We report 2 cases of indocyanine green (ICG) staining used for posterior continuous curvilinear capsulorhexis (PCCC) in congenital cataract combined with anterior vitrectomy. In the first case, because of corneal opacity, the visibility of the posterior capsule was poor without staining. After the extraction of the cataract, a PCCC was performed after ICG staining of the posterior capsule. In the second case, after cataract removal, ICG staining was used to better visualize the posterior capsule. We present a technique, vitrectorhexis, in which a vitrector-cut capsulotomy is used to treat anterior capsule contraction syndrome. A vitrector handpiece is used to remove all fibrous capsule tissue and residual lens epithelial cells from the anterior chamber. Vitrectorhexis may be an alternative to neodymium:YAG laser capsulotomy as it decreases the risk of radial tear extension to the zonules and of secondary IOL decentration. A patient aged 26 was investigated for recurrent severe gastrointestinal bleeding. Over a 4-year period, he was transfused more than 400 units of blood. Multiple investigations included three negative Meckel's scans. A right hemicolectomy for supposed angiodysplasia failed to control his bleeding. The unusual occurrence of an otic capsule fracture with preservation of hearing is presented. In addition, the patient suffered facial paralysis beginning 6 days after the injury that rapidly recovered. Fifteen-month follow-up reveals stable hearing thresholds. The course of a fracture through the inner ear could be an important factor in determining the potential for hearing preservation.

PURPOSE: To present a case of rapidly progressing traumatic cataract caused by posterior capsule rupture after nonpenetrating ocular injury, as well as a surgical procedure to safely implant the intraocular lens in such a traumatized eye.

An 11-year-old boy had posterior lens capsule rupture resulting from a nonpenetrating (blunt trauma) injury to the eye. A rapidly developing cataract required pars plana lensectomy. This report suggests that posterior capsule rupture may occur secondary to blunt trauma and progressive cataract formation after posterior capsule rupture may require surgery soon after the injury.

OBJECTIVE: To determine the status of posterior lens capsule in traumatic cataract with B-ultrasonography.
METHODS: 44 eyes of traumatic cataract were studied. The integrity of posterior lens capsule was evaluated with B-ultrasonography before surgery. Cataract extraction was performed under microscope and the status of posterior lens capsule was recorded.
RESULTS: 21 eyes showed complete posterior lens capsule in B-ultrasonography. Of them, 19 eyes (90.5%) were confirmed by surgery. Other 23 eyes with posterior lens rupture determined before operation were all confirmed by surgery (100.0%).
