Occurrence of basal cell carcinoma (BCC) following radiotherapy for tinea capitis is well known. The aim of this study was to specify the clinical and histological features of these BCC seen in 33 patients (1995-2000). Twenty seven men and six women were diagnosed with BCC. The age of onset varied between 32 an 62 years. Radiotherapy was received between 5 and 17 years of age. The interval between irradiation and the onset of carcinoma varied between 21 and 51 years. A case of basal cell carcinoma presenting in an unusual manner as orbital myiasis is reported. Orbit was full of maggots. Histopathology examination from the margin of the ulcerative area confirmed the diagnosis of basal cell carcinoma, which was infested secondarily with larvae of flies. Turpentine oil packs were used to remove the maggots.

A new atraumatic technique for identifying the medial cut end of a surgically lacerated canaliculus is described. The semicircular flap of Tenzel has proved to be an excellent reconstructive procedure for central eyelid defects. Although originally described for defects measuring half of the eyelid, we have found that it may be used for defects greater than 60% in the lower eyelid, if the lower eyelid retractors and inferior orbital septum are identified and severed from their attachments. This is the key factor in the mobilization and advancement of this flap. It may, in many circumstances, obviate the need for a more complicated reconstructive procedure.

Pigmented basal cell epithelioma (BCE) arose in the scar of an onchocercoma nodule in a 45-year-old white man. There were extensive fibrosis, inflammation, epidermal hyperplasia, and nests of basal cancer cells. The authors believe that the development of epithelioma in this case was more than a chance occurrence. There is little doubt that the malignant change was subsequent to the scarring-proliferative process of the disease. To the authors' knowledge, this complication has not been reported previously.

The use of 5-fluorouracil in three patients with basal cell carcinoma was followed by a peculiar necrosis which, when curetted, shelled out completely like a snake skin. We have termed this "ecdysis." Histologic examination revealed tumor necrosis, leukocytes, and fibrin. There have been no recurrences to date.

In a 52-year old woman, basal cell carcinoma of the back skin recurs after excision and the axillary lymph nodes are involved. This case is to be added to the list of basal cell carcinomas with metastatic evolution. The histologic study of a series of sections allows to show the migration of neoplastic cells in the vessels. Though exceptional, the ability of the basal cell carcinomas to yield metastases must be taken into account.
