BACKGROUND: Diagnostic reference levels are radiation dose levels in medical radiodiagnostic practices for typical examinations for groups of standard-sized individuals for broadly defined types of equipment. This study aimed to contribute to national diagnostic reference levels for common hand and wrist procedures using mini C-arm fluoroscopy. Small joint and digital fracture procedure diagnostic reference levels have not been reported in significant numbers previously with procedure-level stratification.
METHODS: Data were collected from fluoroscopy logbooks and were cross-referenced agains

Mallet finger describes a fingertip deformity where the distal interphalangeal joint (DIPJ) of the affected digit is held in flexion, unable to extend the distal phalanx actively. The deformity is typically a consequence of traumatic disruption to the terminal extensor tendon at its insertion at the proximal portion of the distal phalanx or slightly proximally at the level of the DIPJ. Patients typically present with a history describing the event of injury with a typical mallet deformity. Common mechanisms include sport activities causing a direct blow to the finger, low energy trauma while p

Awareness of the existence of accessory muscles in the hand, such as the extensor digitorum brevis manus (EDBM) muscle, is important when making a differential diagnosis and considering the treatment of mass lesions with similar appearances. Cases of EDBM with associated dorsal wrist ganglion have been reported in earlier published reports. This report describes an unusual case of the EDBM muscle in association with carpometacarpal (CMC) boss. To the best of the authors' knowledge this has not been previously reported.

We used distraction osteogenesis to correct six deformed thumbs in four patients ranging in age from 4 to 7 years. Two of the patients had Apert syndrome (syndromic craniosynostosis with symmetrical syndactyly) and two had polydactyly. We used a small fixator with a ball joint and successfully corrected the angular deformity after lengthening the proximal phalanx by distraction. This single inclusive procedure was extremely useful. We found the optimal distraction regimen for the digital phalanx was a one day waiting period and lengthening at 1 mm/day. The mean healing indexes were 37.2 days/c

Symphalangism is a rare condition which manifests in either PIP or DIP joint congenital fusion. Symphalangism may be with some other skeletal deformities. In our case, all PIP joints of both hands were fused with bilateral hypoplasia of carpal bones and Minaar type III lunatotriquetral coalition congenitally. No motion was detected in PIP joints with absence of cutaneous creases over all PIP joints. Radiologically, carpal hypoplasia and lunatotriquetral coalition were seen in all fingers with the absence of PIP joints. The patient's skeletal survey revealed no other pathology. He had no compla

Patients treated for total radial aplasia have been reviewed. It was possible to re-align the carpus by radialization in five out of six limbs treated with pre-operative distraction, but in only one out of six treated without distraction. The average improvement in radial angulation in the non-distraction group was 19 degrees and in the distraction group 38 degrees. The average improvement in radial translation was 16 mm and 17 mm in the two groups respectively. Pre-operative distraction with the Kessler device permits re-alignment of the hand without skeletal resection or excessive tension on

The management of fractures from the industrial setting is very similar to treatment of fractures that occur elsewhere. However, the surgeon must constantly be on guard for any associated injuries that may be unique to the workplace, such as exposure to heat, cold, chemicals, or electricity. An accurate medical history of the patient and etiology of the injury are necessary. The overall emphasis must be a return to gainful employment. Treatment should be managed from the beginning. The initial action should be an aggressive approach, whether management is conservative or surgical, with a minim

Injuries to the hand are among the most common in all of sports. Appropriate care should include prompt diagnosis and treatment. Dislocations of the digits should be reduced promptly, particularly the carpometacarpal joint of the thumb. Volar dislocations of the proximal interphalangeal joint need to be splinted in full extension, and the more common dorsal dislocations in slight flexion. Collateral ligament injuries of the fingers respond well to initial immobilisation followed by early motion. Although ligament injuries to the thumb metacarpophalangeal joint may be treated closed, they gener

The hands and wrists of 22 karate instructors who had practiced the sport for a minimum of five years were reviewed. Radiological evidence of a total of ten fractures was found. There was no evidence that practice of the sport predisposed to the early onset of chronic tenosynovitis or osteoarthritis.

The article describes a sequestrated portion of the carpal scaphoid bone, which is interpreted as a marginal fragmentation by partial necrosis of the scaphoid bone. In the authors' opinion, earlier writings have not proved incontestably the occurence of a genuine osteochondritis dissecans of the scaphoid bone.

The surgical approach for treatment of scaphoid nonunion usually involves various combinations of screws and Kirschner wires, with or without bone grafts. This article reports our results with 36 dorsal-approach bone grafting procedures for treatment of scaphoid nonunion with Kirschner wires. Union was achieved in 89%. The average follow-up was 5 years. Flexion/extension averaged 76% of the opposite wrist, and grip strength was 88% of the opposite hand. Ninety-one percent of the employed patients returned to their original jobs. The dorsal approach provides satisfactory exposure for fragment r

A new pattern of osteolysis accompanied by detritic synovitis occurred in a 71-year-old woman. The patient had a severe, destructive, and multilating arthropathy of her hands, with distal phalangeal deformity. Radiographs outlined bone resorption of the appendicular and axial skeleton, with involvement of phalanges, metacarpals, metatarsals, clavicles, long bones, and spine. There was no notable occupational history nor laboratory evidence of hyperparathyroidism. Pathological evaluation revealed synovial ulceration and necrosis, and necrotic bone embedded with fibrotic synovium. This patient's

BACKGROUND: Giant cell tumors (GCTs) of the carpal bones are rare. GCTs of the hand tend to be aggressive, could result in multifocal presentation, and have a high rate of recurrence.
CASE DESCRIPTION: We report a rare case of GCT of the capitate with secondary aneurysmal bone cyst (ABC) in a 40-year-old man. The patient presented with complaints of pain and swelling over his right wrist since 6 months. Radiographs showed the presence of a lytic lesion in the capitate. Magnetic resonance imaging scanning revealed a well-defined, lobulated, multiseptate lytic lesion involving the entire capita
