Effect of reducing anatomic dead space on arterial PCO2 during CO2 inhalation.

Carotid body-denervated (CBD) ponies have a less than normal increase in arterial PCO2 (PaCO2) when inspired CO2 (PICO2) is increased, even when pulmonary ventilation (VE) and breathing frequency (f) are normal. We studied six tracheostomized ponies to determine whether this change 1) might be due to increased alveolar ventilation (VA) secondary to a reduction in upper airway dead space (VD) or 2) is dependent on an upper airway sensory mechanism. Three normal and three chronic CBD ponies were studied while they were breathing room air and at 14, 28, and 42 Torr PICO2. While the ponies were br

Management of giant cystic hygromas in infants.

Giant cystic lymphangioma is a rare lesion that presents major therapeutic challenges. Four infants presented in the first weeks of life with diffuse cysts in the neck. The tongue was involved in three, the mediastinum in one, and the pharynx and supraglottic larynx in two. These lesions are characterized by hundreds of cysts that infiltrate in and around muscles, nerves, and vessels. Complete excision is not possible without damaging the normal structures. All four infants initially had partial removal with unroofing and drainage of the remaining cysts and temporary tracheostomy to protect th

[Indwelling transtracheal sound for oxygen administration in the patient with hypoxic chronic respiratory insufficiency].

A new technique for administering oxygen to patients with severe chronic respiratory failure is reported. It consists of introducing a catheter, 2 mm in diameter, into the trachea between the second and third tracheal rings under local anesthesia. The technique was used in a 70-year old patient with severe chronic obstructive lung disease and resulted in significant reduction of dyspnoea, improvement in general condition with a weight gain of 6 kg in 6 months, and a 15 mmHg increase in arterial partial oxygen pressure for the same oxygen flow rate. This technique appears to be indicated for pa

Pull-through esophagectomy without thoracotomy.

Twenty patients with carcinoma of the hypopharynx, esophagus and thyroid underwent pull-through esophagectomy. Seventeen of them received combined resection of the larynx and trachea. Dissection of the lymph nodes at the upper mediastinum was performed in 11 patients by sternotomy. Seven patients received mediastinal tracheostomy after combined resection of the trachea and the larynx. Pull-through esophagectomy was followed by pharyngogastrostomy without thoracotomy via the posterior mediastinum. This technique is described in detail. Because there is no thoracotomy and ligation of the esophag

Periodic breathing and the pathogenesis of occlusive sleep apneas.

In order to study the relationship between sleep-induced periodic breathing and the development of occlusive sleep apneas, 6 patients with hypersomnia-sleep apnea syndrome were studied during nocturnal sleep before and approximately 1 month after therapeutic tracheostomy. Post-tracheostomy studies were performed in open and closed tracheostomy states. Sleep-induced periodic breathing resembling the pattern of Cheyne-Stokes breathing was observed in all patients before and after tracheostomy, even when tracheostomy was left open. When tracheostomy was closed all patients developed periodic hypo

Hypersomnia-sleep apnea due to micrognathia. Reversal by tracheoplasty.

A 67-year-old woman with acquired micrognathia developed severe daytime hypersomnia, loud snoring, nocturnal enuresis, encopresis, and hypertension. A polysomnogram demonstrated 564 sleep apneas, primarily obstructive, recurrent hypoxia, a bradytachycardia, and absent stages III, IV, and REM sleep. Endoscopy during sleep revealed recurrent active closure of the upper pharynx associated with loud snoring. A tracheoplasty was done because of severity of symptoms and failure of conservative therapy. Dramatic improvement in sleepiness and hypertension occurred within 48 hours. On postoperative nig

Acute epiglottitis: intubation versus tracheostomy.

Acute epiglottitis is a disease with significant mortality. The patient, usually an otherwise healthy pre-school child, develops a sore throat and muffled voice from swollen supraglottic structures, and may progress rapidly to respiratory arrest. Early diagnosis and airway maintenance can prevent these fatalities. Whether to secure an airway by tracheostomy or endotracheal intubation is the subject of much discussion. Nineteen series totalling 738 cases of epiglottitis plus 11 new cases are reviewed. These patients were treated as follows: Tracheostomy = 348 (3 deaths - 0.86%); Endotracheal in

The treatment of flail chest injury.

Flail chest injuries are traditionally managed by long-term artificial ventilation, which in itself is hazardous and expensive. A more conservative therapeutic regime is described, and the results of this regime are compared with those following the previous traditional regime used in the Respiratory Unit, Royal Brisbane Hospital. The place of artificial ventilation in relation to spontaneous ventilation is redefined in the context of this new regime.
