Esophageal malposition is a potentially disastrous complication of attempted airway intubation. We report an unusual case in which a promptly recognized esophageal intubation aided detection of a perforated gastric ulcer. After the endotracheal tube was repositioned and the ulcer was surgically repaired, our patient had an excellent outcome.

Complications following use of the rectal suction biopsy technique in the diagnosis of Hirschsprung's disease, are rare. In a series of 1,340 consecutive biopsies, complications included three clinical perforations of the bowel, one resulting in death, and three rectal hemorrhages requiring transfusion. A plea is made for the use of greater care in this technique.

Gastric remnant or duodenal perforation after Roux-en-Y gastric bypass (RYGB) or one-anastomosis gastric bypass (OAGB) is rare and may be missed radiologically. We systematically reviewed PubMed, Scopus, and Web of Science and included 26 reports (34 patients), alongside a video-illustrated late OAGB case.
