Cerebrospinal fluid eosinophilia in a child with neuroborreliosis.

Cerebrospinal fluid eosinophilia is rare and usually associated with eosinophilic meningitis caused by helminthic infections. It is also observed in bacterial or fungal meningitis (syphilis, tuberculosis, coccidioidomycosis), in patients with malignancies, ventriculoperitonial shunts, hypereosinophilic syndrome or allergy to some medications. Here we present a case of an 8-year-old boy admitted with fever and clinical signs of meningitis. Cerebrospinal fluid (CSF) analysis showed marked eosinophilia. Basing on further serological CSF testing the diagnosis of borreliosis was established. Cerebr

Neurocysticercosis: clinical manifestation, neuroimaging, serology and molecular confirmation of histopathologic specimens.

Diagnosis of neurocysticercosis (NCC) is usually based on neuroimaging and/or immunological analysis of cerebrospinal fluid (CSF) and/or serum samples for detection of specific antibodies against T. solium antigens. Additional confirmative diagnosis may be possible by morphological and molecular confirmation of resected histopathologic specimens. The majority of NCC cases do not always show typical neuroimaging figures with invaginated scolex. So, serology using highly specific antigens of T. solium, either semi-purified native or recombinant antigens, is essential for confirming NCC cases. Th

Update on the immunology, diagnosis and management of systemic lupus erythematosus.

Lupus occurs with a prevalence of 2-9 in 10,000 people, targeting female and indigenous populations in particular. Lupus and related systemic autoimmune syndromes (scleroderma, Sjögren's syndrome, rheumatoid arthritis and polymyositis) result from a similar set of genetically and environmentally modulated immune disturbances, and the diagnostic and management approach to these conditions is broadly comparable. Evanescent, vague symptoms, restrictive diagnostic criteria and low diagnostic suspicion might have resulted in the under-diagnosis of these problems in the past, imposing considerable b

Usefulness of serologic analysis as a predictor of the infecting serovar in patients with severe leptospirosis.

The diagnosis of leptospirosis is often made using the microscopic agglutination test (MAT), in which live antigens representing >20 serogroups undergo reaction with patient serum samples to detect agglutinating antibodies. Data derived from this assay are often used to infer the identity of the infecting leptospiral serovar or serogroup; however, paradoxical reactions and cross-reactions between serogroups are common. To evaluate the usefulness of this approach, data on culture-proven cases of leptospirosis that occurred in Barbados from January 1980 through December 1998 were reviewed. A tot

Current approaches to diagnosis and treatment of celiac disease: an evolving spectrum.

Celiac disease (CD) is a syndrome characterized by damage of the small intestinal mucosa caused by the gliadin fraction of wheat gluten and … e and specific algorithm that allows the identification of different manifestations of the disease. Serologic tests developed in the last decade provide a noninvasive tool to screen both individuals at risk for the disease and the general population. However, the current gold standard for the diagnosis of CD remains histologic confirmation of the intestinal damag …

[A new drug for serodiagnosis of listeriosis].

Development and introduction into practical public health of new rapid methods for laboratory diagnosis of listeriosis, that will be characterized by a high sensitivity and specificity, is still a pressing problem. Listeriosis erythrocytic diagnosticum developed at the Omsk Research Institute for Natural Focal Infections permits the detection of species-specific antibodies in patients' sera and convalescents after this infection and is characterized by a sufficiently high diagnostic efficacy. Use of this agent appears to hold good promise in the diagnosis of listeriosis in the acute period of

Neurological manifestations of Lyme borreliosis.

After the isolation of Borrelia burgdorferi, the previously unknown causative agent of Lyme disease, two neurological disorders, Bannwarth's syndrome and acrodermatitis chronica atrophicans-associated neuropathy, which have been known in Europe for decades, gained new interest. With the availability of serodiagnostic tests, a chronic debilitating disorder--progressive borrelia encephalomyelitis--was found to be caused by chronic infection with B. burgdorferi. Beside these typical manifestations, a growing number of publications about various neurological phenomena appeared, which were thought

[Severe pneumopathy due to Legionella pneumophila: report on two cases].

Two patients with severe forms of legionnaires' disease presented similar manifestations of a confusional syndrome, a major infectious syndrome resistant to cyclines, beta-lactams, and aminoglycosides, altered hepatic and renal functions, and a fulminating course with the need for assisted ventilation in one case. Indirect immunofluorescence serology gave very positive results in both patients and changes in serum levels were reported 6 and 8 months after the onset of the disease. A serotype I strain of Legionella pneumophila was isolated by direct immunofluorescence from bronchial aspiration
