas analysis, connecting catheter to transducer, and/or chest film. Modified infusion procedures affect recombinant adeno-associated virus vector type 2 transduction in the liver.

Recombinant adeno-associated virus (rAAV) vectors have therapeutic potential for the treatment of several types of liver diseases including hepato-deficiency disorders. Most of the preclinical and clinical applications involve the use of adeno-associated vector serotype 2 (AAV-2). However, when this vector is delivered at high doses into the portal vein or hepatic artery, a relatively small number of hepatocytes are stably transduced. We elected to determine if the route of vector administration and altering the vascular delivery route within the liver influenced the relative level of transduc

Transcatheter arterial chemoembolization of hepatocellular carcinoma: a Japanese experience.

Hepatocellular carcinoma is one of the most common causes of cancer death in Japan and in 80% of cases is associated with chronic liver disease caused by hepatitis C virus. Poor hepatic function reserve due to underlying cirrhosis is the primary factor which limits extended surgical resection in many cases. Furthermore, in patients treated by curative resection, high incidence of recurrent tumors or/and newly developed tumor in the residual liver was reported. Therefore, the aim of various therapeutic options such as operation, percutaneous ethanol injection, radiofrequency coagulation therapy

Effect of intraarterial papaverine on cerebral circulation time.

PURPOSE: To measure the mean cerebral circulation time (CCT) in patients with symptomatic vasospasm stemming from subarachnoid hemorrhage and to determine any change after papaverine treatment. METHODS: We studied 27 patients who received intraarterial papaverine from November 1992 to August 1995 to determine the CCT in 59 carotid territories. CCT was measured from the first image in which contrast was seen above the supraclinoid internal carotid artery to the peak filling of parietal cortical veins. Angiograms at the time of presentation were examined in 19 of the 27 patients. A control popu
