BACKGROUND: Heyde syndrome is an uncommon clinical condition characterized by the triad of aortic stenosis, gastrointestinal angiodysplasia, and acquired von Willebrand syndrome. Its occurrence in the context of hypertrophic obstructive cardiomyopathy is rare. Common in individuals over 65, diagnosis is challenging due to the prevalence of these conditions. Hypertrophic obstructive cardiomyopathy increases shear stress, leading to von Willebrand Factor degradation and a higher risk of gastrointestinal bleeding.

UNLABELLED: SummaryThirty-seven-year-old male presented with cough, dyspnea, significant weight loss (20 kg) and subacute fever for the past 2 months. Physical examination revealed inspiratory and expiratory wheezing bilaterally. A normal S1, S2 and a 3/6 systolic ejection murmur at the left upper parasternal border with respiratory variation were found during cardiac auscultation. Kidney and bone marrow biopsy reported a high-grade B-cell lymphoma. Echocardiography and cardiac CT findings consisted of multiple intracardiac masses affecting the right ventricular (RV) outflow track, RV apex, me

Between 1993 and 2000, 50 patients (age range, 1 month to 18 years) who had left ventricular outflow tract (LVOT) disease and had undergone a Ross procedure were assessed using echocardiography. Aortic annulus size, valvular gradient, valve insufficiency, LV dimensions at end-systole and end-diastole, LV interventricular septal and posterior wall thickness, and LV mass index (LVMI) were measured. There was 1 early and 2 late deaths, and 5 reoperations. Aortic annulus size increased and degree of aortic insufficiency (AI) decreased, as did LV internal dimensions and LVMI. Peak pressure gradient

Hypertrophic obstructive cardiomyopathy (HOCM) is a heterogenous disease causing obstruction of the left and/or right ventricular outflow tract (LVOT and/or RVOT, respectively). The main symptoms include dyspnea on exertion and syncope, and in severe cases, subsequent development of heart failure symptoms with end stage heart failure occurs. In a certain number of patients, surgical intervention is required since pharmacological and/or interventional treatment using transcoronary ablation of septal hypertrophy (TASH) does not relieve symptoms. The primary goal of any surgical technique is to r

The possible influence of small pulmonary artery size on the results following complete repair of tetralogy of Fallot has not been investigated in patients less than 1 year of age. We, therefore, reviewed our recent experience with infant repair to assess this. Between January 87 and October 90, repair was performed in 58 infants less than 1 year old (median, 6 months) with tetralogy of Fallot and pulmonary stenosis (n = 53) or pulmonary atresia without important aortopulmonary collaterals (n = 5). The McGoon ratio (diameter of the right pulmonary artery at the prebranching point plus that of

BACKGROUND: Resolution of left ventricular outflow tract (LVOT) obstruction predicts symptom relief postmyectomy. Intraoperative measurement of LVOT gradients thus is essential for surgical guidance. We hypothesized that (1) hypertrophic cardiomyopathy patients have lower LVOT gradients when measured intraoperatively with transesophageal echocardiography (TEE) compared with preoperative measurements with transthoracic echocardiography (TTE) and that (2) intraoperative provocative testing can help evaluate the adequacy of surgical resection.
METHODS: We compared resting LVOT gradients on preop

OBJECTIVE: Supra-annular mitral ring (SMR) is a rare developmental abnormality of the supra-valvular area of the mitral valve, which produces a variable degree of obstruction to left ventricular (LV) filling. Management of SMR remains an important therapeutic challenge. SMR constitutes a small but inadequately described subset that has a relatively good outcome with appropriate management.
METHODS: Between 1983 and 2009, 27 patients with SMR underwent surgical treatment directed to the stenosing SMR. The demographic and clinical features, diagnostic modalities and the surgical management were

Acute coronary occlusion during transcatheter aortic valve implantation (TAVI) is an unwarranted complication associated with high mortality. The current TAVI practices recommend a multidetector computed tomography (MDCT) evaluation of the aortic valve, the left ventricular outflow tract, and the aortic root to determine the conventional risk factors for coronary obstruction like low-lying coronary ostia and narrow sinuses of Valsalva, mandating prophylactic coronary protection or native valve leaflet modification in high-risk patients. Despite optimal anatomy, acute coronary occlusion can sti

Transcatheter closure of patent ductus arteriosus (PDA) by Amplatzer duct occluder is the treatment of choice. However, closure of very large ducts in infants with low weight is a challenge for the interventionalist because a large device may obstruct the aorta or left pulmonary artery. Difficulty is also encountered in advancing the device around the curve of the right ventricular outflow tract toward the pulmonary artery; this curve is tight, more or less at a right angle in infants, leading to kinking of the sheath, which increases fluoroscopic time. This is the first reported case of a ver

External outflow graft obstruction (eOGO) is an increasingly recognized long-term complication in patients with HeartMate 3 left ventricular assist devices. While percutaneous treatment provides a less invasive alternative to surgery, it introduces the challenge of stenosis migration-especially when graft twist is a contributing factor. This migration can extend to the edge or even beyond the Bend Relief toward the aortic or pump anastomosis, posing procedural risks in fragile anatomical zones. In this case series, we present three patients with eOGO who experienced migration of the stenotic l

One of the major issues encountered in patients undergoing evaluation for Transcatheter mitral valve replacement (TMVR) is the risk of Left ventricular outflow tract (LVOT) obstruction. LVOT obstruction is a catastrophic complication of TMVR, the result of displacement of the anterior mitral valve leaflet (AML) toward the interventricular septum. Several strategies to mitigate the risk of LVOT obstruction have been described and include percutaneous laceration of the anterior mitral leaflet (LAMPOON), alcohol septal ablation, trans-atrial leaflet modification (SITRAL) and Balloon Assisted Tran

CONTEXT: Several studies have investigated the association between heavy metal exposure and congenital heart defects (CHDs). However, there are limited data regarding the relationship between barium exposure and the occurrence of CHDs. The objective of this study was to analyze the association between barium exposure in mothers and the risk of CHD in offspring.
MATERIALS AND METHODS: We developed a case-control study with 399 cases and 490 controls with normal live births in China. The concentrations of barium in hair of pregnant woman and fetal placenta were measured. We used a logistic regr

INTRODUCTION: Primary cardiac tumors are rare in children. The majority are benign and noninvasive, although they may have significant hemodynamic consequences depending on their location. The most frequent type in children is rhabdomyoma, usually multiple and intraventricular.
OBJECTIVES: To assess the presentation and outcome of patients with cardiac tumors followed by the Department of Pediatric Cardiology of Hospital de Santa Cruz.
METHODS: A retrospective analysis was performed of the clinical records of patients diagnosed with primary cardiac tumors between January 1992 and March 2008.

OBJECTIVES: The aim of this study was to compare outcomes of alcohol septal ablation (ASA) in young and elderly patients with obstructive hypertrophic cardiomyopathy (HCM).
BACKGROUND: The American College of Cardiology Foundation/American Heart Association guidelines reserve ASA for elderly patients and patients with serious comorbidities. Information on long-term age-specific outcomes after ASA is scarce.
METHODS: This cohort study included 217 HCM patients (age 54 ± 12 years) who underwent ASA because of symptomatic left ventricular outflow tract obstruction. Patients were divided into yo

OBJECTIVES: The purpose of this study was to describe the clinical and functional results of combined anterior mitral leaflet extension and myectomy in patients with hypertrophic obstructive cardiomyopathy.
BACKGROUND: Septal myectomy is the most commonly performed surgical procedure in patients with hypertrophic cardiomyopathy and left ventricular outflow tract obstruction. Because of the role of the mitral valve in creating the outflow tract gradient, mitral valve replacement or plication is performed in selected cases in combination with myectomy, often with better hemodynamic results than

BACKGROUND: Right ventricular outflow tract obstruction (RVOTO) is a common problem after repair of congenital heart disease. Percutaneous pulmonary valve implantation (PPVI) can treat this condition without consequent pulmonary regurgitation or cardiopulmonary bypass. Our aim was to investigate the clinical and physiological response to relieving RVOTO.
METHODS AND RESULTS: We studied 18 patients who underwent PPVI for RVOTO (72% male, median age 20 years) from a total of 93 who had this procedure for various indications. All had a right ventricular outflow tract (RVOT) gradient >50 mm Hg on

Corrected transposition of the great arteries (C-TGA) in situs inversus [I,D,D] is a rare variant of C-TGA (5-8% of the cases). Few anatomic and surgical data on this anomaly have been published. Between 1974 and 1990, 13 such patients (mean age, 12.7 +/- 8.6 yr) underwent repair of associated cardiac anomalies, including ventricular septal defect (VSD) and pulmonary outflow tract obstruction (POTO) in all 13 patients and atrial septal defect (ASD) in 11. Systemic venous anomalies were present in four. Nine patients also had dextrocardia. Patches to close the VSD were placed on the right side

BACKGROUND: Left ventricular (LV) unloading during veno-arterial extracorporeal membrane oxygenation (VA-ECMO) is essential for managing refractory low cardiac output after complex cardiac surgery. However, direct LV venting is associated with an increased risk of intracardiac thrombosis, particularly when anticoagulation is delayed.
CASE PRESENTATION: A 69-year-old woman with hypertrophic cardiomyopathy and severe left ventricular outflow tract obstruction presented with exertional dyspnea. Echocardiography showed severe mitral regurgitation with marked annular calcification, moderate aortic
