Excision, suture ligation and endoscopic snaring are the recommended methods of treatment for a juvenile rectal polyp. A successful experience of a simpler technique, digital polypectomy, in 218 cases is reported.

The three autosomal dominant inherited polyposis syndromes, familial adenomatous polyposis, juvenile polyposis, and Peutz-Jeghers polyposis predispose to colorectal cancer as does hereditary non-polyposis colorectal cancer syndrome. Colonoscopy was performed on 210 asymptomatic average-risk persons, aged 50-75 years, who had negative fecal occult blood test results. Colonoscopy was complete to the cecum in 209 subjects. Fifty-three subjects (25%) had adenomas and two had cancer. All of the adenomas greater than or equal to 1 cm in size and both cancers occurred in subjects aged greater than or equal to 60 years. The earlier finding of microhamartomatous rectal polyps in tuberous sclerosis (TSC)2 is detailed. These polyps are common, occurring in 14 of 18 (78%) patients with TSC in the present series, and they do not cause symptoms. Their distinctive appearance and distribution readily distinguish them from other types of rectal polyp and emphasize their importance as a potentially useful clinical marker of TSC.

Gardner's syndrome is a hereditary disorder characterised by gastrointestinal polyposis, bony osteomata, epidermoid cysts, desmoid tumours and unerupted teeth and odontomes. A 51-year-old man, 25 years after undergoing Billroth II gastrojejunostomy with antrectomy and vagotomy, came to our hospital with acute relapsing pancreatitis. At operation he was found to have an inflammatory polypoid lesion at the duodenal stump that consisted of suture and talc granuloma from his previous gastric surgery. The polyp intermittently obstructed the pancreatic duct, causing symptomatic pancreatitis. The patient has remained without symptoms 2 years after submucosal resection of this polyp. This complication of Billroth II gastroenterostomy has not been recognized previously.

In the present paper we report about a patient with a large tubulo-villous duodenal adenoma associated with a carcinoma of the ampulla of Vater. Despite of lacking pre- and intraoperative evidence of malignancy a Whipple operation was performed because of extensive growth of the duodenal adenoma and its location near the ampulla of Vater. The carcinoma of the ampulla could be diagnosed only postoperatively in the resected duodenum. The problems of the presented case and the operative procedure is discussed based on a review of the literature.

The barium enema examination is the most frequently done initial diagnostic evaluation for disease of the lower gastrointestinal tract. Adequate preparation of the colon is essential because inadequate preparation necessitates repetition, with patient discomfort, decreased caloric intake, and increased cost. A patient with Streptococcus bovis endocarditis, on complete gastrointestinal evaluation had only an adenomatous polyp of the sigmoid colon. Twenty-eight months later she was found to have invasive cecal carcinoma. Periodic evaluation of the gastrointestinal tract is therefore recommended for all patients with a history of S. bovis septicemia.

On the basis of an own observation of polyposis coli combined with lipomatous hamartoma of the corpus callosum problems of the genesis of lipomas of the central nervous system and its coverings are discussed. Colectomy was performed in a 37-year-old man because of polyposis coli. His sister suffers from the same disease. The patient died 25 d after the operation as a result of diffuse peritonitis. Colonoscopy in complicated diverticular disease may assist the clinician in establishing the diagnosis of associated strictures. A review of the literature shows that carcinoma was identified in 21 (17%) of 125 patients and that an additional diagnosis was made in 40 (32%) patients. Posterior extrasphincteric rectotomy gives good access to the mid-rectum 8-15 cm from the anus. Proctosigmoidoscopic examinations were performed on 363 patients who had gastrointestinal but no colonic symptoms. Thirty-four, all over the age of 40, were found to have polypoid lesions, 24 of which were adenomatous. Air-contrast barium enemas were utilized to rule out higher lesions in the patients with one or more adenomatous polyps. One early carcinoma of the ascending colon was so discovered. The cost of finding an adenomatous polyp in a patient without colonic symptoms was calculated to be $523.75 and of a carcinoma $12,570.
