The problem with and benefit of ventilations: should our approach be the same in cardiac and respiratory arrest?

PURPOSE OF REVIEW: Recent advances in cardiopulmonary resuscitation have led to greater understanding of cardio-cerebral-pulmonary interactions during the process. The purpose of this discussion is to update the physiologic understanding of these interactions during cardiopulmonary resuscitation, review the detrimental and beneficial effects of ventilation, and identify implications for clinical practice. RECENT FINDINGS: There is an inversely proportional relationship between mean intrathoracic pressure, coronary perfusion pressure, and survival from cardiac arrest. Increased ventilation rat

Screening adults for Asperger Syndrome using the AQ: a preliminary study of its diagnostic validity in clinical practice.

The Autism Spectrum Quotient (AQ) has been developed to measure the degree to which an adult with normal intelligence has autistic traits. In this paper it is evaluated for its potential as a screening questionnaire in clinical practice on one hundred consecutive referrals to a diagnostic clinic for adults suspected of having Asperger Syndrome or high functioning autism (AS/HFA). The results indicate that it has good discriminative validity and good screening properties at a threshold score of 26. The implications of these results are discussed.

Management of unstable/acute slipped capital femoral epiphysis: results of a survey of the POSNA membership.

There is limited literature regarding the treatment of unstable slipped capital femoral epiphysis (SCFE). The Evidence Analysis Work Group (EAWG) initiated a questionnaire assessing management of unstable SCFE by the members of the Pediatric Orthopaedic Society of North America (POSNA). All members were surveyed by e-mail and fax. The survey was accessible through the POSNA web site. One third of the members responded. Seventy-three percent of members used the "stable/unstable" classification, while 27% used the "acute/chronic" terminology. Thirty-one percent of respondents felt that an unstab

[Diagnosis and treatment of hypogonadism in adult males].

Hypogonadism in men is defined as endocrine dysfunction of the testes, and due to reduced serum testosterone levels leads to symptoms of testosterone deficiency. Depending on the location of disruption in the endocrinological cycle, hypogonadism is classified as primary, secondary, or tertiary. In primary hypogonadism, the production of testosterone in the Leydig's cells of the testes does not function properly. Serum LH concentrations are elevated in the sense of counterregulation (hypergonadotropic hypogonadism). In secondary hypogonadism, LH secretion (and usually also FSH) from the hypophy

Screening for psychosocial risk factors in patients with coronary heart disease-recommendations for clinical practice.

Psychosocial risk factors like low socio-economic status, chronic family or work stress, social isolation, negative emotions (e.g., chronic depression or acute anxiety), and negative personality patterns such as Type-D-pattern or hostility, may contribute significantly to the development and adverse outcome of coronary heart disease. Therefore, systematic screening for psychosocial risk factors in cardiological practice is recommended in order to initiate adequate intervention strategies, e.g., to involve additional psychosocial counselling or treatment. Reliable methods to assess psychosocial

Cancer-related fatigue. Clinical practice guidelines in oncology.

These guidelines propose a treatment algorithm in which patients are evaluated regularly for fatigue using a brief screening instrument, and are treated as indicated by their fatigue level. The algorithm's goal is to identify and treat all patients with fatigue that causes distress or interferes with their daily activities or functioning. Management of fatigue begins with primary oncology team members who perform the initial screening and either provide basic education and counseling or expand the initial screening to a more focused evaluation for moderate or higher levels of fatigue. At this

The current practices, training and concerns of a group of hospital-based speech therapists working in the area of dysphagia.

The aim of the study was to investigate the current practices, training, and concerns of a group of hospital-based speech therapists regarding the area of dysphagia practice. Questionnaires were posted to 75 speech therapists working in the major hospitals around Gauteng, Kwa-Zulu Natal and the Cape Province and responses were obtained from 50 therapists within the stipulated time. Closed-ended questions were analysed quantitatively using descriptive statistics, while open-ended items were subjected to content analysis of themes articulated by respondents. Results indicated that the overwhelmi

Evaluating pterygium severity: a survey of corneal specialists.

PURPOSE: Accurate and reliable evaluation techniques are essential for clinical and epidemiologic studies. This survey of corneal specialists was designed to lay a foundation for the further development of methods for evaluating and staging pterygium. METHODS: In a self-administered, mailed questionnaire, 213 corneal specialists rated the importance of nine symptoms, nine signs, and nine clinical tests for the severity of primary pterygium. Severity was defined as the present need for surgical intervention. RESULTS: The most important factors for determining primary pterygium severity were t

[Reduced utilization of antibiotics in Swedish hospitals. Considerable variations between hospitals concerning choice and use of preparations].

Defined daily dose (DDD) consumption of antibiotics in 26 Swedish hospitals decreased by an average of 25% from 1990 to 1998, according to statistics from Apoteket AB. In one hospital, the decrease was 61%. Average consumption of tetracyclines and penicillins, expressed as percentage of total antibiotics consumption, decreased by 5% and 7% respectively, while quinolones and cephalosporins increased by 4% and 7% respectively. Consumption of tetracyclines and quinolones varied between individual hospitals by a factor of four.

Short-lived effects of a formulary on anti-infective prescribing--the need for continuing peer review?

Because of the introduction of indicative drug budgets in the UK and their likely association with prescribing from a drug formulary, we have studied the effect of the introduction without continued reinforcement of a voluntary practice formulary on antimicrobial prescribing in a group practice. Detailed prescribing feedback for two matched periods before and after the launch of the Northern Ireland Faculty of the Royal College of General Practitioners practice formulary was compared. The parameters chosen were adherence to the formulary, generic prescribing and costs. A slight increase in pre

Telling their stories: primary care practitioners' experience evaluating and reporting injuries caused by child abuse.

OBJECTIVE: To learn about primary care physicians' experiences in identifying and reporting injuries caused by physical abuse. METHOD: Two qualitative analysts facilitated a focus group of six Chicago area, primary care physicians. Physicians representing diverse practice settings were selected to participate in the discussion. The analysts reviewed and analyzed the audiotape transcription of the focus group for common emergent themes. RESULTS: Two themes emerged: (1) the importance of the participants' own past experience identifying and reporting suspected child abuse and (2) the responsib

Impact of medical and nonmedical factors on physician decision making for HIV/AIDS antiretroviral treatment.

To examine influences of medical factors (e.g., viral load) and nonmedical factors (e.g., patient characteristics) on treatment decisions for highly active antiretroviral therapy (HAART), we sent a survey to a random sample of 995 infectious disease physicians who treat patients with HIV/AIDS in the United States in August, 1998. The response rate was 53%. Respondents were asked to report their current practices with respect to antiretroviral treatment and the extent to which each of three medical and 17 nonmedical factors would influence them for or against prescribing HAART to a hypothetical

The behavior of Italian family physicians regarding the health problems of women and, in particular, family planning (both contraception and NFP).

The hospital-centered trend that has dominated medical culture and the management of health care during this entire century has, in the last few years, undergone a reversal in Italy. Conditions in other countries suggest that similar changes have or will become increasingly common. The family physician today manages many of the functions previously handled by hospitals and specialists. In the field of reproductive health, family physicians are responsible not only for diagnosis and treatment, but also for prevention and education. The present study considers this new context with the objective

Physicians' perspectives on antimicrobial resistance and rational antibiotic prescribing.

BACKGROUND: Antimicrobial resistance poses a growing global threat. In real-world practice, antimicrobial prescribing is shaped by clinicians' cognitive biases and perceptions of resistance risk, potentially driving unnecessary broad-spectrum antibiotic use and increased antimicrobial pressure. METHODS: We conducted a questionnaire-based study among physicians to assess perceptions on antimicrobial resistance magnitude, determinants of prescription, and barriers to de-escalation. Clinician-estimated resistance rates were compared with observed data, and the main factors influencing antimicrob

Diagnostic accuracy and management of suspected acute otitis media: A multi-continental survey of primary care physicians using tympanic membrane images in clinical cases.

BACKGROUND: Acute otitis media (AOM) is a leading cause of primary care visits and antibiotic prescriptions in children. Diagnostic uncertainty and variability in management contribute to misdiagnosis and inappropriate antibiotic use. OBJECTIVE: This study examines how primary care physicians across countries diagnose and manage suspected AOM through clinical cases. METHODS: International cross-sectional survey using structured clinical cases with validated tympanic membrane images. The survey was administered during professional meetings for primary care physicians in 12 countries (Argentin

Addressing gaps in asthma management during childbearing age and pregnancy: insights from a survey of Italian physicians and patients.

BACKGROUND: Asthma is a common condition among women of childbearing age, requiring careful management, particularly during pregnancy. Despite existing guidelines, significant gaps remain in asthma management during pregnancy, notably for women with moderate-to-severe asthma. AIM: This study aimed to explore the awareness, limitations, and challenges of asthma management during childbearing age and pregnancy from both asthmatic women (AW) and physician perspectives in Italy. Additionally, it sought to identify unmet needs and collect real-life experiences from Italian centers specialized in s

Screening for torticollis and plagiocephaly: The role of the pediatrician.

The purpose of this study was to observe the effects of providing a standardized screening examination method to a group of attending and resident physicians on the rate of torticollis and plagiocephaly diagnoses, the age at time of diagnosis, and the attitudes and practice patterns of the physicians. All subjects were given a standardized screening procedure by verbal instruction and handout, and also completed a pre- and post-intervention questionnaire, which assessed the subjects' practice regarding the diagnosis and management of torticollis and plagiocephaly. A retrospective chart review
