Eight and a Half Syndrome: A Rare Case of Pontine Infarction.

INTRODUCTION: This case report describes "one-and-a-half" syndrome with supranuclear facial palsy. CASE: 39 years old male hypertensive patient presented with right facial nerve palsy, internuclear ophthalmoplegia and conjugate gaze palsy. Neuroimaging showed acute posterior pontine infarct at medial longitudinal fasciculus, paramedian pontine reticular formation, abducens nerve nucleus and seventh nerve fascicular zone. OBSERVATION: "One-and-a-half"syndrome is characterized by a lateral gaze palsy in one direction and internuclear ophthalmoplegia in the other. Our patient had Right sided on

Injury of the cortico-ponto-cerebellar tract in a patient with mild traumatic brain injury: A case report.

RATIONALE: We report on a patient with injury of the cortico-ponto-cerebellar tract (CPCT) following mild traumatic brain injury (TBI), diag … MES: On 20-month DTT, the left CPCT showed tearing at the level of the subcortical white matter and pons, and discontinuation at the cerebellar portion. However, the integrity of the DRTT was well-preserved in both hemispheres. LESSONS: Using DTT, injury of the CPCT was demonstrated in a patient with ataxia and tremor following mild TBI.

Profuse unilateral hyperhidrosis induced by urinary retention in a stroke patient.

INTRODUCTION: The diagnosis of urinary retention in stroke patients is challenging because their symptoms may be subtle, or go unnoticed. Furthermore, patients suffering from aphasia or impaired cognition often have difficulties communicating urination problems, leading to their urinary retention being overlooked. CASE REPORT: We report a 53-year-old man with bilateral pontine hemorrhage resulting in incomplete locked-in syndrome. During the patient's hospital course, he developed 3 episodes of rapid onset of profuse unilateral hyperhidrosis. All 3 episodes were found to be induced by an over

Wolfram syndrome presenting marked brain MR imaging abnormalities with few neurologic abnormalities.

Wolfram syndrome is a rare autosomal recessive disorder featuring diabetes insipidus, diabetes mellitus, optic atrophy, and deafness; DIDMOAD is a commonly accepted anonym for this disorder. We describe a 35-year-old man with Wolfram syndrome, who had marked atrophy of the brain stem, middle cerebellar peduncle, and cerebellum. Despite these MR imaging findings involving the pontocerebellar tract, the patient had no neurologic abnormalities suggesting dysfunction of the brain stem or cerebellum. Patients with Wolfram syndrome may have discrepancies between neurologic and radiologic findings.

Central pontine myelinolysis, an update.

Central pontine myelinolysis (CPM) can be regarded as one of the demyelinating syndromes. First described by Adams et al. in 1959 in their chronic alcoholic patients, it has now been described in the malnourished, the chronically debilitated, the renal, the hepatic and the transplant patient among others. Pathologically, it is defined as a symmetric area of myelin disruption in the center of the basis pontis, although similar symmetric lesions have also been described occurring with CPM as well as independently in other brain areas (extrapontine myelinolysis or EPM) including the cerebellar an

[A juvenile case of locked-in syndrome living long after pontine infarction].

A juvenile case of locked-in syndrome (LiS) caused by pontine infarction was reported. A 26-year-old woman suddenly complained of speech disturbance, weakness of the left upper and lower limbs, and occipital headache. She was admitted to an emergency hospital. Magnetic resonance imaging (MRI) showed a high intensity area on T2-weighted image at the ventral portion of the pons. She was transferred to our hospital for further treatment. On admission, she was mute and quadriplegic, but responded to our questions with vertical eye movements and blinks. Vertebral and carotid angiography demonstrate…

Pontocerebellar hypoplasia--how many types?

An original article describes a sibship with early fatal pontocerebellar hypoplasia of a yet unclassified type, with the accompanying features of polyhydramnios and neonatal myoclonus. Autopsy in one patient excluded spinal anterior horn involvement, which argues against pontocerebellar hypoplasia type I (PCH-1). The present PCH classification and literature are briefly reviewed. Four previous publications bear similarity to the present report. Definite classification as a genetically separate entity, however, remains elusive pending localization and identification of the gene(s) involved.

Cellular basis of pontine ponto-geniculo-occipital wave generation and modulation.

1. Pontogeniculooccipital (PGO) waves are recorded during rapid eye movement (REM) sleep from the pontine reticular formation. 2. PGO wave-like field potentials can also be recorded in many other parts of the brain in addition to the pontine reticular formation, but their distribution is different in different species. Species differences are due to variation in species-specific postsynaptic target sites of the pontine PGO generator. 3. The triggering neurons of the pontine PGO wave generator are located within the caudolateral peribrachial and the locus subceruleus areas. 4. The transferring

Transient atrial fibrillation associated with ischemic pontine stroke. Case report.

Atrial fibrillation (AF) and other cardiac arrhythmias may occur after stroke in absence of cardiac disease. We report a case of pontine ischemic stroke with paroxysmal atrial fibrillation of spontaneous resolution. Cardiac investigations were normal. We propose that the AF was the consequence of the stroke.

[Auditory evoked potentials and magnetic resonance in the diagnosis and follow-up of central pontine myelinolysis].

We report the case of a woman who developed central pontine myelinolysis in spite of gradual correction of hyponatremia. The good clinical evolution as well as the influence of rapid correction of serum sodium concentration in the unfolding of this rare condition are discussed. Several diagnostic procedures, mainly auditory-evoked responses and magnetic resonance imaging are also analysed. We remark the particular interest of the auditory-evoked responses in the attestation of an eventual remyelination.

Duration of retrograde amnesia induced by tetrodotoxin inactivation of the parabrachial nuclei is inversely related to the intensity of footshock in rat's passive avoidance response.

Previous work has demonstrated that in rats post-trial bilateral functional blockade of the parabrachial nuclei by local injection of tetrodotoxin (10 ng in 1 microliter) partially disrupts retention of an overtrained passive avoidance response. The time-course of functional blockade disrupting effects on passive avoidance response has been studied by altering both the acquisition-tetrodotoxin injection interval and footshock intensity as independent variables. When week footshocks (0.8 mA) were delivered, functional ablation of the parabrachial nuclei was still effective when induced up to 8 …

[Total deafness after multiple pontine infarctions. Dolichoectasia of the basilar trunk].

A 54 year-old man was affected by three successive infarctions in the vertebro-basilar territory. These infarctions were related to a dolichoectatic basilar artery visualized by arteriography and NMR. Deafness occurred first on the left side and then, after the third infarction, on the right side. The authors underline that deafness can be observed after a pontine infarction in the territory of the anterior inferior cerebellar artery. A dolichoectatic basilar artery can be the source of thrombotic or embolic strokes. Their prevention by antiaggregant or anticoagulant therapy is suggested.
