Alert burden and clinical relevance of remote monitoring in heart failure patients with cardiac implantable electronic devices: Single-center experience in the reimbursement era.

BACKGROUND: Heart failure (HF) affects 1%-2% of adults in developed countries and is associated with significant healthcare burden. Remote m … ctive single-center study of consecutive HF patients with implantable cardioverter defibrillator or cardiac resynchronization therapy-defibrillator was conducted to analyze unscheduled RM alerts, categorizing them as clinically significant or insignificant and assessing resulting medical interventions. RESULTS: A cohort of 407 HF patients (median age 71 years, 15% female) with im …

Shock Reduction Programming and Heart Function Recovery in Japanese Patients Undergoing Implantable Cardioverter Defibrillator Implantation for Primary Prevention - A Single-Center Prospective Study.

BACKGROUND: Although implantable cardioverter-defibrillators (ICD) offer prognostic benefit for patients with reduced left ventricular ejection fraction (LVEF), they remain underused in Japan. METHODS AND RESULTS: We analyzed 25 patients who underwent primary prevention ICD implantation. During a median follow-up of 52.6 months, no appropriate shocks occurred, but non-sustained ventricular tachycardia (NSVT) was frequently observed. Two patients underwent antitachycardia pacing. LVEF improved to >35% in nearly half of the patients. CONCLUSIONS: Cardiac resynchronization therapy and pharmacot…

A porcine large animal model of radiofrequency ablation-induced left bundle branch block.

BACKGROUND: Electrocardiographic (ECG) features of left bundle branch (LBB) block (LBBB) can be observed in up to 20%-30% of patients suffering from heart failure with reduced ejection fraction. However, predicting which LBBB patients will benefit from cardiac resynchronization therapy (CRT) or conduction system pacing remains challenging. This study aimed to establish a translational model of LBBB to enhance our understanding of its pathophysiology and improve therapeutic approaches. METHODS: Fourteen male pigs underwent radiofrequency catheter ablation of the proximal LBB under fluoroscopy …

Heart failure in Poland: Left ventricular assist device destination therapy and other challenges of interventional cardiology and cardiac surgery.

Patients with severe heart failure (HF), who are not eligible for cardiac transplantation and receive optimal medical management, based mainly on the use of pharmacological treatment and devices such as resynchronization therapy (implantable cardioverter-defibrillator), achieve poor clinical outcomes and constitute a group with extremely poor prognosis. Currently, the technology used in the latest generation left ventricular assist devices (LVADs), such as the HeartMate 3, makes it possible to achieve patient survival at the level obtained by patients after heart transplantation, and they can

Physiologic lead placement with electroanatomic mapping: A case series.

INTRODUCTION: His bundle pacing (HBP) and left bundle branch area pacing (LBBAP) have emerged as attractive alternatives to traditional biventricular pacing to achieve cardiac resynchronization therapy. Early reported results have been inconsistent, particularly amongst patients in whom initial placement with traditional approaches has been unsuccessful or those with complex anatomy or congenital abnormalities. In this report, we describe the use of three-dimensional electroanatomic mapping (EAM) in five selected cases. METHODS: Five patients from multiple clinical sites underwent EAM-guided …

Noninvasive measurement of atrial contribution to the cardiac output in children and adolescents with congenital complete atrioventricular block treated with dual-chamber pacemakers.

The contribution of atrial contraction to cardiac output (CO) has been the subject of extensive research but has yet to be quantified adequately in children and adolescents. Patients with third-degree atrioventricular (AV) block treated with pacemakers (PMs) are ideal candidates to assess the atrial contribution to CO by repeated measurements in single-chamber pacing mode (VVIR) and dual-chamber pacing mode (DDD/VDD). Hemodynamic measurements in children are often complicated by technical restrictions, but more recently a noninvasive method involving inert gas rebreathing has become available,

QT Prolongation After His Bundle Pacing: What Is the Mechanism?

An 82-year-old woman with nonischemic cardiomyopathy underwent cardiac resynchronization therapy by the use of His bundle pacing. After the procedure, the patient had repolarization abnormality with severely prolonged QTc and anterior inferior T-wave inversions, likely resulting from memory T waves associated with the correction of long-standing left bundle branch block. These changes could be potentially arrhythmogenic. (Level of Difficulty: Intermediate.).

DDD pacemaker for severe heart failure-alternate to CRT.

Patients with severe systolic Heart Failure continue to have poor quality of life and increased mortality in spite of optimal medical management. Cardiac Resynchronization Therapy [CRT] is promising modality in patients with systolic heart failure and electrocardiographic [ECG] evidence of left bundle branch block [LBBB]. Cost issues continue to elude many deserving cases of this therapy in our society. Relatively cost effective Dual chamber pacing [DDD] with right atrial and isolated left ventricular pacing [RA-LV] can be a good alternative.

Sleep in congestive heart failure.

Breathing disorders during sleep are common in congestive heart failure (CHF). Sleep-disordered breathing (SDB) in CHF can be broadly classified as 2 types: central sleep apnea with Cheyne-Stokes breathing, and obstructive sleep apnea. Prevalence of SDB ranges from 47% to 76% in systolic CHF. Treatment of SDB in CHF may include optimization of CHF treatment, positive airway pressure therapy, and other measures such as theophylline, acetazolamide, and cardiac resynchronization therapy. Periodic limb movements are also common in CHF.

What can happen during coronary sinus lead implantation: dislocation, perforation and other catastrophes.

Cardiac resynchronization therapy (CRT) using coronary sinus (CS) leads is an established method for the therapy of congestive heart failure (CHF) in the case of asynchronous ventricular contractions. Successful therapy depends on the placement of left ventricular leads usually via the coronary sinus (CS), a technically more challenging procedure than regular pacemaker implantations. Without specific precautions CRT implantation can be the gateway to a time consuming nightmare. Therefore CS lead implantation methods, with a focus on complications, were reviewed according to the literature and …

Last ditch CRT: right is not always wrong!

Cardiac resynchronization therapy (CRT) has become the mainstay of refractory heart failure treatment. Usually the patients having right bundle branch block (RBBB) on electrocardiogram are considered as relative contraindication as there is no or minimal dyssynchrony in them. We present a case where CRT was put in as last resort in a patient with RBBB and was successful. Short literature review is done on the CRT implantation in patients with RBBB.

A conversion CRT strategy combined with AVJA may be a perspective alternative for heart failure patients with persistent atrial fibrillation.

Heart failure (HF) combined with persistent atrial fibrillation (AF) often coexist and may promote the pathological conditions of cardiac dysfunction, leading to poor prognosis. Cardiac resynchronization therapy (CRT) combined with atrioventricular junction ablation (AVJA) is a highly effective treatment for HF patients with underlying AF who either have failed or are not suitable for catheter ablation. The CRT-AVJA combination therapy can improve clinical outcomes in HF patients. Currently, clinical CRT methods are categorized into biventricular pacing (BVP) - based and conduction system paci…

A large inherent delay between the ECG and EGM signals in the pacing system analyzer from Medtronic makes it unsuitable to estimate timing events during CRT implantation.

BACKGROUND: Following an observation of pacing markers being placed after the beginning of the paced QRS complex when using the analyzer module 2290 of the Medtronic 2090 programmer, we decided to determine the delay between the EGM and the ECG channels and comparing it to the St Jude Merlin® programmer. METHODS: We obtained ECG lead I simultaneously from both the ECG cable and by attaching the atrial clamps of the EGM cable on the left and right shoulder. We were then able to directly measure any delay. Then we compared measurements of start QRS to LV sense (qLV) in patients implanted with C

The QR-max index, a novel electrocardiographic index for the determination of left ventricular conduction delay and selection of cardiac resynchronization in patients with non-left bundle branch block.

Non-left bundle branch block (non-LBBB) remains an uncertain indication for cardiac resynchronization therapy (CRT). Non-LBBB includes right bundle branch block (RBBB) and non-specific LV conduction delay (NSCD), two different electrocardiogram (ECG) patterns which are not generally considered to be associated with LV conduction delay as judged by the invasive assessment of the Q-LV interval. We evaluated whether a novel ECG interval (QR-max index) correlated with the degree of LV conduction delay regardless of the type of non-LBBB ECG pattern, and could, therefore, predict CRT response. In 17…
