Lipophilic amino alcohols with calcium channel blocking activity.

A series of novel lipophilic amino alcohols, analogs of the anticholinergic drug vesamicol, were evaluated for Ca2+ channel blocking activity. The effects of these drugs on depolarization-induced intracellular free Ca2+ concentration ([Ca2+]i) transients were examined in single NG108-15 cells and dorsal root ganglion (DRG) neurons in culture. [Ca2+] was recorded with the Ca2+ indicator Indo-1 and a dual emission microfluorimeter. Structure-activity studies indicated that features required for Ca2+ channel blocking activity were distinct from those required for anticholinergic activity. In part

Plasma albumin and calcium levels following cardiopulmonary bypass.

A significant fall in the levels of plasma albumin and a corresponding fall i in the levels of extracellular calcium occurred following cardiopulmonary bypass with the crystalloid prime. Postoperative hypoalbuminaemia was well tolerated, except in the patients with poor left ventricular function. Postoperative hypocalcaemia may increase digoxin sensitivity and may augment the negative inotropic effect of verapamil.

Sarcoplasmic ionic calcium concentration in neuroleptic malignant syndrome.

The neuroleptic malignant syndrome (NMS) is an uncommon but serious adverse effect of antipsychotic medication. Similarities in the clinical picture, and muscle alterations, between NMS and susceptibility to malignant hyperthermia (MH) suggest common mechanisms underlying both disorders. Sarcoplasmic ionic calcium concentration ([Ca2+]i) was measured by means of Ca2+ selective microelectrodes in intact intercostal muscle fibers isolated from NMS patients and from subjects with no evidence of neuromuscular disease, who served as controls. The mean resting membrane potential and [Ca2+]i were -84…

[Subcutaneous heparin-calcium therapy: clinical and laboratory experience].

Fifty patients with acute deep vein thrombosis, pulmonary embolism, acute myocardial infarct and priapism were treated with subcutaneous calcium heparin. Laboratory control was based on the examination of APTT. On the first day of heparin therapy only 52% of patients's APTT was 1.5 times greater than control value but the ratio increased to 82% on the fifth day of treatment. The importance of initial venous sodium heparin bolus (5-10,000 U) followed immediately by subcutaneous calcium heparin is emphasized. Injection of 500 U/kg/day calcium heparin divided in two equal doses is recommended. Fo…

[Preventive medical treatment of recurrent urinary calcium calculi].

In 59 out of 80 patients with recurrent renal calcium stones studied between 1977 and 1982 and followed up for 4,5 to 7 years, an index has been used to determine the activity of the disease, before and after treatment with diet, high water intake, thiazide and/or allopurinol. As estimated by variations of the "activity index", the treatment was effective in all groups studied: patients with idiopathic hypercalciuria were prescribed a low purine and oxalate diet and a large water intake associated or not with thiazide; hyperuricosuric patients were treated by the same diet and allopurinol; pat…

Calcium channel blocking drugs. Part II: Clinical applications.

CCB are a diverse group of drugs that bind to specific glycoprotein receptors associated with the cell membrane that are most likely identical to the voltage-sensitive calcium channel. By inhibiting the influx of calcium into myocardial, pacemaker, and conducting tissues and vascular smooth muscle, these agents modify excitation-contraction coupling in muscle and electrical impulse transmission in the heart. The vasodilating and electrophysiologic actions of these drugs have been harnessed for the treatment of coronary vasospasm, angina pectoris, and supraventricular arrhythmias. They also hav…

Successful sodium thiosulphate treatment for recurrent calcium urolithiasis.

Thirty-four idiopathic rapid calcium stone formers (24 male/10 female) were studied. Their ages ranged from 20 to 60 years (40 +/- 11) and all had good renal function. The trial comprised two consecutive periods of 3 years and 4 years duration respectively. In the first (control period), the patients were maintained on a customary diet with an adequate fluid intake sufficient to produce about 2 liters of urine daily. In the second (treatment period), they received a similar diet plus 20 mmol of sodium thiosulphate daily. New stone development fell from 100 in the control period to 15 in the tr…

Sodium lactate infusion, panic attacks, and ionized calcium.

The ability of sodium lactate to precipitate panic attacks in vulnerable individuals has been repeatedly demonstrated, although its mechanism of action is unknown. Pitts and McClure hypothesized that the panicogenic effect of sodium lactate was due to its induction of a peripheral hypocalcemia via the complexing of infused lactate ions with extracellular ionized calcium. In the current study, serial ionized calcium measurements during lactate infusion are assessed for 22 panic patients who panicked during infusion, 11 panic patients who did not panic during the procedure, and 6 normal controls…

[Anomalies of phospho-calcium metabolism during antiepileptic treatment].

It is known that, particularly in Nordic States, many patients undergoing antiepileptic therapy with Phenobarbital and Phentoin present disorders of the phospho-calcium metabolism, both clinical and biochemical. For what concerns the mediterranean area, few are the papers written concerning this subject in these last years. In this paper we attempt to provide a "case study" contribution (100 cases) to enable us to study the above-mentioned alteration focusing on an area half way between Central and Southern Europe. The Authors discuss the possible mechanism implicated in this affection.

Milk of calcium within tiny benign breast cysts.

Small amounts of milk of calcium settling to the bottom of multiple tiny benign breast cysts produce the mammographic picture of clustered linear and curvilinear calcifications when imaged in lateral projection with a horizontal x-ray beam. These calcifications, although benign, may be confused with the microcalcifications of carcinoma. Milk of calcium is found much more frequently in small breast cysts than anywhere else in the body. It is not at all uncommon, occurring in more than 4% of women undergoing diagnostic mammography. Fortunately, the overall mammographic appearance of milk of calc…

Calcium pyrophosphate dihydrate crystal formation in model hydrogels.

Using powder x-ray diffraction analysis, we studied calcium pyrophosphate crystal formation in silica and gelatin gels was well as in comparable aqueous solutions. We demonstrated that the physiological salts, monoclinic and triclinic calcium pyrophosphate dihydrate can be formed in gels and that gels do affect the type of crystals formed from ions in solution. These observations support our hypothesis that calcium pyrophosphate dihydrate crystal deposition in joints is regulated by the physical chemical gel state of the connective tissue matrix.

Intracellular calcium buffering capacity in isolated squid axons.

Changes in ionized calcium were studied in axons isolated from living squid by measuring absorbance of the Ca binding dye Arsenazo III using multiwavelength differential absorption spectroscopy. Absorption changes measured in situ were calibrated in vitro with media of ionic composition similar to axoplasm containing CaEGTA buffers. Calcium loads of 50-2,500 mumol/kg axoplasm were induced by microinjection, by stimulation in 112 mM Ca seawater, or by soaking in choline saline with 1-10 mM Ca. Over this range of calcium loading of intact axoplasm, the ionized calcium in the axoplasm rose about …

Requirement for digestible calcium by eleven- to twenty-five-kilogram pigs as determined by growth performance, bone ash concentration, calcium and phosphorus balances, and expression of genes involved in transport of calcium in intestinal and kidney cells.

Two experiments were conducted to determine the requirement for standardized total tract digestible (STTD) Ca by 11- to 25-kg pigs based on growth performance, bone ash, or Ca and P retention and to determine the effect of dietary Ca on expression of genes related to Ca transport in the jejunum and kidneys. Six diets were formulated to contain 0.36% STTD P and 0.32, 0.40, 0.48, 0.56, 0.64, or 0.72% STTD Ca by including increasing quantities of calcium carbonate in the diets at the expense of cornstarch. Two additional diets contained 0.72% STTD Ca and 0.33% or 0.40% STTD P to determine if 0.36…

Mitochondrial Calcium Signaling in Hepatocyte Health and Disease.

Calcium (Ca2+) is vital in hepatocyte metabolism and plays a dual role in liver mitochondrial function: Physiological Ca2+ stimulates respiration and mitochondrial dynamics-processes crucial for proper metabolic functioning. However, Ca2+ overload can be catastrophic, leading to mitochondrial dysfunction and the halt of metabolic processes. This dichotomy plays out in liver diseases such as metabolic dysfunction-associated steatohepatitis (MASH) and alcoholic liver disease (ALD), where excess lipid and alcohol, respectively, result in pathological changes in this precarious Ca2+ balance, impai…
