Management of facial trauma: lessons of war.

Soft tissue trauma of the head and neck is encountered frequently by the battlefield head and neck surgeon. Repair of the soft tissue (as well as bony fractures) of the head and neck is undertaken with the intent of providing return of form and function to the injured patient. Aggressive management of head and neck wounds is necessary to prevent excessive tissue loss due to ischemia or infection. Fortunately, the battlefield head and neck surgeon is armed with an array of surgical advances including local, regional, and free tissue grafts as well as advances in wound care and wound care produc

[Soft tissue defects following tumor resection in the limbs and trunk : Plastic reconstructive soft tissue and revision concepts].

BACKGROUND: Tissue defects of the trunk and limbs after oncologic surgery and radiation require plastic reconstructive tissue coverage. Depending on the location and size of the wound as well as the interdisciplinary treatment concept, different reconstructive procedures are performed. These range from skin transplantation to local and pedicle flaps, to perforator flaps and free microsurgical tissue transfer. METHODS: The modern "reconstructive ladder" can be regarded as an orientation for the sequence of the reconstructive options. Considering the patient's wishes and risk profile, an indivi

Breast augmentation with round, smooth saline or gel implants: the pros and cons.

Breast augmentation is a procedure that can be performed successfully in many different ways. Large series are reported routinely, with greater than 95% patient satisfaction. It is clear that this is a procedure that has greatly benefited women and thereby society in general. It is inappropriate to be too dogmatic about each individual's technique, because many different techniques have achieved a high degree of success in terms of patient satisfaction and final results. That is not to say, however, that surgeons still are not plagued by the two most common downsides to this operation: capsula

Evidence-Based Perioperative Nutrition Recommendations: Optimizing Results and Minimizing Risks.

Plastic surgery patients span the nutritional spectrum from generally healthy, nutritionally competent patients to inherently catabolic, nutritionally deficient, and chronic wound patients. Therefore, plastic and reconstructive surgery affords the opportunity to investigate the impact of nutrition across a heterogeneous patient population following a wide variety of procedures. Although patients may be nutritionally deficient in certain vitamins warranting perioperative repletion, other supplements have the potential to benefit all patients, regardless of nutritional status. Despite these puta

Different aspects of informed consent in aesthetic surgeries.

Providing an informed consent has an important role in promotion of medical treatments and reduction of judiciary litigations in this process. Today with cultural changes and wide propagation that is usually charming, the request for aesthetic surgery has an increasing trend. These problems with complexity of cosmetic surgeries lead to deeper differences of information between plastic surgeons and patients, so the discussion on giving information to a patient is of great importance. Regarding the elective choice of aesthetic surgeries, there is a need on providing a standard informed consent f

The Eye Cannot See What the Brain Does Not Know: A Report of Two Cases of Morton's Neuroma.

It is rare for patients to present to the plastic surgery department with foot pain. If this occurs, there is a high probability that these are patients who would have visited numerous hospitals without finding a solution to their complaints and have not recovered despite receiving various treatments. In such cases, Morton's neuroma is a diagnosis that remains unseen by the eyes unless it is kept in mind. Following its correct diagnosis, surgical treatment is definitely within the scope of plastic surgery. In this report, we present two male patients, aged 35 and 40 years, treated in our clini

Von Willebrand disease: screening, diagnosis, and management.

Von Willebrand disease (vWD), a hemorrhagic disorder mimicking a defect in platelet function, is the most commonly inherited coagulopathy, resulting in a deficiency that may prolong bleeding time and increase risk for major bleeding complications during surgery. Von Willebrand factor (vWF) serves a dual role in hemostasis: mediating the initial platelet adhesion to damaged endothelium at the site of vessel injury and stabilizing coagulation factor VIII, an important cofactor in the generation of a fibrin clot. Although quantitative or qualitative defects in vWF protein can manifest as a mild t

Perceptions and Understanding of Transgender Patient Care: A Survey of Practicing Facial Plastic Surgeons.

Transgender individuals face significant health disparities including deficiencies in physician education, knowledge, and comfort with care. The objective of this study was to determine the perceptions, practice patterns, and familiarity of facial plastic surgeons with transgender health care. An anonymous questionnaire was sent to facial plastic surgeons within the American Academy of Facial Plastic and Reconstructive Surgery list-serve. Primary datapoints included participant characteristics, transgender-related experience, and educational goals. Of the 66 facial plastic surgeons surveyed, 4

Comparison on functional and esthetic outcomes between single and combined surgery to crooked nose deformities.

BACKGROUND: Nose deviation deformities pose a complex cosmetic and functional problem. The most common surgical intervention to manage such deformities is septorhinoplasty, typically performed by a plastic surgeon or a rhinologist. This study investigated the effect of a combined operation by a plastic surgeon and rhinologist, comparing them with those operations performed singlehandedly by a plastic surgeon. METHODS: From January 2017 to January 2022, 99 patients with deviated noses were treated. Fifty-nine underwent the combined operation, whereas 40 underwent surgery performed by a plastic

Liraglutide ameliorates delirium-like behaviors of aged mice undergoing cardiac surgery by mitigating microglia activation via promoting mitophagy.

OBJECTIVE: Postoperative delirium (POD) is a prevalent complication in cardiac surgery patients, particularly the elderly, with neuroinflammation posited as a crucial contributing factor. We investigated the prophylactic effects of liraglutide, a GLP-1 analog, on delirium-like behaviors in aged mice undergoing cardiac surgery and explored the underlying mechanisms focusing on neuroinflammation, mitochondrial dysfunction, and synaptic plasticity. METHODS: Using a cardiac ischemia-reperfusion animal model to mimic cardiac surgery, we assessed delirium-like behaviors, microglial activation, NLRP

Necrotizing fasciitis: act fast and act together.

Necrotizing fasciitis (NF) is a rare and potentially fatal infection of the soft tissues characterized by widespread fascial necrosis and rapid evolution. Diagnosis is often difficult and relies on a high index of suspicion. The approach to this disease requires a timely diagnosis, early antibiotic therapy, aggressive surgical debridement, and reconstructive procedures. All patients treated by the Plastic Surgery department with the diagnosis of NF between 2006-2019 were identified. A retrospective study was conducted with the treatment of patients' demographic and clinical data. Sixty patient

Ortho-plastic approaches to fracture related infection (FRI).

When a plastic surgery opinion is sought to support orthopaedic colleagues, invaluable contributions to decision-making in fracture-related infection (FRI) can be made. We summarise the plastic surgeon's perspective on the most pertinent questions which are relevant when assessing infections as a sequelae of fractures: 1.) How to define the soft tissues involved. 2.) Local & free tissue cover: options & outcomes. 3.) What to do when flaps fail. 4.) When simple dressings are an option.

[Surgical policy in chronic purulent otitis media in children].

Surgical outcomes in 603 children with otitis media purulenta chronica (OMPC) have shown that early microsurgery (endaural tympano-, attico-, aditotomy) improved or reestablished normal anatomy and physiology of the middle ear in 85% of the patients. One-stage cleansing or plastic surgery is preferable. In operations on mastoid process in the absence of complications, prevention of reoperation seems most effective by autotissue plastic reconstruction of the mastoid bone wound in the end of the operation.

Current Management of Zygomaticomaxillary Complex Fractures: A Multidisciplinary Survey and Literature Review.

Despite the prevalence of zygomaticomaxillary complex (ZMC) fractures, there is no consensus regarding the best approach to management. The aim of this study is to determine differences in ZMC fracture treatment among various surgical specialties. A survey was conducted regarding treatment of patients with different ZMC fractures that included a minimally displaced fracture (Case 1), a displaced fracture without diplopia (Case 2), a displaced fracture with diplopia (Case 3), and a complex comminuted fracture (Case 4). The survey was distributed to members of plastic surgery, oral maxillofacial

A Novel Approach to Surgical Markings Based on a Topographic Map and a Projected Three-Dimensional Image.

UNLABELLED: Surgical markings play a crucial role in the planning of plastic surgery procedures. However, despite their importance, they are often imprecise. For instance, when assessing patients in need of autologous fat grafting, surgeons often base markings on estimations of where volume deficiency exists and how much volume will correct the deficiency. In this article, the authors describe a novel approach to lipostructure, guided by a computer-based roadmap. A digital three-dimensional topographic surgical map is created using three-dimensional photography and analytic software and then p

A Comparison of Pumping Infusion-Induced Contamination With Different Syringe Types.

Health care providers use several types of syringes in their daily routine to provide or safely deliver therapeutic agents to patients. Pump infusion with a syringe (PIS) are used for the rapid delivery of large amounts of fluid or blood to patients in critical conditions, such as hypovolemic shock. Patients often experience bacterial infections, such as catheter-related bloodstream infections, specifically when they are in critical condition in the intensive care unit (ICU) after surgery or undergoing resuscitation in an emergency department. A previous study has shown that PIS may cause intr
