[Repair of degloving injury of fingertip with vascular pedicled cross finger flap].

OBJECTIVE: To investigate the effectiveness of cross finger flap pedicled with the dorsal branch of proper digital artery in repairing degloving injury of the fingertip. METHODS: Between January 2010 and June 2012, 13 cases of degloving injury of single fingertip were treated, including 9 males and 4 females with an average age of 33.6 years (range, 17-46 years). The defect locations were index finger in 5 cases, middle finger in 3 cases, ring finger in 3 cases, and little finger in 2 cases, including 4 cases of mechanical injury, 6 cases of twist injury, and 3 cases of crushing injury. The e

Surgical Management of Proximal Interphalangeal Joint Fracture-Dislocations: A Review of Outcomes.

Background: Treatment of proximal interphalangeal joint (PIPJ) fracture-dislocations is difficult given the potential long-term complications of the involved finger and entire hand. Several surgical methods have been utilized for management of these injuries, none of which have shown consistently favorable results. The purpose of this systematic review of the literature is to report the post-operative outcomes of multiple treatment modalities for PIPJ fracture-dislocations in various studies. Methods: A literature review of PubMed and EMBASE databases was performed for all articles on PIPJ fra

Congenital absence of the juncturae tendini contributing to dislocation of the extensor tendons.

We report a patient with bilateral congenital absence of the juncturae tendini who developed dislocation of the extensor tendons of the long and ring fingers of both hands. Reconstruction of the juncturae tendini restored extensor function. We describe our technique and the results 9 months after surgery.

A rare combined fracture and ligamentous injury of the thumb.

Bennett's fracture of the base of the metacarpal of the thumb and avulsion of the ulnar collateral ligament of the thumb are both relatively common injuries. It is, however, rare for the two lesions to occur simultaneously and we report such a case.

Osteoarthritis of the hip and Heberden's nodes.

One hundred consecutive patients with proved osteoarthritis were assessed for the presence of terminal interphalangeal joint disease. There was a significant association found between the presence of Heberden's nodes and primary (axillary) arthritis. Secondary osteoarthritis was relatively free of nodal involvement. It is suggested that Heberden's nodes are a helpful clinical marker in differentiating between the two major groups of osteoarthritis.

Intra-articular fractures of the basilar joint of the thumb.

This article on intra-articular fractures of the basilar joint of the thumb discusses the biomechanical and treatment complexities of fractures involving the thumb carpometacarpal joint. This review focuses on the treatment of thumb carpometacarpal fractures and dislocations, consolidating current philosophy and rationale regarding operative and nonoperative treatment. Treatment recommendations are based upon principles established in previous clinical and biomechanical studies emphasizing fracture-specific modalities. Emphasis is placed on maintenance of articular congruity, fracture stabilit

[Dorsal Dupuytren's disease].

Two different manifestations of the disease are discussed: hyperextension of the distal interphalangeal joint through mechanical fibrous contracture or through pathological involvement of the retinacular ligaments. Surgical correction is performed by transection of the contracted structures but this manifestation tends to be found in severe disease and thus to presage a poor prognosis. Dorsal localisations of the disease itself in the form of (1) knuckle pads when localized on the dorsal aspect of the PIP joint where they do not impair flexion, or (2) distal nodules situated dorsal to the exte

Volar dislocation of the metacarpophalangeal joint. Pathologic anatomy and report of two cases.

Of two cases of volar metacarpophalangeal joint dislocation, the thumb was affected in one. This is the first case of its kind reported in the English-language literature. Patients with this injury are older than patients with the more common dorsal dislocation. The mechanism of injury seems to be direct trauma to the dorsum of the hand with the metacarpophalangeal joint acutely flexed. The pathologic anatomy may be a combination of a dorsal capsular tear, a tear of one or both collateral ligaments, and a tear of the volar plate with interposition in the joint. Attempt at closed reduction, eve

[Onychoheterotopia with polyonychia associated with Pierre Robin syndrome: apropos of a new case. (Trial of classifying nail ectopia)].

The authors report the second case in the English language literature of a rare abnormality of the fingers and nails associated with Pierre Robin syndrome. The patient was born at term to non-consanguineous parents. The only potentially toxic drug taken by the mother during the first months of pregnancy was Sympathyl, a combined preparation of phenobarbitone, methenamine and plant extracts (boldo, crataegus) used as sedative. The child presented at birth with typical Pierre Robin syndrome responsible for respiratory distress, associated with hypotonia, dysplasia of the right hip and cardiac mu

Multiple unusual complications after extensive chronic sarcoid tenosynovitis of the hand: a case report.

Sarcoidosis is an idiopathic disease characterized by multiple noncaseating granulomatous nodules. Involvement of the hand is extremely uncommon and tenosynovitis of the hand is rare. We present a patient who developed sarcoid tenosynovitis of multiple digits involving both the flexor and extensor tendons. He also developed flexor tendon rupture, subchondral bone erosions, and multiple joint subluxations.

Heberden oration 1985. The rheology of joints.

The value of rheological studies of joint tissues has been illustrated by studies from our department. These have included studies of the stiffness of joints which suggest that subjective stiffness is more likely to be related to limitation of movement of a joint than to increased physical stiffness (either elastic or dissipative torques). A review of goniometry suggests that the diminution of joint movement with advancing age varies with the frequency of use of the joint. A simple goniometer for the hip has been described, and in contrast a sophisticated telemeterized system has been devised.

The sesamoids of the metacarpo-phalangeal joint of the thumb: an anatomical and clinical study.

Two unusual cases of injury to the sesamoids of the M.P. joint of the thumb are described. An anatomical dissection of the M.P. volar plate region, including the sesamoids, was undertaken to delineate the details of the sesamoids' relationship to the M.P. joint, flexor tendon, flexor tendon sheath and capsule of the joint. Histological studies were carried out to demonstrate a tendinous extension of the muscles that attach to the sesamoids.

[Congenital subluxation of the metacarpophalangeal joint of the thumb as the cause of writer's syndrome].

A 26-year-old male patient developed a writer's cramp syndrome, the underlying cause of which was shown to be a congenital subluxation of the first metacarpophalangeal joint. The clinical symptoms are presented, the indications for surgery reviewed and the operative technique in accordance with A.N. Witt described. Post-operatively, the pathological hypermobility of the first metacarpophalangeal joint has been eliminated, and global function of the hand has normalized.

Primary osteoathrosis of the hip and Heberden's nodes.

One hundred patients with primary osteoarthrosis of the hip were examined for evidence of generalised arthrosis associated with Heberden's nodes, and in addition their hips were graded by the radiographic pattern of loss of joint space. Twenty-nine patients had bilateral concentric loss of joint space, and 18 in this group had Heberden's nodes. None of the 17 patients with bilateral upper pole changes had Heberden's nodes, and only 7 of the 31 patients with unilateral upper pole changes had Heberden's nodes. None of the 6 patients with medial loss of joint space had Heberden's nodes. This stud
