Traumatic Bilateral Asymmetrical Hip Dislocation with Acetabular Fracture: A Case Report and Review of Current Evidence.

Bilateral asymmetrical hip dislocations are rare, occurring in only 0.01-0.02% of all joint dislocations, typically following high-energy trauma. We present a 22-year-old male involved in a high-speed motor vehicle collision, sustaining a right posterior hip dislocation with an associated posterior wall acetabular fracture and a left obturator-type anterior dislocation. He underwent successful closed reduction within two hours post-injury, but due to persistent instability of the posterior acetabular wall fracture, open reduction and internal fixation (ORIF) via a Kocher-Langenbeck approach wa

Talar neck fractures.

Talar neck fractures represent a serious injury, and a review of the literature reveals the controversies surrounding the treatment options. In spite of the differences, there are many aspects of management where little disagreement exists. Anatomic reduction is the goal in situations where a primary salvage procedure is not performed. If closed treatment is chosen, careful follow-up is necessary to prevent unrecognized displacement as swelling subsides in the cast. Weight-bearing should be delayed until radiographic signs of fracture healing are obvious. There is a growing tendency toward ope

Evaluation and treatment of ankle fractures.

The ability to classify ankle fractures allows one to determine which fractures will probably do well with nonoperative treatment and which fractures will fare best with open reduction because of their inherent instability. An understanding of the Lauge-Hansen system also allows one to predict the degree of ligamentous injury on the basis of the osseous pattern of the injury. Operative management of ankle fractures requires a thorough understanding of ASIF technique. Open reduction is best performed with a fracture that is not anatomically reducible or with a fracture type that has been histor

Surgical management of fractures of the acetabulum: the Sheffield experience 1976-1994.

Of 56 acetabular fractures treated in Sheffield between 1976 and 1994, 43 fractures in 40 patients underwent open reduction and internal fixation. This paper reviews the surgically managed fractures with emphasis on the quality of operative reduction and outcome, in particular the development of degenerative osteoarthritis leading to total hip replacement. A good clinical result following operative management was seen to correlate closely with a near perfect reduction; in contrast, all cases with a poor reduction underwent joint replacement in the follow-up period. The relatively few cases man

Intramedullary osteosynthesis with Grosse-Kempf nailing for the treatment of distal fractures of the femur.

The authors report the results they obtained in a homogeneous series of fractures of the distal third of the femur treated by Grosse-Kempf nailing. Of the 67 fractures (58 closed, 9 exposed) treated with closed surgery (except for 1 case treated by nailing 35 days after trauma) non-union was never observed. The two cases of delay in consolidation (one related to an infection) did not require a change in method. Bone grafting was never necessary. Only in 3 cases (2 with multiple trauma, 1 treated by open reduction) was moderate reduction in range of movement of the knee observed.

Open vs Closed Management of Condylar Fracture Our Experience of 100 Cases in a Suburban Tertiary Care Hospital.

STUDY DESIGN: Retrospective Observational Study. OBJECTIVE: Mandibular condyle fractures are distinctive among maxillofacial injuries in which they disrupt mandibular function in a way that other traumatic injuries do not. Condylar fractures can be treated using either the conservative (closed reduction and immobilisation) or surgical (open reduction and internal fixation) approaches. Both of these modalities of treatment have advantages and disadvantages, as well as indications and contraindications. The purpose of this study is to compile and compare our experience in the management of cond

Treatment for Acute Proximal Interphalangeal Joint Fractures and Fracture-Dislocations: A Systematic Review of the Literature.

Proximal interphalangeal joint (PIPJ) fractures and fracture-dislocations are common hand injuries and recognition of this injury pattern is essential in the management of these fractures. Although a variety of treatment options have been reported in the literature, the optimal treatment remains controversial. MEDLINE, EMBASE, and The Cochrane Library Database were screened for treatment strategies of PIPJ fracture and fracture-dislocation. Demographic data and outcome data were collected and recorded. A total of 37 studies including 471 patients and 480 fingers were reviewed. PIPJ range of mo

[Volkmann's contracture and suprcondylar fractures of the humerus in children].

The most frequent cause of the compartment syndrome with subsequent Volkmann's contracture of the upper extremity in children is a supracondylar fracture of the humerus. Often it is being associated by some with the concurrent injury and obliteration of the brachial artery. In the majority of cases of Volkmann's contracture in children with dislocated supracondylar fractures of the humerus, described in the available literature, the development of Volkmann's contracture was preceded by reduction of the fracture and fixation with bandage or in cast with the elbow in flexion. The aim of this stu

Trans-scaphoid perilunate fracture-dislocation and isolated perilunate dislocations; surgical versus non surgical treatment.

BACKGROUND: Trans-scaphoid perilunate fracture-dislocation and perilunate dislocations are among uncommon injuries, most commonly seen in young patients due to high energy trauma. The treatment can be achieved either surgically by open reduction and internal fixation or closed reduction and casting. METHODS: To compare surgical versus non-operative results of treatment after trans-scaphoid perilunate fracture-dislocation and isolated perilunate dislocation, we collected the data of 34 patients who were treated at least 5 years before our study, twenty of whom were treated surgically and fourt

Deformity after fracture of the lateral humeral condyle in children.

We studied 28 displaced (Jacob types II and III) fractures of the lateral humeral condyle in 28 children. There were only two Milch type I fractures. Twenty-one fractures were treated by open reduction and internal fixation with K-wires, Palmer nails, or sutures. Seven patients were treated with a plaster cast, five following closed reduction, and the primary displaced position being accepted in two. Anatomic reduction was obtained in 18 patients. All but one fracture were united at review 2 years to 16 years later. The distal humerus was wider on the injured side in all patients. Six patients

Non-union of the great toe in a 4-year-old child.

Phalangeal fractures of the foot are very rare in children. They are treated with closed reduction and splinting. Cases reported of non-union in children are rare. We report on treatment of a 4-year-old child with non-union of the proximal phalanx of the great toe foot following an open fracture caused by a motor vehicle accident 4 months prior. No graft was used. We present this case with good clinical outcome at 1 year follow-up.

Unusual traumatic anterior dislocation of the hip.

Four interesting cases of traumatic anterior dislocation of the hip are presented. All were easily reduced by closed manipulation, including one complicated by a subtrochanteric fracture of the same femur. Early reduction and mobilization have resulted in good early results, though the risk of late change in the shape of the femoral head still exists in one of the cases. Another case confirmed that an abduction force on the flexed hip can produce anterior dislocation of the hip.

External fixation of the pelvis.

During the past decade the useful classification and principles of management of the fractured pelvis have progressed rapidly. For the acute management the principal consideration is the control of severe intrapelvic hemorrhage. A protocol has been developed that combines the early application of external fixation with a closed reduction of the pelvic fracture. A marked decrease in the anticipated amount of intrapelvic hemorrhage has been documented. The coupled double anterior frame and its biomechanical equivalent, the newly developed triangular system, are easily applied to the pelvic ring

Displaced Distal Tibia Physeal Fractures: Short Leg Versus Long Leg Casting-A Prospective Study.

BACKGROUND: The optimal treatment of distal tibia physeal fractures in pediatric patients remains controversial. Traditionally, after a closed reduction is performed, a long leg cast (LLC) is used to provide rotational control and prevent loss of reduction. Recent retrospective data suggest that short leg casts (SLC) may be equally effective. This study compares outcomes of SLC versus LLC in treating these fractures in pediatric patients. METHODS: This prospective study enrolled patients from a pediatric level I trauma center with displaced distal tibia physeal fractures requiring reduction.

Successful Non-fusion Management of a Healed Pediatric Odontoid Synchondrosis Fracture: A Case Report.

Odontoid synchondrosis fractures are rare pediatric spinal injuries that pose significant diagnostic and treatment challenges. These fractures, typically occurring in the C-2 vertebra, often result from high-energy trauma and are difficult to diagnose due to subtle radiographic findings. We present the case of a 4-year-old male who sustained a head injury while playing on a trampoline. Initially diagnosed with a head injury and discharged without noting any neck complaints, the patient returned to the ER two months later with neck pain and stiffness. Radiographs revealed a C2 fracture at the b

Minimally displaced acetabulum fractures in geriatric patients: a comparison of open, percutaneous and non-operative treatment from the German Pelvic Injury Register data.

PURPOSE: In elderly patients with minimally displaced acetabulum fractures, the patients' inability to partially weight-bear and the need for early mobilisation may trigger the decision towards a treatment with higher primary stability. The purpose of this study was to compare open reduction and internal fixation (ORIF), closed reduction and percutaneous fixation (CRPIF) and non-operative treatment in geriatric minimally displaced acetabulum fractures with regard to complications and quality of reduction. METHODS: Data from the prospective German Pelvic Injury Register collected between 2008
