Sleep. 4: Sleepiness, cognitive function, and quality of life in obstructive sleep apnoea/hypopnoea syndrome.

Sleepiness, cognitive performance, and quality of life are overlapping aspects of daytime function that may be affected in patients with obstructive sleep apnoea/hypopnoea syndrome. The evidence is compatible with hypotheses that these deficits are reversible with treatment, particularly for patients with severe disease.

[Sleep hypoxemia during CPAP treatment in patients with obstructive sleep apnoea syndrome with chronic hypercapnia].

UNLABELLED: The aim of the study was to find the clinical features of the hypercapnic OSA patients, indicating possible failure of CPAP treatment in preventing sleep hypoxemia. The material of the study consisted of 37 hypercapnic OSA patients (age 49 +/- 9 years, apnea index 50 +/- 15, daytime PaCO2 52 +/- 6 mm Hg). In 1 patient arterial oxygen desaturation persisted because of the replacement of all mixed apnoeas by central apnoeas. In 26 patients SaO2 during CPAP treatment (85 +/- 27% in NREM and 72 +/- 12% in REM sleep) was lower during sleep than baseline SaO2 (91 +/- 2%, p < 0.05), in sp…

Sleep and daytime sleepiness in upper airway resistance syndrome compared to obstructive sleep apnoea syndrome.

This study has investigated differences in the nocturnal sleep and daytime sleepiness among patients with obstructive sleep apnoea syndrome (OSAS), upper airway resistance (UARS), sleep hypopnoea syndrome, and normal control subjects, using sleep scoring and spectral activity analysis of the electroencephalogram (EEG). Twelve nonobese males with UARS aged 30-60 yrs were recruited. These subjects were strictly matched for age and body mass index with twelve OSAS patients, 12 sleep hypopnoea syndrome patients, and 12 normal controls, all male. Daytime sleepiness was evaluated using the Epworth S…

Peculiar respiratory response observed during sleep-onset REM sleep of an infant with Ondine's curse.

We treated an infant with congenital central hypoventilation syndrome ("Ondine's curse"). She was cyanotic and given ventilatory support at the first hour after birth. An investigation of sleep state and respiration performed at the age of 3 months led to this diagnosis. Hypoventilation persisted in all sleep stages, with the most severely reduced tidal volumes occurring during delta-wave sleep (stages 3 and 4). In addition, severe secondary reduction in tidal volumes occurred in sleep-onset REM sleep. This phenomenon was absent in non sleep-onset REM sleep. At 4 months of age, her respiratory…

Sleep-related breathing disorders. 5. Nasal continuous positive airway pressure treatment for obstructive sleep apnoea.

CPAP should be considered the first line of treatment in patients with moderate to severe obstructive sleep apnoea. In our centre in Sydney this generally means patients with more than 20 apnoea/hypopnoeas per hour with repeated dips in oxyhaemoglobin saturation and usually some symptomatology. Despite this first line role of nasal CPAP, recent objective studies question whether earlier enthusiastic reports on adherence to CPAP are correct. The role of technical innovations in new CPAP machines in improving usage remains to be tested. The "drop out" rate from physician selection for a CPAP tri…

Naltrexone improves blood gas patterns in obstructive sleep apnoea syndrome through its influence on sleep.

Endogenous opiates have been shown to depress ventilation, and could therefore play a role in sleep apnoea syndrome (SAS). Hence, opiate antagonists have been used to treat SAS. The improvement they seem to give in blood-gas monitoring could derive either from a direct blocking of endorphins that inhibit respiration or else, indirectly, through an influence on sleep patterns. The present study used a double blind cross-over protocol to investigate the relationships between the effects on blood-gas and on sleep patterns of the oral opiate antagonist naltrexone in obstructive SAS. Sleep patterns…

Periodic leg movement, sleep fragmentation and central sleep apnoea in two cases: reduction with Clonazepam.

Two subjects presented with periodic leg movement (PLM) syndrome during sleep that was characterized by marked sleep fragmentation and repetitive short central apnoeas. Treatment of PLM using Clonazepam, a benzodiazepine with hypnotic properties, markedly reduced the sleep fragmentation due to PLM and, despite its depressant properties on the central nervous system, controlled the repetitive central apnoeas. These two observations, although rare, give insight into the role of non-ventilatory variables in the development of sleep apnoea. Significant sleep fragmentation should be considered when…

Atopic Dermatitis and Sleep Disorders.

PURPOSE: Atopic dermatitis (AD) is a common inflammatory skin disease affecting 129 million people worldwide and is characterized by recurrent eczema, severe pruritus, and skin barrier damage. Previous studies on AD-sleep disorder associations were limited by small sample sizes or single-center designs, lacking large-scale epidemiological evidence. This study aimed to investigate the association between AD and sleep disorders using the UK Biobank prospective cohort and to explore effects of sex, race, and obesity on this relationship. METHODS: A prospective cohort design was employed, includi…

[Obstructive sleep apnea in adults].

Obstructive sleep apnea is a prevalent disorder characterized by symptoms such as snoring, apneas, excessive daytime sleepiness, and a variety of other clinical complaints. Dental comorbidities, including bruxism and xerostomia, are also commonly observed. If left untreated, OSA may result in significant cardiovascular and metabolic complications. The diagnosis is established through a sleep study, following a comprehensive clinical history and physical examination. Drug-Induced Sleep Endoscopy provides additional diagnostic insight, particularly in patients being evaluated for potential surgi…

[Sleep disorders and mental health].

Sleep disorders occur in almost one out of three people and are therefore among the most common disorders that physicians encounter in their practice. They often also present in the context of mental disorders. While it was previously assumed that they were merely a symptom of mental illness, current literature emphasizes the bidirectional connection between sleep and mental disorders. Even in the most common sleep disorders, namely sleep-related breathing disorders, insomnia, and restless legs syndrome / periodic limb movement disorder, reciprocal and sometimes complex interactions between sl…

Patient-Ventilator Interaction During Sleep.

Sleep in the ICU is subjectively and objectively poor. Patients in the ICU have short sleep duration and highly fragmented sleep. Poor sleep in the ICU is associated with worse outcomes, particularly the development of ICU delirium. Patients undergoing noninvasive or invasive mechanical ventilation struggle with sleep. For those patients on noninvasive ventilation, settings may need to be adjusted compared to wake to account for the physiological changes that occur with the sleep state. Specifically, upper airway collapsibility (ie, obstructive sleep apnea) must be considered as do changes in …

[Sleep disorders and oxidative stress].

Sleep disorders represent a significant medical and social issue, closely linked to the development of oxidative stress and cognitive impairment. This review summarizes current evidence for a bidirectional association between sleep disorders and oxidative stress. Sleep deprivation and chronic insomnia are associated with the accumulation of reactive oxygen species, mitochondrial dysfunction, neuroinflammation, and lipid damage, which together create a cycle that exacerbates sleep disturbances and contributes to cognitive impairment. The role of intermittent hypoxia in obstructive sleep apnea s…

Mitochondrial dysfunction in sleep deprivation.

Insufficient sleep gradually elevates oxidative stress and impairs mitochondrial function. Both oxidative stress and mitochondrial dysfunction are critical factors in the pathology of neurodegenerative disorders. Prolonged sleep deprivation SD ultimately leads to an imbalance of normal cellular processes and an overproduction of reactive oxygen species. The detrimental compounds can damage fat molecules, proteins, and even DNA, and, as a result, intracellular mitochondria, which produce energy, become dysfunctional, leading to reduced ATP production and further compromising the energy supply. …

Vitamin D and Sleep Disorders.

Sleep is a complex behavioral state necessary for neurological, physical, cognitive, metabolic, and cardiovascular health throughout the lifespan. Poor sleep increases the risk of mortality among the general population. Sleep deprivation causes memory deficits, neuropsychiatric diseases, mood disorders, metabolic syndrome, cardiovascular diseases, stroke, and neurodegenerative diseases and carries an increased risk of suicide and motor vehicle accidents. Vitamin D is a fat-soluble secosteroid and its inadequacy is associated with multiple disorders, including bone health, neuropsychiatric dise…

Sleep neurology: pearls and pitfalls.

Sleep disorders are among the most frequent and disabling conditions encountered in neurological practice. Chronic insomnia alone affects approximately 10% of adults, while other sleep disorders, including obstructive sleep apnoea, restless legs syndrome, circadian rhythm disturbances, parasomnias and narcolepsy, are often under-recognised and misdiagnosed. Over-reliance on investigations and inappropriate use of pharmacotherapy represents recurring pitfalls. Cognitive behavioural therapy for insomnia remains the gold-standard first-line therapy, yet access is limited and medications are frequ…
