Brain injury guidelines for the management of traumatic brain injury: a systematic review and meta-analysis.

OBJECTIVES: The Brain Injury Guidelines (BIG) serve to predict the severity of traumatic brain injuries resulting in radiologic findings. The use of BIG promises significant time and resource savings, over routine neurosurgical consultation and repeat imaging. After more than a decade of validation, studies have demonstrated BIG to reliably predict adverse outcomes. We sought to analyze the existing literature to determine the pooled efficacy of BIG and its variations. METHODS: A systematic review and meta-analysis were conducted in accordance with PRISMA guidelines (PROSPERO ID: CRD420234811…

Brain-to-Brain Coupling Is Associated With the Benefits of Delayed Imitation in Skill Acquisition: An fNIRS Study.

PURPOSE: Observational learning through imitation is an important ability across various domains. Although research has found that delayed imitation produces superior learning outcomes compared with other observational learning strategies, the underlying reason for this advantage remains unclear. This study investigated whether interpersonal brain synchronization (IBS) between learners and demonstrators is related to the benefits of delayed imitation. METHODS: A total of 48 pairs of participants (Mage = 19.44, SDage = 0.71) engaged in an abacus learning task under three learning conditions: o…

Glymphatic Dysfunction and Aquaporin-4 Dysregulation in Traumatic Brain Injury and Brain Tumors: A Review.

The glymphatic system is a cerebrospinal fluid-interstitial fluid exchange pathway that clears metabolic waste and maintains brain fluid homeostasis. Aquaporin-4 (AQP4), a water channel at astrocytic endfeet along the neurovascular interface, supports perivascular water transport and glymphatic flow. Disruption of this glymphatic-AQP4 unit is implicated in conditions with altered fluid dynamics, including traumatic brain injury (TBI) and brain tumors. We reviewed experimental and clinical studies examining glymphatic pathways and AQP4 regulation in TBI and brain tumors, and synthesized evidenc…

Accelerated Brain Aging in Multiple Sclerosis: Microstructural and Metabolic Correlates of the Brain Age Gap.

BACKGROUND/OBJECTIVES: Multiple sclerosis (MS) can cause neurodegeneration leading to accelerated brain atrophy. Brain-predicted age (BA) is an emerging neuroimaging biomarker for neurodegeneration but remains underexplored in MS. This study examines the pathophysiological substrates associated with the brain age gap in MS compared with healthy controls (HCs) through a combination of volumetric, spectroscopic, and diffusion imaging. METHODS: This retrospective cross-sectional study included 33 HCs and 124 MS patients. Participants underwent 3T MRI including 3D-T1, MR spectroscopy, magnetizati…

Gut-Brain Hydraulics: Brain motion and CSF circulation is driven by mechanical coupling with the abdomen.

The brain moves within the skull, but the drivers and function of this motion are not understood. We visualized brain motion relative to the skull in awake head-fixed mice using high-speed, multi-plane two-photon microscopy. Brain motion was primarily rostrally and laterally directed, and was tightly correlated with locomotion, but not with respiration or the cardiac cycle. Electromyography recordings in abdominal muscles and microCT reconstructions of the trunk and spinal vasculature showed that brain motion was driven by abdominal muscle contractions that activate a hydraulic-like vascular c…

The development of a brain injury survivor patient and public involvement group by a brain injury survivor.

BACKGROUND: Patient and public involvement (PPI) in research is seen as key to ensuring applicability and impact. Undertaking PPI in people after brain injury has long been seen to be a challenge. In 2020 The NIHR Brain Injury MedTech Cooperative developed a programme with the aim of improving PPI involvement, impact and diversity in this population. METHODS: Through a process of iterative development, a PPI programme was created. It built on an existing underutilised database of people after brain injury and their carers who were interested in engaging with PPI and utilised video-calling sof…

Unveiling the rumen-microbiome-brain circuit: a unique dimension of gut-brain axis in ruminants.

Gut-brain communication via the peripheral neural network is vital for regulating local digestive function and systemic physiology. Gut microbiota, which produces a wide array of neuroactive compounds, is a critical modulator in this bidirectional dialog. Perturbations in the gut microbiota have been implicated in neurological disorders such as depression and stress. Distinct from humans and other monogastric animals, ruminants possess a unique, microbially dense gastrointestinal compartment, the rumen, that facilitates the digestion of fibrous plant materials. These ruminal microbes are likel…

Research progress on the kidney-gut-brain axis in brain dysfunction in maintenance hemodialysis patients.

Maintenance hemodialysis (MHD) has become the primary renal replacement therapy for patients with end-stage renal disease. The kidney-gut-brain axis represents a communication network connecting the kidney, intestine and brain. In MHD patients, factors such as uremic toxins, hemodynamic changes, vascular damage, inflammation, oxidative stress, and intestinal dysbiosis in MHD patients refers to a range of clinical syndromes, including brain injury, and is manifested by conditions such as white matter disease, brain atrophy, cerebrovascular disease, cognitive impairment, depression, anxiety, and…

Tenofovir Activation Is Diminished in the Brain and Liver of Creatine Kinase Brain-Type Knockout Mice.

Tenofovir (TFV) is a nucleotide reverse transcriptase inhibitor prescribed for the treatment and prevention of human immunodeficiency virus infection and the treatment of chronic hepatitis B virus infection. Here, we demonstrate that creatine kinase brain-type (CKB) can form tenofovir-diphosphate (TFV-DP), the pharmacologically active metabolite, in vitro and identify nine missense mutations (C74S, R96P, S128R, R132H, R172P, R236Q, C283S, R292Q, and H296R) that diminish this activity. Additional characterization of these mutations reveals that five (R96P, R132H, R236Q, C283S, and R292Q) have A…

Preoperative Blood-Brain Barrier Integrity Influence on the Impact of Anesthesia and Surgery on Mice Brain.

BACKGROUND: Brain homeostasis imbalance, characterized by cognitive dysfunction and delirium, frequently occurs in the elderly after surgery. Investigating why this complication only affects part of patients undergoing the same surgery, and anesthesia remains intriguing. This study tested the role of preoperative blood-brain barrier (BBB) integrity in the occurrence of postoperative brain homeostasis imbalance using mice with conditional BBB damage. METHODS: Preoperative BBB breakdown was induced in End-SCL-Cre-ctnnb1 fl//fl (iCKO) mice by administering tamoxifen (intraperitoneal [i.p.]). Thi…

Effect of non-invasive brain stimulation in children with acquired brain injury-a scoping review.

BACKGROUND: Children and young people (CYP) with acquired brain injury (ABI) require early and effective neurorehabilitation to improve long-term functional outcomes. Non-invasive brain stimulation (NIBS), including transcranial magnetic stimulation (TMS) and transcranial direct current stimulation (tDCS), have been used to improve motor and sensory skills for children with cerebral palsy. However, there is limited evidence supporting its use in CYP with ABI. OBJECTIVE: To systematically review the TMS and tDCS intervention effects on motor, sensory and other functional issues in CYP with ABI…

Modeling of the brain-lung axis using organoids in traumatic brain injury: an updated review.

Clinical outcome after traumatic brain injury (TBI) is closely associated conditions of other organs, especially lungs as well as degree of brain injury. Even if there is no direct lung damage, severe brain injury can enhance sympathetic tones on blood vessels and vascular resistance, resulting in neurogenic pulmonary edema. Conversely, lung damage can worsen brain damage by dysregulating immunity. These findings suggest the importance of brain-lung axis interactions in TBI. However, little research has been conducted on the topic. An advanced disease model using stem cell technology may be an…

Eye-brain connections revealed by multimodal retinal and brain imaging genetics in the UK Biobank.

As an anatomical extension of the brain, the retina of the eye is synaptically connected to the visual cortex, establishing physiological connections between the eye and the brain. Despite the unique opportunity retinal structures offer for assessing brain disorders, less is known about their relationship to brain structure and function. Here we present a systematic cross-organ genetic architecture analysis of eye-brain connections using retina and brain imaging endophenotypes. Novel phenotypic and genetic links were identified between retinal imaging biomarkers and brain structure and functio…

Rapid effects of valproic acid on the fetal brain transcriptome: Implications for brain development and autism.

There is an increased incidence of autism among the children of women who take the anti-epileptic, mood stabilizing drug, valproic acid (VPA) during pregnancy; moreover, exposure to VPA in utero causes autistic-like symptoms in rodents and non-human primates. Analysis of RNAseq data ob-tained from E12.5 fetal mouse brains 3 hours after VPA administration revealed that VPA significant-ly increased or decreased the expression of approximately 7,300 genes. No significant sex differ-ences in VPA-induced gene expression were observed. Expression of genes associated with neu-rodevelopmental disorder…
