We present a rare complication of percutaneous osteosynthesis of a scaphoid fracture. A 19-year-old professional handballer presented with a fracture of the scaphoid classified B2 according to the Herbert classification: the fracture was fixed with a percutaneous screw. The fracture was united by six weeks despite resumption of training eight days postoperatively. Subsequently, the patient presented with pain over the flexor carpi radialis (FCR) tendon together with reduction of wrist flexion strength suggested an FCR tendonitis. A CT scan with injection eliminated the diagnosis of non-union a

We conducted a systematic quantitative meta-review of the literature to provide evidence-based suggestions for the treatment of scaphoid nonunion. This search identified 1,121 articles of which 36 met eligibility requirements. In unstable nonunions, screw fixation with grafting at 94% union was superior to K-wires and wedge grafting (77% union). Immediate mobilization versus 6 weeks or more of casting showed the same union rate of 74%. For avascular necrosis of the proximal fragment, union was achieved in 88% of those patients with a vascularized graft versus 47% with screw and wedge fixation.

AIMS: The aim of this study was to describe the introduction of a virtual pathway for the management of patients with a suspected fracture of the scaphoid, and to report patient-reported outcome measures (PROMs) and satisfaction following treatment using this service.
METHODS: All adult patients who presented with a clinically suspected scaphoid fracture that was not visible on radiographs at the time of presentation during a one-year period were eligible for inclusion in the pathway. Demographic details, findings on examination, and routine four-view radiographs at the time of presentation w

PURPOSE: Four-corner arthrodesis with scaphoid excision has been shown to be an acceptable method for treating wrist degenerative changes. Some recent studies have identified higher complication rates when circular plates are used. This study examined a consecutive case series with defined technique for outcome and complications.
METHODS: A retrospective assessment was performed in a consecutive cohort of 28 patients who underwent a standardized 4-corner arthrodesis with a 2nd-generation circular plate and distal radius bone grafting for a diagnosis of scapholunate advanced collapse, scaphoid

We examined the mechanical response of the distal radius pre-fracture and at fracture under dynamic impact loads. The distal third of eight human cadaveric radii were potted and placed in a custom designed pneumatic impact system. The distal intra-articular surface of the radius rested against a model scaphoid and lunate, simulating 45° of wrist extension. The scaphoid and lunate were attached to a load cell that in turn was attached to an impact plate. Impulsive impacts were applied at increasing energy levels, in 10 J increments, until fracture occurred. Three 45° stacked strain gauge rosett

The present report is based on assessment of 48 patients who underwent carpal Silastic H.P. implant arthroplasty (trapezium, condylar, scaphoid, lunate and scapholunate implants). Mean follow-up period was 29 months (range 6-82). Recurrent pain and/or evidence of wrist synovitis and lytic lesions made early subsequent surgery necessary in 10 patients. In the remaining 38 patients the post-operative course was followed for an average period of 33 months (range 8-82). Severe giant-cell silicone synovitis combined with isolated or disseminated osteolytic lesions were found in 17/30 (56%) patients

BACKGROUND: Carpometacarpal joints can be affected by traumatic or degenerative pathology. Although different techniques have been described to treat these conditions, most authors agree that arthrodesis is an effective treatment modality. Vascularized bone grafts of the distal radius have been used to treat carpal conditions, such as scaphoid nonunion or Kiënbock disease, and they have been shown to have several advantages over nonvascularized bone grafts.
METHODS: We report a case of a carpal boss in a female patient treated with an arthrodesis of the second and third carpometacarpal joints

Unicameral bone cysts (UBC) and aneurysmal bone cysts (ABC) are benign cystic lesions of bone which are easily diagnosed. However, unusual locations may lead to a false diagnosis. Therefore the aim of this retrospective study was to determine the frequency of unusual localizations. The authors studied 451 cases with histopathologically confirmed diagnosis of UBC or ABC, seen between 1981 and 2012. In the UBC group (352 cases) humerus, femur and calcaneus were found to be the most common sites, while acetabulum, scapula, scaphoid, lunatum, metacarpals, metatarsals, toe phalanges and ulna each a

BACKGROUND: Scaphoid fractures are the most common carpal fractures. They often need to be treated by surgery, where the use of a compression screw is the globally accepted gold standard. Surgeons may choose between different implant materials including titanium alloys, which remain in the body or are removed after healing. An alternative are biodegradable magnesium-based implants. Properties of magnesium alloys include high stability, osteoconductivity, potential reduction of infections and few artifacts in magnetic resonance imaging (MRI). The aim of this trial is to demonstrate non-inferior

Scapholunate instability (SLI) is the most frequent cause of carpal instability. There is a plethora of repair methods with a trend towards minimally invasive techniques, with the general principles aimed at restoring the integrity of the ligament and physiological carpal kinematics. We introduce a novel, all-arthroscopic one-needle-one-suture repair technique as a potential repair technique for SLI. Utilising a single 0 FiberWire suture threaded through an 18-gauge (18-G) needle, this technique allows for precise suture placement within the scapholunate ligament complex. The suture is secured

Scaphoid fractures are common injuries with high risk of nonunion. Various fixation techniques exist for managing scaphoid nonunions, including Kirschner wires, single or dual headless compression screws, combination fixation techniques, volar plating, and compressive staple fixation. The indication for each fixation technique varies depending on the patient, type of nonunion, and clinical scenario.

Cast selection for conservatively treated acute scaphoid fractures remains controversial. Cast options include short arm versus long arm, and those that include the thumb or leave it free. We sought to investigate the role of how cast choice affects nonunion rates after conservative management of scaphoid fractures. We searched PubMed, Embase, and Google Scholar from inception through July 14, 2020, according to the Preferred Reporting Items for Systematic Review and Meta-Analyses guidelines. We extracted information of interest, including cast type, and non-union rates at the end of the treat

PURPOSE: Selecting treatment for scapholunate (SL) instability is notoriously difficult. Many methods of reconstruction have been described, but no procedure demonstrates clear superiority. New methods proposed use internal bracing (IB) with suture anchors and flat braided suture (FBS), alone or as an augmentation with tendon autograft for SL ligament injuries. Our goal was to use computed tomography (CT) to analyze alignment of the SL joint after 3 different modes of fixation of SL instability: after reconstruction with IB incorporating either tendon autograft or the dorsal intercarpal ligame

The aim of this study was to compare the accuracy and reliability of percutaneous fixation of minimally displaced scaphoid fractures using a customized three-dimensional (3-D)-printed guide with a conventional freehand method. A prospective cohort of ten patients underwent scaphoid fixation with the aid of a customized 3-D-printed guide. The final screw position, total surgery time (minutes) and fluoroscopy time (seconds) was compared with a retrospective cohort of ten patients who underwent fixation with a conventional technique. There were no differences in final screw position between both

Heterogeneity in the anatomical definition of 'proximal' affects the comparison of outcomes of these scaphoid fractures. This study aims to review published outcomes of all variants to determine both, differences in terminology, and union rate based upon definition. A literature search was conducted to identify articles that reported descriptions and union rate of all acute (<8 weeks of injury) proximal scaphoid fractures in adult patients (>16 years old). Proximal fractures were grouped as reported ('third', 'pole', 'fifth' or 'undefined'). The data were pooled using a fixed-effects method, a
