Microsurgical reconstruction of the head and neck: interdisciplinary collaboration between head and neck surgeons and plastic surgeons in 305 cases.

Three hundred five microsurgical free flaps have been performed for defects of the head and neck by a team of two head and neck surgeons and … ryngo-esophageal reconstruction, respectively. There still remains no universally accepted flap for mandibular reconstruction, but the fibular osteocutaneous flap and a reconstruction plate protected by a radial forearm flap have largely superseded the iliac crest and scapular osteocutaneous flaps. Radical resection of tumors of the head and neck with immediate reconstruct …

Mesenchymal Stem Cells-Derived Exosomes Combined With Bone Grafts Ameliorate Bone Regeneration in Mandibular Defects.

Although scaffold materials serve as effective bone substitutes for mandibular reconstruction, their high resorption and biodegradation rates decrease the success of the therapeutic effect. This study aims to explore the bone regenerative potential of bovine-derived osteoid matrix xenografts coupled with cell-free treatments. The study was conducted as a randomized in vivo experiment to repair critical-sized defects in rabbit mandibles. Two treatment groups were established with 10 rabbits: the right hemimandibles were reconstructed with xenograft (control), and the left hemimandibles were rec…

Use of the Teres Major Muscle in Chimeric Subscapular System Free Flaps for Head and Neck Reconstruction.

IMPORTANCE: We present what we believe to be the first case series in which the teres major muscle is used as a free flap in head and neck r … protection for vascular anastomoses and/or great vessels and to enhance soft-tissue coverage of the mandibular reconstruction plate. In addition, the muscle was selected as a substrate for skin grafting where inadequate neck skin remained. Flap survival occurred in 10 of 11 flaps (91%). Two flaps (18%) demonstrated venous congestion that was managed successfully. Two patie …

Feasibility analysis of bone density evaluation with Hounsfield unit value after fibula flap reconstruction of jaw defect.

BACKGROUND: Implant-supported dentures have become an essential means of restoring occlusal function after jaw reconstruction. Bone mineral density (BMD) may influence the success rate of implant denture restorations. This study aimed to explore whether the Hounsfield unit (HU) value can be used to monitor the changing trend of fibular BMD after jaw reconstruction. RESULTS: A total of 54 patients who underwent maxillar/mandibular reconstruction with a fibula flap were included in this study. There was a significant correlation between the HU value and BMD at 1 week, 3 months, and 6 months aft…

Functional improvements after orthodontic-surgical reconstruction in a patient with multiple maxillofacial fractures.

Patients with multiple craniofacial fractures often suffer from stomatognathic problems after their primary treatment, because administering emergency care is the clinician's highest priority. Therefore, optimal bone repositioning is sometimes difficult because bone fixation is delayed. Moreover, neither an adequate radiographic examination nor an evaluation of primary occlusion is available during the repair of fractured bones. The lack of these assessments can also lead to occlusal dysfunction after bone fixation. As a result, patients with craniofacial fractures often require occlusal recon

Virtual Reality Planning for Mandible Reconstruction: A Case Report.

Mandibular reconstruction with a free fibula flap is considered the gold standard for implant-based prosthodontic rehabilitation. Doing so with presurgical virtual planning has become commonplace, resulting in procedure-specific osteotomy guides and fixation plates, but is limited by a cumbersome user experience. A new virtual reality (VR)-based mandible reconstruction application, ImmersiveRecon, could address these shortcomings. This case report discusses a free fibula-based mandible reconstruction and osseointegrated dental implant placement surgery planned in VR. A 17-year-old man with a h…
