The Acutrak plus compression screw (APCS) (Acumed Inc., Beaverton, OR, USA) is an intramedullary implant which can achieve stable fixation with minimum soft tissue dissection. A 55-year-old fisherman sustained an unusual and irreducible fracture-dislocation of the ankle joint. An entrapped medial malleolar fragment was discovered by computed tomography between the distal tibia and lateral malleolus. The fragment was removed during an emergent open reduction, during which deltoid ligament repair and internal fixation of the bimalleolar fracture were undertaken. At the 18-month follow-up visit, the patient was working normally and was asymptomatic. At previous visits, he demonstrated a full range of ankle motion and had normal radiograph.

A prospective study of 236 patients with neglected femoral neck fractures was done to determine the usefulness of a preoperative classification based on the radiologic changes that occur at the site of the fracture. Thirty-four patients were observed in Group I, 125 patients were observed in Group II, and nine patients were observed in Group III. Ninety-four conservatively treated patients with Lauge Hansen Stage II supination-eversion fractures of the ankle were interviewed after 16-25 years. Patients with pain were examined clinically and radiographically. Eighty-nine patients had good and five medium results. Our observations compare well with published reports of open treatment. We conclude that this particular fracture type is so benign that it can be treated closed without reduction.

Thirty-three cases of recalcitrant osteomyelitis treated by free tissue transfer are reviewed. The immediate success rate was 76 per cent; however, on follow-up for at least 1 year, there was a 12 per cent rate of recurrent drainage. The authors conclude that this is a good operation for this problem but that it is not a panacea because if followed long enough, a significant number of patients will develop recurrent drainage.
