Persistent pulmonary hypertension of the newborn: not a honeymoon anymore.

Persistent pulmonary hypertension of the newborn is a disorder of transition to extrauterine life, in which the newly born baby cannot decrease the high pulmonary vascular resistance and low pulmonary blood flow, characteristic of the fetus, to that of a low pulmonary vascular resistance and high pulmonary blood flow necessary for postnatal survival. The syndrome primarily affects the neonate 34 weeks postmenstrual age and greater. The article will summarize the latest understanding of the pathophysiology and review innovations in management strategies that have greatly decreased mortality and

A case of shaken baby syndrome after discharge from the newborn intensive care unit.

Preterm infants may be at higher risk of physical abuse after hospital discharge. Nonaccidental or inflicted head neurotrauma is the most common cause of mortality and morbidity in physical-abuse cases, and shaken baby syndrome (SBS) is the most common form of abuse. In the majority of the cases, parents who shake their infant do not intend to harm the infant. This article presents a report of a former preterm infant who presented to the pediatrician's office with a maternal report of an accidental fall. Shaken baby syndrome was suspected based on bilateral subdural hemorrhages of varying ages

Heart rate variability responses of a preterm infant to kangaroo care.

OBJECTIVE: To examine the effect of kangaroo care on heart rate variability in a healthy preterm infant. DESIGN: Case study. SETTING: Private room on a postpartum unit. PARTICIPANT: A mother-preterm infant dyad. INTERVENTION: Kangaroo (skin-to-skin) care. MAIN OUTCOME MEASURE: Heart rate variability, a noninvasive measurement of the sympathetic and parasympathetic components of the autonomic nervous system's influence on heart rate. RESULTS: Heart rate variability, especially the parasympathetic component, was high when the infant was fussy in the open crib, indicating increased autonomi

Developing context-sensitive, comprehensive newborn care protocols: integrating technologies with clinical care pathways for level 2 newborn units in Kenya.

BACKGROUND: An estimated 2.3 million neonates die worldwide each year (47% of under-five mortality), with 75% occurring during the first week of life. The burden is highest in sub-Saharan Africa (n/N = 27/1000 neonatal mortality rate) and largely results from preventable conditions, such as prematurity, birth asphyxia, and infections. The Newborn Essential Solutions and Technologies (NEST360) programme supports health systems in resource-constrained settings (RCSs) through appropriate technologies, training, data use, and mentorship to reduce preventable neonatal deaths. The NEST360 programme,

A study to assess knowledge and acceptability of foetal surgery for spina bifida amongst healthcare professionals in the UK.

Foetal surgery to repair open spina bifida before birth improves infant motor function and reduces ventriculoperitoneal shunt requirement. Alongside the development of the first UK foetal surgery service, we surveyed the knowledge and acceptability of this treatment to healthcare professionals in the UK and Ireland via an electronic questionnaire. Ninety-eight (98) responses were received: MFM clinicians (21), midwives (20), paediatric neurosurgeons (17), obstetricians (15), neonatologists (13), theatre nurses (11) and commissioners (1). Overall 70% of responders agreed with the concept that f

LMA Supreme for neonatal resuscitation: study protocol for a randomized controlled trial.

BACKGROUND: The most important action in the resuscitation of a newborn in the delivery room is to establish effective assisted ventilation. The face mask and endotracheal tube are the devices used to achieve this goal. Laryngeal mask airways that fit over the laryngeal inlet have been shown to be effective for ventilating newborns at birth and should be considered as an alternative to facemask ventilation or endotracheal intubation among newborns weighing >2,000 g or delivered ≥34 weeks' gestation. A recent systematic review and meta-analysis of supraglottic airways in neonatal resuscitation

'Opportunity to bond and a sense of normality': Parent and staff views of cuddling babies undergoing therapeutic hypothermia in neonatal intensive care: 'CoolCuddle'.

BACKGROUND: Currently, parents whose sick babies are undergoing three days of cooling therapy for hypoxic-ischaemic encephalopathy in neonatal intensive care units (NICUs) are not permitted to cuddle their cooled babies, due to concerns of warming the baby or dislodging breathing tubes or vascular catheters. Parents want to stay and care for their cooled babies and have reported that bonding is adversely affected when they are not permitted to hold them. DESIGN AND PARTICIPANTS: Qualitative interviews with 21 parents of cooled babies in NICU (11 mothers and 10 fathers) and 10 neonatal staff (

Neonatal resuscitation--a practical assessment.

Current European neonatal resuscitation guidelines (Zideman et al. Resuscitation 1998;37:103-110) advocate cardiopulmonary resuscitation (CPR) at 120 compressions per minute in a ratio of 3:1. This is commonly interpreted as a net rate, thus requiring delivery of 40 breaths per minute, which is the upper end of the range of 30-40 breaths per minute suggested in the guidelines. At least one other interpretation is available, but the correct one is not specified. No studies have evaluated whether the rates inferred by the guidelines can be achieved. This study assesses the ability of trained hos

Antenatal Care Attendance and Multiple Micronutrient Supplementation Intake: Perspectives from Women and Antenatal Care Service Providers in Rwanda.

BACKGROUND/OBJECTIVES: Emerging evidence suggests that multiple micronutrient supplements (MMS) provide additional benefits for maternal and neonatal health compared with iron and folic acid (IFA) supplements. To achieve effective coverage, acceptability, and adherence-and to inform a nationwide rollout of MMS-it is essential to understand the context-specific factors that shape implementation. This study evaluated the pilot implementation of MMS in Rwanda to identify key enablers, areas for improvement, and challenges related to antenatal care (ANC) attendance and MMS use. METHODS: Data were

National Initiatives to Improve Systems for Postpartum Care.

Purpose To showcase several current national initiatives that focus on reducing maternal mortality and severe maternal morbidity and promote postpartum health and wellness for all women. Description Maternal injuries and deaths are a serious public health concern with tremendous impact on families, communities, and healthcare providers. Over the past two decades, it has become apparent that the timing of serious maternal complications has shifted, with more than half of deaths occurring in the immediate postpartum period up to 1 year following birth. Many of these reported deaths could have be

The resuscitation team members' lived experiences regarding futile cardiopulmonary resuscitation in children and newborn infants: A phenomenological study.

Futility is difficult to define in the context of Cardiopulmonary Resuscitation , as it refers to the low likelihood of success. Healthcare providers face challenges when performing pediatric/neonatal Resuscitation. This study aimed to explore the lived experiences of resuscitation team members regarding futile Cardiopulmonary Resuscitation in children/newborns. A qualitative descriptive phenomenological approach was used with purposive sampling, resulting in 11 members of the Cardiopulmonary Resuscitation team for children/newborns (7 nurses/4 physicians). Data were collected through in-depth

Effect of Skin to Skin Contact between the Biological Mother and Their Baby Who Born Via Surrogacy on Mother Infant Attachment.

BACKGROUND: The attachment between mother and infant, beginning from pregnancy and reaching a peak with skin-to-skin contact, has a considerable effect on infants' growth and development. As stress and anxiety in contact with infants and unwillingness to embrace them have been reported in biological mothers in surrogacy, this study was conducted to assess the effect of skin-to-skin contact between the biological mother and their baby who born via surrogacy on mother-infant attachment. MATERIALS AND METHODS: In this clinical trial, 50 biological mothers were randomly assigned to either interve

The impact of pandemic restrictive visiting policies on infant wellbeing in a NICU.

BACKGROUND: Given the countrywide lockdown in the first pandemic period and the respective Hospital restrictive policies, we aimed to investigate if the SARS-COV-2 pandemic was associated to a reduced parental presence in the NICU and in which form this had an impact on infant wellbeing. METHODS: Retrospective cohort study about altered NICUs parental presence (measured by number of visits and kangaroo care time) due to pandemic restrictive policies and its impact on infant wellbeing (measured through The Neonatal Pain Agitation and Sedation scale and nurses' descriptive documentation). The relationship between rooming-in/not rooming-in and breast-feeding variables.

We studied the relationship between rooming-in/not rooming-in and breast-feeding variables such as breast feeding frequency, breast milk intake, supplements of other human milk or 5% glucose solution, cumulative weight loss, weight recovery and hyperbilirubinemia. We found that the breast feeding frequency was significantly higher in infants rooming-in than in those not rooming-in. Intake of breast milk on days 3 and 5 was significantly lower and maximum weight loss was significantly higher in infants rooming-in than in those not rooming-in. Infants rooming-in also had less supplement of other

After neonatal care, what next? A qualitative study of mothers' post-discharge experiences after premature birth in Kenya.

BACKGROUND: Approximately 15 million babies are born prematurely every year worldwide. Sub-Saharan Africa (SSA) and Asia account for more than half of the global preterm deliveries. Prominent healthcare structural and socio-economic factors in SSA, for example poverty and weak health systems, amplify vulnerabilities for mothers and premature babies; often leading to poor outcomes. Post-discharge mortality rates are high, and readmission is common. For mothers of premature babies, the transition home from hospital is marked by challenges and uncertainties. This study explored the post-discharge
