Standardized exercise training is feasible, safe, and effective in pulmonary arterial and chronic thromboembolic pulmonary hypertension: results from a large European multicentre randomized controlled trial.

AIMS: This prospective, randomized, controlled, multicentre study aimed to evaluate efficacy and safety of exercise training in patients with pulmonary arterial (PAH) and chronic thromboembolic pulmonary hypertension (CTEPH). METHODS AND RESULTS: For the first time a specialized PAH/CTEPH rehabilitation programme was implemented in 11 centres across 10 European countries. Out of 129 enrolled patients, 116 patients (58 vs. 58 randomized into a training or usual care control group) on disease-targeted medication completed the study [85 female; mean age 53.6 ± 12.5 years; mean pulmonary arterial

Minimal versus specialist equipment in the delivery of pulmonary rehabilitation: protocol for a non-inferiority randomised controlled trial.

INTRODUCTION: Pulmonary rehabilitation (PR), an exercise and education programme for people with chronic lung disease, aims to improve exercise capacity, breathlessness and quality of life. Most evidence to support PR is from trials that use specialist exercise equipment, for example, treadmills (PR-gym). However, a significant proportion of programmes do not have access to specialist equipment with training completed with minimal exercise equipment (PR-min). There is a paucity of robust literature examining the efficacy of supervised, centre-based PR-min. We aim to determine whether an 8-week

Exercise Capacity in Young Adults Born Small for Gestational Age.

IMPORTANCE: Being born small for gestational age (SGA), approximately 10% of all births, is associated with increased risk of cardiovascular mortality in adulthood, but mechanistic pathways are unclear. Cardiac remodeling and dysfunction occur in fetuses SGA and children born SGA, but it is uncertain whether and how these changes persist into adulthood. OBJECTIVE: To evaluate baseline cardiac function and structure and exercise capacity in young adults born SGA. DESIGN, SETTING, AND PARTICIPANTS: This cohort study conducted from January 2015 to January 2018 assessed a perinatal cohort born a

Influence of left ventricular assist device pressure-flow characteristic on exercise physiology: Assessment with a verified numerical model.

Current left ventricular assist devices are designed to reestablish patient's hemodynamics at rest but they lack the suitability to sustain the heart adequately during physical exercise. Aim of this work is to assess the performance during exercise of a left ventricular assist device with flatter pump pressure-flow characteristic and increased pressure sensitivity (left ventricular assist device 1) and to compare it to the performance of a left ventricular assist device with a steeper characteristic (left ventricular assist device 2). The two left ventricular assist devices were tested at cons

Exercise-based cardiac rehabilitation for adults with atrial fibrillation.

BACKGROUND: Exercise-based cardiac rehabilitation may benefit adults with atrial fibrillation or those who had been treated for atrial fibrillation. Atrial fibrillation is caused by multiple micro re-entry circuits within the atrial tissue, which result in chaotic rapid activity in the atria. OBJECTIVES: To assess the benefits and harms of exercise-based rehabilitation programmes, alone or with another intervention, compared with no-exercise training controls in adults who currently have AF, or have been treated for AF. SEARCH METHODS: We searched the following electronic databases; CENTRAL

[The importance of evaluating the effectiveness of the ventilation VE/VCO2 slope in patients with heart failure].

Chronic Heart Failure (CHF) is accompanied by a whole range of symptoms, which significantly decrease the quality of life; these typically concern dyspnoea, fatigue and exercise intolerance. The objectification of the patients condition but must have an effective means of evaluation. One such resource is cardiopolmonary exercise (CPX) testing with it is functional parameters. CPX test with its functional parameters is one of such means. VO2peak is undoubtebly the parameter, which was used the most often in the past 25 years, however, it has many limitations. It is reason, why the evaluation of

Myoadenylate deaminase deficiency: a frequent cause of muscle pain A case detected by exercise testing.

Myoadenylate deaminase deficit (MAD, MIM#615511) is the most common cause of metabolic myopathies with an estimated prevalence of 1-2% in the general population. We report the case of a 39-year-old man suffering from severe skeletal muscle pain that had developed gradually for 4 years. A moderate increase in creatine kinase (CK) was the only biological sign observed. This study takes a closer look at a common but poorly known pathology and highlights the interest of the dynamic metabolic investigations carried out during exercise stress test with a cycle ergometer. Our non-invasive clinical an

Separate and combined effects of exposure to heat stress and mental fatigue on endurance exercise capacity in the heat.

PURPOSE: This study investigated the effects of exposure to pre-exercise heat stress and mental fatigue on endurance exercise capacity in a hot environment. METHODS: Eight volunteers completed four cycle exercise trials at 80% maximum oxygen uptake until exhaustion in an environmental chamber maintained at 30 °C and 50% relative humidity. The four trials required them to complete a 90 min pre-exercise routine of either a seated rest (CON), a prolonged demanding cognitive task to induce mental fatigue (MF), warm water immersion at 40 °C during the last 30 min to induce increasing core temperat

Dyspnoea: a multidimensional and multidisciplinary approach.

Dyspnoea is a debilitating symptom that affects quality of life, exercise tolerance and mortality in various disease conditions/states. In patients with chronic obstructive pulmonary disease (COPD), it has been shown to be a better predictor of mortality than forced expiratory volume in 1 s. In patients with heart disease it is a better predictor of mortality than angina. Dyspnoea is also associated with decreased functional status and worse psychological health in older individuals living at home. It also contributes to the low adherence to exercise training programmes in sedentary adults and…

Physical fitness and exercise training on individuals with spina bifida: a systematic review.

Spina Bifida (SB) is characterized by several physical impairments; however, data on physical fitness and on the benefits of exercise training in individuals with SB are dispersed in the literature. Thus, this systematic review aimed to describe (i) physical fitness components (cardiorespiratory endurance, muscle strength, body composition, flexibility and neuromotor) and (ii) exercise training effects on the physical fitness of individuals with SB. CINAHL, MEDLINE and EMBASE were searched from January to March 2013 and updated in December 2013. Twenty-three studies were included. A summary of

Correlation of subjective questionnaires with cardiac function as determined by exercise testing in a pediatric population.

BACKGROUND: Although exercise testing is an important objective method used to assess cardiopulmonary function, subjective assessment often is used as a proxy in the clinical setting. This study aimed to determine whether responses to a subjective functional capacity questionnaire administered to parents and patients in a pediatric exercise laboratory correlate with objective assessment of functional capacity, measured by peak oxygen consumption during maximal voluntary exercise testing. METHODS: Subjective questionnaire responses and exercise test results collected over 10 years were retrosp

Origin of symptoms in chronic heart failure.

Skeletal muscle abnormalities are highly prevalent in chronic heart failure and are associated with an increase in the ergoreflex, a muscle reflex stimulated by work done. Stimulation of the ergoreflex results in increased ventilation and contributes to the increased sympathetic activation of the heart failure syndrome. The origin of the skeletal myopathy is related to a chronic imbalance between catabolic and anabolic processes, presumably as a consequence of chronic haemodynamic stress. Symptoms arise from the skeletal myopathy, causing the sensation of fatigue and contributing to the sensat

Factors affecting the exercise capacity of pediatric patients with aortic regurgitation.

Although exercise testing is commonly employed to identify adult aortic regurgitation (AR) patients with early left ventricular (LV) dysfunction, the role and value of exercise testing in the management of pediatric AR patients have not been established. The purposes of this study were to evaluate the cardiorespiratory response to exercise of pediatric patients with chronic AR, examine the relation between exercise function and baseline echocardiographic measurements, and identify factors related to diminished exercise capacity (EC). The study group consisted of 26 patients aged 8 to 21 years

Fluctuation of physical function during chimeric antigen receptor T-cell therapy during rehabilitation intervention: Real-world data and risk factor analyses.

INTRODUCTION: Patients undergoing chimeric antigen receptor (CAR) T-cell therapy face prolonged treatment timelines and are prone to cytokine release syndrome (CRS) and immune effector cell-associated neurotoxicity syndrome (ICANS) after infusion. Disabilities in physical function and the importance of rehabilitation during CAR-T-cell therapy to maintain physical function have been poorly documented. METHOD: We performed a retrospective cohort study to assess changes in exercise tolerance via differences in a 6-min-walking distance (Δ6MWD) and factors influencing it. RESULTS: A total of 77 p…
